Before Starting the Project Listings for the CoC Priority Listing The FY 2018 CoC Consolidated Application requires TWO submissions. Both this Project Priority Listing AND the CoC Application MUST be submitted prior to the CoC Program Competition deadline as required by the FY 2018 CoC Program Competition NOFA. The FY 2018 CoC Priority Listing includes the following: - Reallocation forms must be fully completed if the CoC is reallocating eligible renewal projects to create new projects as described in the FY 2018 CoC Program Competition NOFA. - New Project Listing lists all new project applications created through reallocation, the bonus, and DV Bonus that have been approved and ranked or rejected by the CoC. - Renewal Project Listing lists all eligible renewal project applications that have been approved and ranked or rejected by the CoC. - UFA Costs Project Listing applicable and only visible for Collaborative Applicants that were designated as a Unified Funding Agency (UFA) during the FY 2018 CoC Program Registration process. Only 1 UFA Costs project application is permitted and can only be submitted by the Collaborative Applicant. - CoC Planning Project Listing Only 1 CoC planning project is permitted per CoC and can only be submitted by the Collaborative Applicant. - HUD-2991, Certification of Consistency with the Consolidated Plan Collaborative Applicants must attach an accurately completed, signed, and dated HUD-2991. Things to Remember: - All new and renewal projects must be approved and ranked or rejected on the Project Listings. - Collaborative Applicants are responsible for ensuring all project applications are accurately appearing on the Project Listings and there are no project applications missing from one or more Project Listings. - If a project application(s) is rejected by the CoC, the Collaborative Applicant must notify the affected project applicant(s) no later than 15 days before the CoC Program Competition application deadline outside of e-snaps and include the reason for rejection. - For each project application rejected by the CoC the Collaborative Applicant must select the reason for the rejection from the dropdown provided. - If the Collaborative Applicant needs to amend a project application for any reason after ranking has been completed, the ranking of other projects will not be affected: however, the Collaborative Applicant MUST ensure the amended project is returned to the applicable Project Listing AND re-rank the project application BEFORE submitting the CoC Priority Listing to HUD in e-snaps. The AHA Rapid Re Housing program will house 24 families who have become homeless and will be housed in scattered site permanent housing. Rapid Re Housing program will house 30 families who have become homeless and will be housed in scattered site permanent housing. The RRH expansion project will serve an additional 15 families with children, expanding current programming to a total of 45 families. The RRH expansion will fund 15 scattered site, single family units at capacity and provide essential tenant based rental assistance through rapid rehousing for up to 12 months to 15 families comprised of approximately 42 individuals (20 adults over 18 years of age and 22 children). Supportive services will include hiring of a case manager, a percentage FTE of current RRH support staff, childcare services through stipend payments to vendors to allow families to pursue employment, and transportation through purchase of bus passes and project staff mileage reimbursement. CitySquare OnTRAC Tarrant TH/RRH will be a designated program for 6 homeless, unaccomapanied youth age 18 24 who are chronicall homeless, former foster youth, or have a criminal background. The CEC Rapid Rehousing program will provide housing and assistance to families who are homeless coming from unsheltered and emergency shelter settings. The CEC will serve 25 families during the contract period. Gateway to Housing project will provide 32 units of permanent supportive housing for homeless and chronically homeless adults wo have disabilities related to serious mental illness, substance abuse, or co occurring disorders. Housing Solutions Combined is designed to stabilize housing, increase income, and enhance self determination for 57 chronically homeless individuals and 8 families through permanent supportive housing.

10 Presbyterian Night Shelter Housing Solutions Combined (Expansion) PSH Arlington, TX Housing Solutions Combined (HoSo)is a permanent supportive housing program that follows the Housing First Model outlined by HUD. This project seeks to expanded by 15 units of permanent supportive housing for a total of 112 units (104 for individuals and 8 for families). The program also seeks to stabilize housing long term, increase income, and improve the self sufficiency of individuals and families with the highest needs. This is achieved by having a very low barrier entry (chronic homelessness is the only qualification); employing client directed case management; and using harm reduction strategies. Case management services are centered around a thorough needs assessment, collaboratively developed goals, measurable outcomes, and referrals to other supportive and mainstream services. 11 SafeHaven of Tarrant County SafeFoundations for RRH RRH Arlington, TX 12 SafeHaven of Tarrant County SafeSolutions for RRH RRH Arlington, TX 13 SafeHaven of Tarrant County Supporting SafeSolutions (Expansion) RRH Arlington, TX 14 Tarrant County SIMON PSH Arlington, TX 15 Tarrant County Salvation Army Veterans PSH Program PSH Arlington, TX The SafeFoundations Rapid Rehousing program will provide 16 units of rapid rehousing to homeless families impacted by domestic violence. The SafeSolutions Rapid Rehousing program will provide 16 units of rapid rehousing to homeless families impacted by domestic violence. Supporting SafeSolutions is designed to provide housing and supportive services to individuals and families who are homeless because they ve fled domestic violence (DV). It will rapidly rehouse homeless victims of DV in 8 scattered site units across Tarrant County. SHTC will provide supportive services, including case management, for program participants. This expansion will provide housing and direct supportive services to 8 adults and 8 children (6 households with children; 2 individual adults). The SIMON PSH program serves individuals afflicted with mental illness and alcohol/drug abuse. The project based program serves 40 persons. The Salvation Army s Supportive Service Housing Program provides permanent housing, long term comprehensive case management, and supportive services for 28 chronically homeless, disabled veterans at scattered sites within Tarrant County. The program is designed to 1) stabilize housing, 2) increase income, and 3) enhance self determination for chronically homeless, disabled veterans. 16 Tarrant County Housing SPC PSH Arlington, TX The Tarrant County Shelter Plus Care program proposes to provide permanent housing and supportive services for 8 chronically homeless individuals 17 Tarrant County TSA Housing First PSH II PSH Arlington, TX The Salvation Army Housing First Permanent Supportive Housing II Program provides permanent housing, long term comprehensive case management, and supportive services for sixteen chronically homeless, disabled individuals at scattered sites within Tarrant County. The program is designed to 1) stabilize housing, 2) increase income, and 3) enhance self determination for chronically homeless, disabled individuals. 18 Tarrant County TSA Housing First PSH PSH Arlington, TX The Salvation Army Housing First Permanent Supportive Housing Program provides permanent housing, long term comprehensive case management, and supportive services for 14 chronically homeless, disabled individuals at scattered sites within Tarrant County. 19 Tarrant County SafeTomorrows PSH Arlington, TX 20 Tarrant County TSA Housing First PSH Expansion PSH Arlington, TX 21 Tarrant County 3CP RRH Arlington, TX SafeTomorrows is a permanent supportive housing project (PSH) for 8 chronically homeless and/or disabled homeless individual or family households experiencing domestic violence. The Salvation Army Housing First Permanent Supportive Housing Program provides permanent housing, long term comprehensive case management, and supportive services for 15 chronically homeless, disabled individuals at scattered sites within Tarrant County. The program is designed to 1) stabilize housing, 2) increase income, and 3) enhance self determination for chronically homeless, disabled individuals. CEC Comprehensive Care Program (3CP) exists to provide case management and rapid re housing opportunities for 17 households to gain a greater sense of self determination, increase their income, and become stably housed over the course of a year period. 22 Tarrant County Homeless Coalition HMIS HMIS Arlington, TX The CoC HMIS project will provide all aspects of data collection for the Continuum of Care utilizing Social Solutions ETO HMIS solution. The CoC HMIS will include all CoC Program Funded programs, VASH, SSVF, HOPWA, ESG, SAMHSA PATH, State of Texas HHSP, and City of Fort Worth Directions Home Homeless Program data. The HMIS will provide all PIT, AHAR, HIC, APR and additional performance tracking and analysis to monitor, plan and respond to trends and gaps in homeless housing and services. The HMIS will house intake, assessment and prioritization functions for the Coordinated Assessment System compliant with the HUD CPD notice on prioritization and the Continuum of Care policies and procedures. The HMIS will provide Executive Director and Program Director level dashboard reporting on project performance and grant management. The HMIS Project will provide technical assistance, monitoring and services for data collection, data quality, data security, program evaluation, program reporting, performance evaluation, performance reporting and other analysis support needed for agency and community accountability.

23 Tarrant County Homeless Coalition HMIS HMIS Arlington, TX TCHC serves as the HMIS Administrator for TX 601 and utilizes Social Solutions Efforts to Outcomes as its HMIS system. The current software is able to fulfill all HUD requirements in relation to data collection and program/system level reporting. The CoC has established the need to produce more enhanced and integrative reporting and data collection and has identified the Green River software to complement existing HMIS resources to better automate the local Coordinated Entry System processes and allow for the integration of outside system data, including health care, criminal justice, independent school districts and housing authority systems. The integration of these data sets will allow for data cross reference for increased support and service to clients interacting with multiple systems. TX601 has also identified the Outreach Grid mobile software for enhanced coordination of services for unsheltered homelessness. Outreach grid is a mobile technology that integrates HMIS required data elements and coordinated assessment functionality for outreach staff to more simply collect information while working in the field. The software allows for a more systematic approach to coordinate CoC outreach teams through an automated and improved referral process and known campsite GPS plotting. The software will allow unsheltered homeless service requests from the public at large and full data integration with local HMIS system. 24 Tarrant County Homeless Coalition Coordinated Assessment SSO Arlington, TX The CoC Coordinated Assessment System (CAS) will provide telephone, text and email based information and referral services for individuals at risk of homelessness in Tarrant and Parker counties through a dedicated Homeless Helpline, which is staffed six days a week. This program provides both rental assistance as well as supportive services. Housing consists of financial assistance for leasing of both multifamily and single family scattered site units. The project will serve at least 80 households. 27 Tarrant County Homeless Coalition CoC Planning Planning Arlington, TX CoC planning activities will include, but not limited to: TCHC will conduct monthly full membership meetings with agenda's posted on the TCHC website. TCHC will conduct and annual call for membership in January and conduct one meeting per year specifically to address the needs and progress of the system of care for the homeless. TCHC will staff the Continuum of Care Board of Directors consistent with the Collaborative Applicant CoC Board of Directors Memorandum of Agreement. TCHC will work directly with the CoC Board in the establishment and implementation of the CoC Policies and Procedures. TCHC will assist the CoC Board in reviewing the CoC Charter that sets the governance structure of the CoC at least every 5 years. TCHC will staff all CoC Board committees and work groups. TCHC will call CoC Planning Operations level committee and work groups with emphasis on those directed to ending veteran and chronic homeless and developing the CoC capacity on rapid rehousing. TCHC will provide quarterly, mid year and annual monitoring and performance reporting for all CoC and ESG reporting. TCHC will follow the monitoring and evaluation procedures in the CoC Policies and Procedures.
