QIN-QIO Contract Tax Dollar Stewardship $5,256,280.53 Saved in Readmissions Diabetes Self- Management 869 Medicare 1,110 Total Increased Recruitment 131% Antibiotic Stewardship Beneficiaries Impacted 61% Providers Assisted >12,000 3 Diabetes Empowerment Education Program (DEEP) TM DEEP classes teach individuals how to take charge of their health and get control of their diabetes while learning through fun and interactive activities. These classes are designed for people with Medicare, but all are welcome. They support (not replace) education from health care providers and certified diabetes educators. 4 2

Three Special Innovation Projects Super-Utilizer: Awarded in 2015 to Montana to work with community resources and wrap services around complex care patients. Project ECHO: Awarded in 2016 to Montana and Wyoming to create a virtual learning and sharing forum for frontier communities focused on care coordination. Advance Care Planning: Awarded in 2016 to Hawaii to work with providers and beneficiaries on end of life planning. 5 Project ECHO (Extension of Community Healthcare Outcomes) A model aimed at increasing access to specialty expertise, knowledge and support in underserved areas. Target audience are CAH clinicians, CEO s, care coordinators and QI directors Audience meets every other Thursday using a very easyto-use platform called ZOOM 6 3
