Where practices are vulnerable, support through the Vulnerable Practice Scheme Investment 15,000 spent by 31/3/17 Secure improvements in vulnerable GP practices to help build resilience in primary care and to support delivery of new models of care. The practices will be in a position to meet the transformation agenda set on the GPFV.
