Eligibility: Must be an employee in good standing of Mercy Medical Center (full or part-time) and working in the specialty of pulmonary medicine/critical care as a registered nurse, respiratory therapist or other health professional. 1) Minimum GPA of 3.0 if the award will be used for academic courses. 2) Employee in good standing. 3) Must commit to work at Mercy Medical Center for one year following graduation, or reimburse the Dr. Darrell Dennis Endowed Pulmonary Medicine/Critical Care scholarship. WATSON ENDOWED NURSING (Post-Baccalaureate Nursing Scholarship) Medical Center who are committed to the delivery of quality care, by providing scholarships for post-baccalaureate academic coursework at the masters or doctoral level for an education, clinical, or administrative advanced practice role. Center, full - or part-time, and a student in a NLN or CCNE accredited masters or doctoral nursing program. ANNA PURNA GHOSH ENDOWED ONCOLOGY NURSING Purpose: The Anna Purna Ghosh Endowed Oncology Nursing Scholarship provides financial support for professional development activities of oncology nurses who are committed to ongoing nursing excellence in their oncology discipline. The annual award may be used to support an outstanding oncology nurse(s) in the participation of, or personal career growth in the following areas of educational development: attending national conferences, preparing for nurse practitioner certification, participation in certification programs and/or developing and delivering unit-based projects. Criteria for Eligibility and Selection: 1) Demonstrated excellence in caring and compassion, providing person-centered care through a focus on comfort and consolation and attention to the patient s physical, emotional and spiritual needs. 2) Demonstrated excellence in professional oncology practice, especially in clinical assessment skills and concentration on the application of current science and art in his/her bedside practice. The recipient will maintain an ongoing commitment to continuing education to remain knowledgeable of current medical practices and to apply this knowledge to their clinical decision making. 3) Recipient must be experienced in managing the patient s primary clinical problem and identifying any changes in their condition. In addition, they will support their patients in dealing with their illness and treatment physically, spiritually and emotionally. The recipient will be skilled in conducting a thorough review of systems, completing physical assessments, soliciting patient s feedback, and sharing their concerns. 4) Recipient will recognize that hospitalization is an episode of care along a continuum, and emphasis will be placed upon ensuring smooth transitions between levels of care and the various providers caring for them. 5) Recipient must demonstrate good corporate citizenship, a willingness to work effectively with co-workers and promote the mission and ICARE values of Mercy 6) Two (2) letters of recommendation from qualified professionals citing examples of dedication to oncology nursing and professional development are required. 7) Must be employed by Mercy Medical Center in oncology for a minimum of two (2) years. 8) Submit an essay of 300 words or less addressing the applicant s statement of interest, educational goals, and summary of personal dedication to the profession of oncology nursing. 9) Membership in ONS or another professional organization is preferred. Due to the excellent care that Tina received from the hospice staff at the Dennis and Donna Oldorf Hospice House of Mercy, the scholarship recognizes outstanding Mercy Medical Center staff who are committed to providing quality care to their patients. Eligibility: Individual has achieved an advance degree either through attainment of a BSN or MSN degree. 2) Must demonstrate financial need. 3) Evidence of commitment to the profession of nursing. 4) Must be willing to commit to working at Mercy Medical Center as a bedside staff nurse for two years. Award: The monetary amount of the award up to $1,000. KATHLEEN F. MERFELD, RN ONCOLOGY EDUCATION ENDOWED (Registered Nurse, Radiation Therapist, or Other Health Professional working in Oncology) Purpose: To recognize outstanding staff working in Oncology at Mercy Medical Center who are committed to the delivery of quality care, by providing financial support for academic courses, continuing education, educational conferences, or professional certification. Eligibility: Must be an employee in good standing of Mercy Medical Center, full or part-time, and working in the specialty of oncology, as a registered nurse, radiation therapist, or other health professional. 1) Minimum GPA of 2.70 if the award will be used for academic courses. 2) Employee in good standing. 3) Preference will be given to an employee in the Hall- Radiation Center. 4) Commitment to work at Mercy Medical Center for one year after receipt of the award. 3) Description of anticipated contributions to the profession of nursing and to Mercy Medical Center. Award: The monetary amount of the award up to $750. Recipient must commit to work at Mercy Medical Center for one year following receipt of the award or reimburse the Kathleen F. Merfeld, RN Oncology Education Endowed Scholarship amount of the award. THE SHELLIE DOCHTERMAN MEMORIAL (Surgical Technician Students) Medical Center who are committed to the delivery of quality care at Mercy Medical Center by providing scholarships for academic courses in a surgical technician program. Center (full or part-time) as a surgical technician working in the Birthplace or gynecological surgery. : 2) Evidence of portraying exemplary attitude and dedication to the profession; 3) Evidence of commitment to the delivery of quality patient care; and 4) Coursework applicable to furthering a nursing /hospital career. Award: The monetary amount of the award is $500. Recipient must commit to work at Mercy Medical Center for one year following graduation, or reimburse the Shellie Dochterman Memorial Scholarship fund the amount of the scholarship. 5

BASILE NURSING (Mount Mercy University BSN Student) quality care, by providing scholarships for academic courses in nursing at Mount Mercy University. Center (full or part-time) and a Mount Mercy University nursing student who is currently completing the sophomore year beginning the junior year. 1) Minimum GPA of 2.70. 2) Must demonstrate financial need. 3) Commitment to work at Mercy Medical Center for one year following completion of the generic nursing or BSN completion program. $1,000 and will be deposited in the applicant s account at the school in which applicant is enrolled. Recipients must commit to work at Mercy Medical Center for one year following graduation. ROZELLE CRAM NEEDS-BASED FOR NURSING (ADN, BSN, or MSN Nurse) Medical Center who are committed to the delivery of quality care at Mercy by providing scholarships for academic courses in nursing at any NLN-accredited nursing program. Center (full or part-time) and a nursing student in an ADN, BSN or MSN program. 1) Minimum GPA of 2.70. 2) Evidence of commitment to the delivery of quality patient care; and 3) Evidence of commitment to the profession of nursing. $500. Preference will be given to a single-parent student who lives in the Area 10 area and is a Mt. Mercy University nursing student. TILDEN ENDOWMENT EDUCATIONAL OPPORTUNITY Purpose: The purpose of this Endowment is to recognize those outstanding staff members at Hospice of Mercy who are committed to the delivery of quality end-of-life care by providing them with financial support for annual professional development activities. Grants from this fund may be used to pay the cost of attending educational seminars, conferences, and or for obtaining certification in areas related to hospice, palliative and end of life care. Grants may also be used by Hospice of Mercy to purchase educational development materials, up to $1,000. Eligibility: Other than a grant to Hospice of Mercy, the person requesting a grant must be a full or part-time employee of Mercy Medical Center, in good standing, who is involved in any of the disciplines of its hospice program, including nursing, social work, therapy, spiritual care, personal care, volunteers, bereavement care. 1) Educational development activity, it must be approved by a Hospice of Mercy administrator. 2) Employee must exhibit evidence of commitment to the delivery of evidence-based, high-quality hospice care. 3) Employee must agree to share materials, experiences, information and messages with other staff members upon completion of activity. 4) If award is to an individual, commitment to a minimum of one year of employment with Mercy Medical Center upon completion of activity. $1,000. and will be deposited in the applicants account at the school in which the applicant is enrolled. DONALD AND RITA ERUSHA ENDOWED NURSING quality care, by providing scholarships for nursing staff of Mercy Medical Center for academic courses furthering their nursing career. Center, full or part time, or a student in an accredited nursing program who is currently enrolled in the BSN completion program. Preference may be given to Mt. Mercy University nurs ing students. : 1) Minimum GPA of 2.70 2) Must demonstrate financial need. 3) Evidence of commitment to the delivery of quality patient care. 4) Commitment to work at Mercy Medical Center for one year after receipt of the award. $1,000. DALE AND LOUISE MORGAN ENDOWMENT EDUCATIONAL OPPORTUNITY (For the sole purpose of continuing education and professional development for perioperative services staff) Purpose: The purpose of this Endowment is to recognize those outstanding staff members within the perioperative services department who are committed to the delivery of quality and excellent care for patients who come to Mercy needing surgery by providing them with financial support for annual professional development activities. Grants from this fund may be used to pay the cost of attending educational seminars, conferences, and training, as well as, of obtaining certification in areas related to perioperative services. Costs would include registration/enrollment fees, transportation and/or lodging. Grants may also be used to purchase educational development materials. Eligibility: The person requesting a grant must be a full or part time employee of Mercy Medical Center, in good standing, who is involved in any of the disciplines of perioperative services. Criteria: Requests for grants will be awarded based upon consideration of the following: 1) If the request is for an educational development activity, it must be approved by the service line administrator. A request can then be made following the Mercy Foundation s departmental request process. 2) The requesting employee must exhibit evidence of her or his commitment to the delivery of evidencebased, high quality patient care. 3) The requesting employee must agree to share materials, experiences, information and messages with other staff members upon completion of activity. 4) If award is to an individual, the person must commit to remain employed with Mercy Medical Center for at least one year after completion of the activity or agree to reimburse Mercy Medical Center for the full amount of the grant. 5) Funding may be used for registration fees, transportation and/or lodging. The scholarship supports outstanding staff at Mercy quality care by providing financial assistance for academic courses supporting a MSN degree or ARNP program completion. Preference will be given to an applicant enrolled at Mount Mercy University. Over the past 60 years, and in service to her community and to Mercy Medical Center, Sr. James Marie s accomplishments have been extraordinary. She has been a nurse, teacher, administrator and mentor. In her role as an administrator, she was Director of the Mercy School of Nursing for 6 years, Chairman of the Department of Nursing at Mount Mercy College for 17 years, President of the Cedar Rapids Sisters of Mercy for eight years, and served on the boards of the Catherine McAuley Center, Mount Mercy College and Mercy Medical Center. She also serves as the Chairman of the Mercy Medical Center Foundation Board of Directors, a position she has held for the past 14 years. SHEDEK MEMORIAL Purpose: This scholarship has been established in memory of Mary Shedek, Hospice Social Worker, who passed away July 21, 2014. Mary was a dedicated Social Work professional who stretched herself in educational pursuits beyond minimum requirements for a job. Mary was an avid believer in education and this scholarship will be awarded to employee(s) who are also committed to enhancing their skills by taking academic courses, continuing education or pursuing licensure or additional certifications to provide higher quality care to their patients/clients. Eligibility: Must be an employee in good standing of Mercy Health System, full or part-time. Social workers pursuing licensure, (LBSW, LMSW, LISW) or a Masters Social Work degree (MSW) or specialty certifications will be given preference for this scholarship. Other disciplines may be considered if pursuing human service, hospice or case management related education. RITA KRUEGER WHITTERS MEMORIAL ENDOWED NURSING Purpose: This scholarship has been established in memory of Rita Krueger Whitters who graduated from the Mercy School of Nursing in 1951 and to recognize outstanding staff at Mercy Medical Center who are committed to the delivery of quality care at Mercy Medical Center by providing scholarships for academic courses to further their nursing careers. Center, full or part-time, enrolled in a BSN completion program at any accredited nursing program in Iowa. Criteria for selection: 1) Minimum GPA of 2.7. 2) Employee in good standing. 3) Evidence of commitment to the delivery of quality patient care and to the profession of nursing. 4) Financial need will be considered in the selection of the recipient. 5) Commitment to work at Mercy Medical Center for one year following receipt of the award. 6) Preference will be given to a graduate of Xavier High School (or graduates from the former Regis or LaSalle High Schools) in Cedar Rapids, Iowa. 2) Letters (2) of endorsement; one must be from a faculty member at the school in which the applicant is enrolled and one from the Mercy Medical Center manager. $500 and will be deposited in the applicant s account at the school in which applicant is enrolled.
