Scope of Practice KEY TASKS Pharmacist as defined by the Pharmacy Council of NZ Communication and Key Relationships 1. To develop collaborative relationships with all members of the team. 2. To develop strong communication links with the Clinical Director, senior nursing staff & the Manager of the team, and attend meetings / provide reports as requested. 3. Promote team work practices in the pharmacy, participating in consultative processes within the department 4. To communicate clearly and effectively with all stakeholders Clinical Pharmacy Activities 1. To provide a clinical pharmacy service that adheres to the Waitemata DHB Clinical Pharmacy Guidelines and the NZHPA Clinical Pharmacy Guidelines Patient Medication Histories and Medicine Reconciliation Clinical & Medication Order Review Drug Therapy monitoring Therapeutic Drug Monitoring Adverse Drug Reaction Monitoring and reporting Participation in ward rounds and meetings Patient counseling Education on medicines to patient groups and health professionals Provision of Medicine Information 2. To participate in the collection of clinical pharmacy interventions and workload statistics as requested by the Clinical Pharmacist Coordinator 3. Participation in Research & Drug Utilisation Evaluation activities 4. Participation in Outpatients Clinics as required 5. Education & Competency 1. To participate in peer review and competence assessment programmes 2. Participation in Continuing Education and maintenance of CE Logbook 3. Participation in post-graduate clinical pharmacy studies where appropriate General 1. To participate in the Pharmacy Service To attend staff meetings and to promote team work practices in the Pharmacy To participate in the clinical pharmacist service to other areas if required To maintain dispensing skills by participating in the dispensary roster at a level agreed with the Clinical pharmacist Coordinator To participate in the weekend service To participate in the on-call roster if required To participate in the training of junior pharmacists, interns & externs To undertake other duties as requested by the Chief Pharmacist 2.
