ADDENDUM NO. 1 Dated: 10/31/17 RFP No. P18024 For Pest Control: Bed Bugs Heat Treatment All Contractors bidding the project are hereby advised to incorporate the following changes and/or clarifications and related dollar amounts in their bids for the above-referenced project. All conditions described in the project SOW and Specifications shall apply in full force. Attendance at the Pre-Bid Meeting: SNRHA: Wanda L. Beckett, Contracts Administrator, SNRHA Procurement Dept.; Companies/Individuals Represented: Joe Restifo, Guaranteed Pest Solutions and Rob Suckoll, Prochem Proactive; Meeting was conducted by W. Beckett, Contracts Administrator who provided an overview of the RFP P18024 requirements per the attached agenda. Meeting was called to order at 10:05 a.m. This meeting is being recorded. W. Beckett provided an overview of the solicitation process (noted below) using overhead projection of the solicitation package. 1.1 All participants in this solicitation process have to be registered on ha.economicengine.com to access and participate in this bid process. All pricing information has to be submitted in ha.economicengine.com website only. This RFP is also available for downloaded only from the SNVRA.org website. Note: It is important that bidders register on the ha.economicengine.com to input the cost proposals. 1.2 W. Beckett is the only SNRHA contact for this RFP. It is prohibited to make contact with any other SNRHA employee, resident or Board of Commissioner regarding this solicitation. Failure to comply with this requirement shall result in the bid being considered non-responsive and not evaluated. 1.3 Question and Answer (Q/A) period: The Q/A period ends November 17, 2017, 10:00am. All questions must be submitted in writing and will be responded to through the ha.economicengine.com only. Bidders are allowed to submit their questions via email to me (wbeckett@snvrha.org) however the response to your questions will be issued through ha.economicengine.com. 1.4 Submission Deadline: The submission deadline for all proposals is November 27, 2017, 10:00am, per the RFP instructions all hardcopy proposals must be delivered to SNRHA Page 1 of 5

Late submission will not be accepted. 1.4.1 Clarification: Do not include any cost or pricing information in the hardcopy proposals submitted. 1.5 Contract: SNRHA is federally funded and so is this contract. The award of this contract will be presented for Board of Commissioners approval on December 21, 2017 due to the anticipated service cost exceeding the $100k threshold. The Contract(s) awarded shall be for a total of 5 yrs. One (1) year and four (4), one (1) year options to extend. 1.5.1 Clarification: The contract to be awarded is an Indefinite Quantity Requirements Contracts, SNRHA makes not any guarantees as to the quantity of services required. 1.5.2 SNRHA maybe awarding contracts to more than one (1) company depending on the bid results. 1.6 Protest Procedures Pg. 6, if you feel the solicitation process is unfair or the contract award was unfair here are the procedures to follow; 1.8 Section 2 -Scope of Service pg. 10, companies must be qualified provide resumes, licenses, certifications etc. 1.8.1 Service methods provide a description of the inspections; Heat treatment to be provided; An any alternative methods that are used; 1.8.2 Q: Who makes the determination of what protocol is to be used? A: As the expert the SNRHA will look to the contractor to make recommendations as to the service protocol to be used. To be approved by the SNRHA representative at the site which will be the Property/AMP Manager? Based on your inspection you as the contractor should be able to let us know the treatment protocol to be used. If chemicals are to use in conjunction with heat treatments then you are required to provide the MSDS on those chemicals. Please note that we have seniors and family sites. 1.8.3 Preparation of the unit description of the preparation process, any precautions taken in providing this service, etc.; Identify the number of staff to be utilized for this service if feasible. 1.8.4 Q: Talking about protocol, who makes the decision on what is needed to treat a unit? A: You, the contractor are the expert and as the expert should inform the SNRHA of the process to be applied, heat and any other proposed treatment to control the infestation. 1.8.5 Emergency response within 4hrs. Clarification: Respond to request for service, to Page 2 of 5
establish when the service is required. Bidders must provide an emergency contact phone number. 1.8.6 Bed Management Plan: the HA spends in access of $500k (approx.) a year. We are seeking bidders to provide a Bed Bug Management Plan to aggressively attack this problem to eliminate or minimize the problem. To include SNRHA staff training to educate on Bed Bugs; 1.8.7 Page 14, contains a list of SNRHA where services may be required. Please not the properties marked with (RAD) are properties that are not under SNRHA direct management; but have been listed here in case the SNRHA has a need for the service for these sites; To highlight some of the forms and paper work requirements contained within the RFP document: 1.9 The RFP include instructions Section 3.0 page 15 on how the proposal shall be assembled. This section provides detail of what information shall be contained in each section. 1.9.1 Attachment A Form of Proposal summarizes how the proposal should be organized. The Form of Proposal should be filled out signed and placed under Tab A along with any Addendums or Notices issued in connection with this RFP. Attachment B contains the Disclosure of Ownership and Conflict of Interest forms, to be filled out completely and signed; To be submitted in accordance with the Form of Proposal Attachment A; 1.10 Attachment C: Contains HUD documents (HUD 5370C, 5369C) that are the terms and conditions of your contract; and required SNHRA forms. Bidder should read these documents to ensure you understand the contract that you will be required to sign. Please note that the contract executed is subject to HUD audit just as SNRHA is subject to HUD audit. Therefore I would advise you set up your files for this contract separately from your other contracts. 1.11 The SNHRA contract in Attachment G is a boilerplate that is used, wherein it includes terms and condition and specifically references the HUD documents provided in this solicitation. 1.11.1 Please note that SNRHA contracts are not transferable, meaning if you sale your business this contract cannot and will not transfer to the new owners. The contract will be cancelled. 1.12 Safety and Training Plan must conform to the OSHA regulations and the state agencies that regulate this service. The Plan may be submitted on a CD. 1.13 Management Capacity/Financially solvent. Provide information on the management qualification and information from your CPA that establishes you as financial solvent. Page 3 of 5
1.14 Client Information: List of clients that you have provided services that are similar to the Housing Authority. Bidders can include letters of recommendation in this section. 1.15 Tab 7 EEO Policy Provide a copy of your companies Equal Employment Opportunity Policy; 1.16 Subcontractor/JV Partner: provide information on any subs or JV partners that will be providing service under the contract awarded. Subs /JV are subject to the same verifications bidders are. Provide a Disclosure of Ownership form, licenses, certification, etc. Background checks will be done on all contractors, subcontractor and Joint Venture Partners. All companies doing business with SNRHA must be registered on SAM.gov; 1.17 Section 3: All HUD funding is subject to the Section 3 requirements therefore the contract awarded is subject to the Section 3 requirements. The SNRHA provides supportive services to help our residents be self-sufficient by providing education, training and employment. SNRHA has established a job bank of residents that are seeking employment. 1.18 Therefore SNRHA requests first a commitment to the regulation by signing the Attachment D Contractor Initial Response form and policy statement; and secondly if an employment opportunity opens in your company we require first notification and look at the job announcement. The Section 3 Coordinator will look in the job bank to see if a SNRHA resident is interested in applying. Please Note: SNRHA residents that apply for a job with your company are subject to your qualification requirements as with any potential applicant. Additionally, the 1.19 Section 3 SNRHA Quarterly Reporting requirements: The SNRHA Section Coordinator will be contacting you quarterly to request hiring information from your company. This only applies to employees that are paid directly from SNHRA funds. Contracts are requested to report if any personnel have been hired during the quarter. 1.19.1 Debi Kenney, Sec 3 Coordinator notified me today that she is leaving SNHRA, so if you have questions you need to call her this week, her information is in the solicitation. As soon as I know who the new Sec3 Coordinator is I will send out notification. 1.20 Section 3 Business Certification: Attachment E provides information on what is required to be certified as a Section 3; If you seek Certification please allow 5 days and submit your information to Debi Kenney; Company must have the Certification in the proposal submitted to obtain the additional preference points identified on page 21 of the RFP; Overview of Attachments Appendix 1 Description of the sites/properties listed with sq. ft. and additional detail of the sites; This was provided because some companies charge by the square foot while others charge by the units. SNRHA is requesting a unit cost. So this information has been provided to assist bidders in determining a unit cost including profit and overhead; Page 4 of 5
Attachment A Form of Proposal is to be placed under Tab 1 of the submittal; this form gives you the directions on how to organize your submittal; Also under this section behind the Form of Proposal place any Addendums or notices issued in connection with this RFP. The minutes from this meeting shall be Addendum No. 1 along with any question received prior to issuing Addendum No. 1; Attachment B Disclosure of Ownership, please fill out all the information completely including the demographic information. Conflict of Interest form must be filled out completely and bidders must disclose if there is a conflict of interest meaning a family member that works with the Housing Authority; SAM.gov System for Award Management - All companies doing business with the SNRHA must be registered on SAM.gov. Please note that your company information in SAM must be updated annually. We will not make an award if your company is not in SAM.gov; Insurance requirements also contained in Attachment B; Attachment C HUD and SNRHA forms, we have listed the forms that have to fill out and returned with your proposal and those that should be retain for your information. Please note that the ones retained for your information are the forms that relate directly to and are noted in the contract document. Please read every document carefully and if you have questions please contact me.
