Patient Safety Professional Program: Pre-work Credit Types: AMA, ACPE, ANCC, ADA, and IACET Jointly provided by Postgraduate Institute for Medicine and Defense Health Agency Patient Safety Program Activity Information The goal of the Patient Safety Professional Program: Pre-work is to provide entry-level Patient Safety Managers (PSMs) with the competencies needed for their first year on the job. General topic areas include Patient Safety, Quality, Process Improvement, Change Management, and Risk Management. The Patient Safety Professional Program: Pre-work covers a wide range of topics structured around four main topic areas: Systems thinking, human factors, and high reliability principles and strategies Basic principles of performance improvement and measurement Identification, mitigation, and reporting of risk Leadership and change management Release date: 10-23-2017 Expiration date: 06-29-2018 Estimated time to complete activity: 6 hours To participate in this online activity, you will need the following type of hardware and software: Computer with Internet connection and Microsoft Internet Explorer version 8 or above required to run course. Course is compliant with Section 508 of the Americans with Disabilities Act (ADA) and includes text captioning for audio components. Target Audience This activity has been designed to meet the educational needs of physicians, physician assistants, pharmacists, pharmacy technicians, radiologists, dentists, registered nurses, nurse practitioners, nurses-lvn/lpn, med techs, corpsmen, medics/technicians (EKG/radiologists/labs/pharmacy), dental assistants/hygienists/technicians, health administrators, and other allied health professionals involved in the care of patients within the military health system.

Method of Participation and Request for Credit During the accreditation period, participants must read the learning objectives, faculty disclosures and study the educational activity. If you wish to receive acknowledgment for participation of this CME/CE activity, you must complete the following: Bring the completed pre-work packet with you to the Patient Safety Professional Program: Core Content class. By the second day of the Patient Safety Professional Program: Core Content class, you will receive an email with a link to the pre-work post-test and evaluation form. Complete the post-test and evaluation online within 10 days of the activity. Upon successfully completing the post-test with a score of 75% or better and the activity evaluation, your certificate will be made available via email. For Pharmacists: Upon successfully completing the post-test with a score of 75% or better and the activity evaluation form, transcript information will be sent to the NABP CPE Monitor Service within 4 weeks. AVAILABLE CME/CE CREDITS AND MANDATORY ELIGIBILITY REQUIREMENTS FOR EACH AMA PRA: Offered by the Accreditation Council for Continuing Medical Education (ACCME), the Continuing Medical Education AMA PRA Category 1 Credits is available if you are a Doctor of Medicine (MD), Doctor of Osteopathic Medicine (DO), Physician s Assistant (PA), or Nurse Practitioner (NP). You must certify at the time of registration that you are an MD, DO, PA, or NP to earn this type of CE credit. ACPE: Offered by the Accreditation Council for Pharmacy Education (ACPE), this credit is available to Pharmacists and Pharmacy Technicians. In order to earn ACPE contact hours, we are required by the National Association of Boards of Pharmacy (NABP) and ACPE to collect both your NABP number and date of birth in MM/DD format. You must enter it at the time of registration to receive credit. You CANNOT use an organization/mtf NABP number, it must be 4

YOUR number. If you do not have an NABP number, please go to https://store.nabp.net/oa_html/xxnabpibegbllogin.jsp and sign up for this six digit number. If you do not provide information, you will not earn credit. Please note, due to ACPE guidelines, all credits provided by ACPE-accredited providers to Pharmacists and Pharmacy Technicians will be available via CPE Monitor (https://store.nabp.net/oa_html/xxnabpibegbllogin.jsp). If participants would like a paper statement of credit, they may print an individual statement of credit or a transcript from their NABP e-profile. ANCC CNE: Offered by the American Nurses Credentialing Center, the Continuing Nursing Education (CNE) credit is available to Nurses and Nurse Practitioners. ADA CERP: Offered by the American Dental Association, Continuing Education Recognition Program (CERP) credit is available to Dentists and Dental Technicians. Non-Physician Medical Staff: Offered by the Accreditation Council for Continuing Medical Education (ACCME), this designates AMA PRA Category 1 Credits to medical professionals who are not physicians, including Chiropractors, Paramedics, Emergency Medical Technicians (EMT), etc. IACET CEU: Offered by the International Association for Continuing Education & Training (IACET), this non-specific Continuing Education Unit (CEU) is available to any attendee who is an allied health professional and may not be eligible for any of the other continuing education credits offered above. Please note, CEUs are calculated differently that other CE types. One CEU equals onetenth contact hour of participation where other CEs earn one unit per contact hour. 5
