What is an Federally Qualified HC? Federally Qualified Health Center Federally funded to assure access to affordable, accessible, quality and cost-effective primary health care to patients regardless of ability to pay
Continuous Learning Health Center System Health Technology Continuous Learning systems include HIT, quality improvement and clinical (HIT) performance, operational performance and infrastructure and workforce to support the Health Center Program. continuous Learning Health Systems enable research Health to influence practice Quality and practice to influence research. Continuous Learning Health System collect and analyze data Center from patient encounters Improvement to inform changes to care delivery. HRSA Funded Support Primary Care Associations (PCAs): Statewide networks which help FQHCs improve programmatic, clinical, and financial performance and operations. Health Center Controlled Networks (HCCNs): Regional networks which help member FQHCs address operational and clinical challenges related to the use of health IT. National Cooperative Agreements (NCAs): 20 National organizations providing free subject matter expertise across a broad range of operational and programmatic topics. 8
Role of Partners Opportunities to accelerate performance: Improve data and analytics capacity Identify and disseminate best/evidence-based practices Support practice transformation and quality improvement activities Advance operational performance and business acumen (including governance) Participate in delivery system reform 9
Health Center Controlled Networks HCCN funding will support training and technical assistance (T/TA) for existing and potential health centers to: Adopt and implement ONC certified electronic health record technology; Enhance comprehensive, integrated data collection, analysis, and reporting; Meet the requirements of the CMS EHR Incentive programs; Improve clinical and operational quality to reduce health disparities and improve patient and population health outcomes through the use of health information technology; Develop Health Information Exchange (HIE) infrastructure to improve care coordination and manage population health; and Provide peer support for sharing data and best clinical and business practices. 10
NACHC Network Guide The NACHC Network Guide provides and overview of HCCNs Information is updated by the HCCNs with secure log in Areas of interest: Basic information including location and contact details Characteristics of Network members EHR Vendor and Type of Support Quality Reporting and Performance Measures Practice Transformation 11
HITEQ Center The HITEQ Center is a HRSA-funded Resource Center that collaborates with PCAs and HCCNs to support health centers in full optimization of their EHR/Health IT systems
