Burbank Unified School District Technology Services REPORT TO THE BOARD TO: FROM: PREPARED BY: SUBMITTED BY: Members of the Board of Education Rick Nolette, Chief Facilities and Information Technology Officer Roberto Jurado, Director of Information Technology and Educational Support Lourdes Moya, Administrative Secretary DATE: February 2, 2017 SUBJECT: Approval of Amended Agreement for Professional Services, Massadi Enterprises, District-Wide (Bond Fund) Background: On May 18, 2013, Burbank Unified School District issued a Request for Qualifications for Division of State Architect (DSA) inspection services. On July 18, 2013, the Board of Education approved three firms, Tony Massadi Inspections (TMI) (TMI is now Massadi Enterprises), Stephen Payte DSA Inspections and the G Crew as qualified to provide DSA inspection services. The District had been using inspection services from Stephen Payte DSA Inspections, Inc. since November of 2015. They provided inspection services at various school sites in conjunction with the 21 st Century Technology and network infrastructure projects as well as the new data center. The 21 st Century Technology project includes audio/visual components, projectors, mounts, speakers, controls, cabling and wiring for classrooms. District Wide Goals for 2016-2017: 7.1 Provide a clean, safe, and energy-efficient learning environment for all students and employees. 7.7 Continue construction/modernization projects according to the General Obligation Bond Implementation Plan and develop a long-term maintenance plan. 8.3 Continue to upgrade the infrastructure at all sites to provide access to Wi-Fi services and 21 st Century classrooms. Discussion/Issues: Massadi Enterprises has been providing inspection services for Facilities construction projects, Technology Services is requesting to continue inspection services through Massadi Enterprises for the 21 st Century Technology and network infrastructure projects, District-wide, per the attached amended contract. They will continue to inspect all aspects of the projects to protect the interest of the District, and ensure that all work is compliant with building code and industry cabling standards.

Fiscal Impact: The cost for DSA inspection services for the remaining 21 st Century Technology projects is $28,700 and the cost for network infrastructure inspection services is $18,200, based on $70 per hour. Recommendation: Rick Nolette, Chief Facilities and Information Technology Officer, recommends that the Board of Education approve the amended agreement between Burbank Unified School District and Massadi Enterprises, to continue inspection services for the 21 st Century Technology (21.0-00000.0-93001-85000-6281-xxxxxxx) and network infrastructure projects (21.0-00000.0-93000-85000-6281-xxxxxxx), District- Wide, not to exceed $46,900 (Bond Fund), effective February 3, 2017 through February 3, 2018, and that authority to sign the agreement be exercised pursuant to BUSD-AR 3314, as presented.

BURBANK UNIFIED SCHOOL DISTRICT 1900 West Olive Avenue Burbank, CA 91506 AMENDMENT TONY MASSSADI, MASSADI ENTERPRISES, INSPECTION SERVICES THIS AMENDMENT is made and entered into 3rd day of February, 2017, by and between BURBANK UNIFIED SCHOOL DISTRICT, a public educational agency located at 1900 West Olive Avenue, Burbank, CA 91506 hereinafter referred to as the "DISTRICT, and Tony Massadi, Massadi Enterprises, hereinafter referred to as the CONSULTANT, mutually agree to amend the existing agreement dated: May 22, 2015, approved by the Board of Education May 19, 2016. WHEREAS, the DISTRICT desired to contract to receive Division of State Architect (DSA) Inspection Services at various sites as listed below for network infrastructure and 21 st Century Technology projects, Districtwide: John Burroughs High School, $5,600 Luther Burbank Middle School, $4,200 Joaquin Miller Elementary School, $3,500 George Washington Elementary School, $3,500 William McKinley Elementary School, $5,600 Walt Disney Elementary School, $5,600 David Starr Jordan Middle School, $8,400 Robert Louis Stevenson Elementary School, $5,600 Providencia Elementary School, $2,800 District Office, $2,100 WHEREAS, the Consultant is fully licensed to provide said services in conformity with the laws of the State of California, both parties agree to amend the Agreement as follows: 1) Pursuant to proposal dated January 20, 2017, the contract is amended to add an additional not to exceed $46,900 (Forty Six Thousand, Nine Hundred Dollars) for additional DSA inspection services to be provided as approved by the Board of Education on February 2, 2017, District-wide, effective February 3, 2017 through February 3, 2018. 2) WHEREAS, the DISTRICT and the CONSULTANT mutually agree to this amendment which is effective upon execution, the DISTRICT and the CONSULTANT further agree all other terms and conditions of the original AGREEMENT shall remain the same.
