Entrance Requirements General Entrance Requirements: 1. Applicants must apply before the designated deadline(s). 2. Complete a South Florida College of Nursing application. 3. Submit official transcripts from high school and from all colleges attended. 4. Submit results of the ACT test. SFCN code 0605. 5. Minimum grade point average (GPA) of 2.5. 6. Complete a level II background check. 7. Submit immunizations records (Measles, Mumps, Rubella (MMR), Varicella and Hepatitis B. 8. Submit three letters of recommendation (non-family members). Nursing Program Entrance Requirements: Students are also required to provide a satisfactory criminal background check, Department of Health and Human Services (HHS)/ Office of Inspection General (OIG) clearance and drug screening. Science courses must have been completed within the past 5 years. 1. Completion of general admission requirements. 2. Entrance Testing: A2 HESI test is required. HESI cut scores for consideration are 75% or higher in all of the following areas: Reading, Vocabulary, Grammar, and Math. There is a $65.00 registration fee for the HESI exam. 3. Completion of all pre-requisites for nursing core courses (PNCC) with a minimum cumulative GPA of 2.5. 4. All applicants will participate in a formal interview. 5. Current American Heart Association adult and pediatric CPR certification and health form are required prior to the start of the nursing program. Final decisions as to granting prior credits, transfer credits, or equivalencies will be made by the Dean in consultation with the Nursing Committee.
College Application College Information and Admission Application Instructions Welcome to South Florida College of Nursing. Please read the following instructions carefully. All information must be complete before the application can be processed. All correspondence is to be emailed to rodsanchezrn@outlook.com. An online version of the application can be found at www.sfcn.us. A nonrefundable application fee of $75 is required from all new college students. Payment can be made on www.sfcn.us website, by regular mail or paid in person. Payment can be made with check or money order, please write the last four digits of your social security number on your payment. Application will not be processed until payment is received. A letter of acceptance will not be offered until all transcripts are evaluated. Notification of Social Security Number Collection and Usage In compliance with Section 119.071(5), Florida Statutes, South Florida College of Nursing (SFCN) issues this notification regarding the purpose for the collection and use of your Social Security Number (SSN). South Florida College of Nursing collects and uses your SSN only to perform College duties and responsibilities. To protect your identity, SFCN will maintain the privacy of your SSN and never release it to unauthorized parties in compliance with state and federal laws. The College assigns you a unique student/employee identification number which is used for associated employment and educational purposes at SFCN, including the access of your college records. South Florida College of Nursing may collect and/or use your Social Security Number for the following purposes: Federal legislation relating to the Hope Tax Credit requires that all postsecondary institutions report student SSNs to the Internal Revenue Service (IRS). This IRS requirement makes it necessary for SFCN to collect the SSN of every student. A student may refuse to disclose his/her SSN for this purpose, but he/she may be subject to IRS penalties. SSNs appear on official transcripts and are used for business purposes in accordance with parameters outlined by the U.S. Department of Education. Federal and State Regulations Tracking uses are authorized by SBE Rule 6A-10955(3)(e); 1008.386, F.S. and the General Education Provisions Act (20 USC 1221(e-1)). The College Reach-Out Program (CROP) uses are authorized by 1007.34, F.S. Hope/Lifetime Tax Credit uses are authorized by 26 USC 6050S and Federal Register, June 16, 2000/IRC Section 25A Registration uses are authorized by 119.071(5), F.S. Issuance of Form 1098T for tuition payment reports are authorized by 26 USC 3402, 6051 General Education Applicants Admission to the college does not constitute acceptance into the nursing program. Students applying with a high school diploma or GED must submit transcripts AND ACT test scores. Once accepted into the college, a student can complete the general education courses / pre-requisites to apply for the nursing program. South Florida College of Nursing is a new post-secondary institution, and not accredited. Accreditation by a governing body may be obtained once the institution has a graduating class.
