About the Workforce Development Directorate SEMPHN will lead substantial reform in the primary health care arena over the coming months and years. Providing understanding, engagement and partnership in this change will be essential for success. The Workforce Development team will work proactively with the provider community to facilitate understanding, engagement and partnership in this domain. The use of technology and the role of Digital Health will be an important element of this work. New models of support will be developed which take into account our digital strategies and how to best facilitate an integration of this work into this dynamic environment. My Health Record A My Health Record (MHR) is a secure online summary of a person s health information. The person controls what goes into it, and who is allowed to access it. It enables healthcare information to be shared with patients, doctors, hospitals, and other healthcare providers. SEMPHN is supporting the Australian Digital Health Agency to implement a change that means all Australians will be automatically enrolled in the My Health Record (MHR), with the option to opt out if they choose. Job Summary The Team Leader My Health Record is responsible for managing the team who are preparing primary care clinicians to understand the change and confidently explain it if they are asked by their patients. This role: Coordinates the development and implementation of a multi-pronged change management program. Manages the My Health Record team. Acts as one of SEMPHN s front line staff; communicating: o And facilitating change regarding MHR, its purpose, the changes to opt-out, and developing the skills and knowledge to support patients to make an informed choice. o SEMPHN s purpose, activities and connecting providers to our programs and services. Builds strong relationships with general practices, pharmacies, and other providers of primary healthcare to support the MHR. Actively helps build the capacity, skills and knowledge of primary health care providers. Plans, coordinates, implements and supports the activities of the project. Engages and collaborates with staff from the Australian Digital Health Agency on designing effective resources and strategies for the change program. The program of work for the Team Leader My Health Record includes: Coordinate the planning and development of the SEMPHN MHR project and its rollout. Identify, monitor, and address barriers and promote enablers to the awareness, understanding, and adoption of MHR uptake amongst providers and the community/patients. Visit primary health providers, in particular general practitioners and pharmacies, to inform and educate about the MHR and the change to opt-out. Revision No: Date: October 2017 Page 2 of 5
Engage with community providers, groups and other stakeholders to inform and educate them about the MHR and the change to opt-out. Work with SEMPHN teams, in particular Strategic Relations, Service Innovation and Workforce Development to ensure support for the MHR is embedded in planning and activity. Follow good record keeping practices to ensure progress can be monitored and opportunities for improvement identified. The Team Leader, My Health Record will bring highly developed stakeholder engagement skills, well developed project coordination knowledge, an understanding of change management, advanced written and verbal communication skills, facilitation skills, and an understanding of the primary health care system. Key Responsibilities Leadership Effectively manage and lead identified projects and teams. Manage the priorities and work plans of the identified projects and teams. Ensure probation and regular performance based conversations with all staff are undertaken. Champion the building of positive relationships with stakeholders and providers. Stakeholder Engagement Establish strong and effective relationships with general practices, service providers and other key stakeholders. Work with and support general practices and other primary health care providers so they understand the MHR, and can knowledgeably respond to enquiries from patients and clients. Respond to and analyse feedback from general practices and other primary health care providers. Talk at events as required. Project Coordination Take part in all aspects of project delivery. Ensure targets are established, monitored and reported. Build positive relationships with internal and external stakeholders. Develop and ensure that effective and efficient program systems and process are in place to support high quality delivery of services. Change Management Support and advocate change management initiatives for providers so that they can build their capability. Key Selection Criteria Qualifications Tertiary qualifications in health, social sciences and/or extensive relevant experience. Skills, Knowledge and Experience Proven ability to influence positive change to general practices and primary health care providers at the micro level. Highly developed communication, presentation, and interpersonal skills including ability to build influential and productive working relationships with a range of stakeholders. A good knowledge of contemporary issues for general practices and the primary health sector. Demonstrated experience in managing a small, mobile and geographically dispersed team. Demonstrated project management skills in: o planning o implementation o monitoring o report writing Sound experience with written documentation such as reports, emails and other administrative functions. Ability to multi-task, set priorities and meet strict deadlines. Intermediate to advanced skills in Microsoft office. Other A demonstrable commitment to SEMPHN organisational values. A National Police Check and Working with Children Check may be required in accordance with government funding requirements.
