An estimated 13% of the overall U.S. population continues to go without health insurance and finds cost an impediment to accessing care, even more are ineligible or underinsured. Locally, Open Door is here to help people navigate their options under the Affordable Care Act and sign them up for insurance. 43% of Open Door patients do not have insurance. What is the Patient Advocacy program at Open Door? Our patients face serious issues that impact their ability to be and stay healthy and our patient advocate program is a critical component to our success. Patient Advocates are non-clinical staff who help patients navigate the health care system. What is the Behavioral Health program at Open Door? Too often, mental health and chemical dependency are under-treated and stigmatized yet for countless people, early intervention is critical. Physical and emotional health are interconnected, which is why we integrate mental health services, offered on site, within each health center.

The Open Door Foundation provides philanthropic support for critical programs and services that are not covered through government reimbursements or grants. This investment of less than 4% of our annual budget reaches 70% of our patients with cost saving and life saving interventions. How can people get involved at Open Door? Open Door s community of volunteers has grown to 500 people. We match the interest and experience of volunteers with Open Door s needs and priorities. What is the Wellness Program at Open Door? Each month, Open Door s certified Wellness Instructors offer dozens of exercise, nutrition, healthy eating for children and adults. Our Wellness program also includes an evidencebased Chronic Disease Self- Management Program and a Diabetes Prevention Program. Our nutritionists help patients create and monitor healthy eating plans, including grocery store tours and cooking classes. How many patients does Open Door serve a year? Who are Open Door s patients? Racially and economically diverse, most of our patients live in poverty and experience on daily basis the psychological and emotional problems that living without money, skills, education bring. The majority of our patients live at or below the federal poverty level, which is $24,600 for a family of four. What s at stake?
