US Med will no longer be the mail-order pharmacy provider for icare members. Be assured that NoviXus will offer the same great services as US Med with faster delivery times. March 2015 IMPORTANT ANNOUNCEMENTS FROM icare Attention Diabetic icare Medicare SNP and Family Care Partnership Members icare would like to provide you with the best products for the management of your diabetes, so we are excited to announce that beginning April 1, 2015 we will work with Abbott Diabetes Care (the makers of the Freestyle Meter) to provide all of your blood glucose meters and testing supplies. If you are with US Med now, prescriptions will automatically transfer to NoviXus if the prescriptions have been previously filled by US Med and have not expired. However, if you have a prescription for a controlled substance, you will need to obtain a new prescription and send it in to NoviXus. You may begin requesting prescription refills or sending new prescriptions to NoviXus on March 1, 2015. icare BADGERCARE PLUS AND SSI/MEDICAID PLAN INFORMATION Introducing icare for Mom and Baby A FREE program for pregnant icare BadgerCare Plus and SSI/Medicaid members What is icare for Mom and Baby? icare for Mom and Baby is a free program offered to BadgerCare Plus and Medicaid SSI members. The program offers prenatal services, postpartum care, access to community resources, care for your baby, and additional support to meet your needs. Prenatal Services Pregnant icare members will be connected to a specially trained nurse and care coordinator to provide support throughout the pregnancy. This may include obtaining baby supplies, scheduling OB visits, and making connections to pregnancy resources in the community. Postpartum Care After the birth of your baby, the icare for Mom and Baby Team will continue to provide you with support and education. You will be offered services to meet your post-partum needs including: A home visit with the icare Team (Milwaukee County members) Breast feeding support Education on newborn cares Scheduling your postpartum visit Scheduling well-baby visits Maternal support Birth control Screening for baby blues and postpartum depression Support for new fathers Community Resources Your icare for Mom and Baby team will help you find the services that you or your baby may need, including: Food Share, WIC, Food Pantries Prenatal care groups Breast feeding classes and support Housing Baby supplies Transportation Childbirth and parenting classes Prenatal yoga classes Enrolling is easy! Your icare Mom and Baby team will help to ensure that your baby is on track with their immunizations by providing you with a customized calendar. Additional Support Smoking Cessation and Addiction Treatment There are many benefits to quitting smoking, especially while you are pregnant. We will help you learn about quit-smoking programs in your area. You may be eligible to receive up to $80 for participating in the program. Your care team can also assist you in locating drug and alcohol treatment centers and support groups. 24/7 Toll-Free Nurse Advice Line The icare Nurse Advice Line will be there for you 24 hours-a-day/7 days-a-week. icare Member Spotlight Jonathan Heider Taking a break from swim meets with the University of Wisconsin Oshkosh swim team as well as the US Paralympic swim team, icare member Jonathan Heider, 22, transferred to UW Milwaukee to study Real Estate Development and Architecture as a third-year student. He has won over 30 gold medals for swimming, has swum with Team USA in world-wide competition, and holds national records as well as international rankings in paralympic swimming. Heider describes himself as a congenital quad amputee, but that didn t stop him from pursuing his love of sports from an early age. He has siblings who were also very sports oriented, which inspired him to take part in a variety of sports, especially swimming. He started participating in swim meets around the age of nine. He says his parents were supportive when he expressed interest in competing in swim meets. Heider sometimes uses a power chair or prosthetic legs to get around but he prefers not to use them, I get around just fine without it, but that s the thing with swimming, you aren t allowed equipment during competition, it s just you and the water, and I think it best describes me in general as having the freedom to live without it. This is the only way I ve known, and as much as the power chair and prosthetic legs are helpful, it s still nice to do my own thing without anything. Heider admits that he does face obstacles, I think society as a whole is rather challenging... As a little kid growing up you want to be accepted and have friends to play with. That was a challenge growing up, just to get kids to think that it was OK and that I can play backyard football with them and I m not going to get hurt. Another obstacle is with adults... especially now with being a college student in the hunt for jobs and internships, and always having to prove that I can step up to the plate. He explained that there are employers who do not have the patience to work with people with disabilities or are not willing to take the necessary steps to be compliant with regulatory requirements. Describing himself as a hard-driven and motivated person, he said it is hard when society looks at someone accepting disability benefits in a critical light. I think those are the main obstacles... Society is not ready to take a stand and move forward pass the judgments that they make. Heider is not currently working, but he is signed with The College Agency as a motivational speaker. He has worked as a head coordinator at a physically disabled sports camp for two summers, and he held a telemarketing job to help save money for college. He said one of the most rewarding jobs he ever held was as an unpaid assistant swim coach for the Navy/Coast Guard team of disabled veterans participating in the Wounded Warrior Games, held in Colorado Springs, Colorado.
