What is Open Door? Open Door has been delivering top-notch health care services since 1973. We provide prevention-focused health care for low-income people in Westchester and Putnam, regardless of ability to pay. Our medical services are on par with the region s finest health care providers. Is Open Door a free clinic? Open Door is NOT a free clinic. We bill for our services through Medicaid, Medicare and other commensurate insurers. For those without insurance, our sliding-scale fee is based on family income and is deeply discounted. We also assist eligible patients in signing up for health insurance. No one is ever turned away. Are the clinicians at Open Door Volunteers? No. Open Door s clinicians are licensed medical professionals. Under the direction of a Chief Medical Officer, Chief Dental Officer and Senior Director of Behavioral Health, our staff consists of highly trained, clinicians, who represent the very best of their field. Our doctors are all board-certified, credentialed to the highest level, and passionate about delivering the very best in patient care. What services does Open Door provide? What are Open Door s Residency Programs? Open Door s Residency Programs: Train up to 24 primary care residents and 10 dental residents each year, who learn and work along-side Open Door s experienced team of clinicians for hands on training in community health. Why does Open Door need to fundraise? Some of Open Door s most critical and life-saving services are not covered or reimbursed through insurance. They include: Patient Advocacy, Wellness and Behavioral Health Care Integration. What is Open Door s Impact? Open Door exceeds national health benchmarks for: Primary care quality. Maternal, infant and child health outcomes. Mental health assessments. Tobacco cessation. Substance abuse support and referral. Obesity prevention. How does Open Door save tax dollars? Our commitment to community includes responsible use of public resources to maximize health care impact while lowering costs. Our prevention-focused approach reduces emergency room usage, hospitalizations, and other costly medical services. Open Door saves Medicaid an average of 24%, when compared to other providers.

Why is Open Door needed? Westchester and Putnam are known as affluent communities, but a growing number of our neighbors struggle with poverty, unemployment, lack of health insurance, and life-threatening health disparities. Open Door serves people from across the socioeconomic spectrum, with a special focus on the tens of thousands of local individuals and families who would otherwise be left out of the health care equation. Why are Open Door s programs so critical? The need for Open Door s affordable, barrier-free approach remains as vital as ever. Healthier communities mean fewer sick days for workers and students, fewer emergency room visits and hospitalizations and lower health care costs. And that benefits everyone in Westchester and Putnam. What is the Open Door Foundation? The Open Door Foundation provides philanthropic support for critical programs and services that are not covered through government reimbursements or grants. These areas Patient Advocacy, Behavioral Health Integration and Wellness are the cornerstone of our integrated approach. Together, these services cost $2 million each year. This investment of less than 4% of our annual budget reaches 70% of our patients with cost saving and life saving interventions.

How can people get involved at Open Door? Open Door s community of volunteers has grown to 500 people. We match the interest and experience of volunteers with Open Door s needs and priorities. Volunteer opportunities include: Collecting and organizing books and gift bags for children in our Reach Out and Read Program and toys for our Santa Days Learning and mentoring through college and high school internships Reading to young patients in the waiting rooms Baby Box Fundraising and Friend-Raising Unloading and bagging produce for those in need at the Fresh Food Market in Mamaroneck Joining our board What is the Wellness Program at Open Door? Each month, Open Door s certified Wellness Instructors offer dozens of exercise, nutrition, healthy eating for children and adults. Our Wellness program also includes an evidence-based Chronic Disease Self- Management Program and a Diabetes Prevention Program. Our nutritionists help patients create and monitor healthy eating plans, including grocery store tours and cooking classes. How many patients does Open Door serve a year? In 2018, Open Door will serve: 55,000 patients. 35% below the age of 19. What is Open Door s annual budget? $50 million

Racially and economically diverse, most of our patients live in poverty and experience on daily basis the psychological and emotional problems that living without money, skills, education bring. The majority of our patients live at or below the federal poverty level, which is $24,600 for a family of four. What s at stake? We know that in order to ensure stable communities, we need to break cycles of poverty. Accessible health care is a major contributor to wellness and a sense of security. Healthy, safe families contribute to a thriving and engaged society. How do we handle medical emergencies? We work closely with EMS and Phelps. Our Residents follow the care of our patients who have been hospitalized. We have nebulizers and other emergency equipment on site.

What does it mean to be an FQHC? FQHC s like Open Door are required to: Serve a medically underserved area or population. Offer a sliding fee scale. Provide comprehensive services. Meet rigorous health and safety standards. Have a governing board of directors, that includes patients. Why do we still need Open Door if people can be covered under the Affordable Care Act? An estimated 13% of the overall U.S. population continues to go without health insurance and finds cost an impediment to accessing care, even more are ineligible or underinsured. Locally, Open Door is here to help people navigate their options under the Affordable Care Act and sign them up for insurance. 43% of Open Door patients do not have insurance. What is the Patient Advocacy program at Open Door? Our patients face serious issues that impact their ability to be and stay healthy and our patient advocate program is a critical component to our success. Patient Advocates are non-clinical staff who help patients navigate the health care system. What is the Behavioral Health program at Open Door? Too often, mental health and chemical dependency are under-treated and stigmatized yet for countless people, early intervention is critical. Physical and emotional health are interconnected, which is why we integrate mental health services, offered on site, within each health center. ation?
