REGISTERED PROFESSIONAL NURSE CARE COORDINATOR DISTINGUISHING FEATURES OF THE CLASS: The professional nursing health care position is responsible for performing highly complex duties and administrative support to further the overall work of the department as instructed by the Director of Public Health and the Director of Patient Services. Work is performed under the general supervision of the Director of Public Health, with the use of independent judgement for the completion of the work assigned. TYPICAL WORK ACTIVITIES: Screens calls/visitors to determine the nature of the inquiry. Answers questions on established policy/procedure and/or refers the call to the appropriate party or office. Monitor and track all personnel files, and is responsible for obtaining all required documentation from agency staff and contracted personnel. Prepare data for annual Medicare and Medicaid Cost Report. Organizes, prepares charts for, and takes notes during the CQIC/PAC/Staff meetings. Coordinate CHHA referrals and intake from providers, ensuring complete information to facilitate referrals. Data entry of referrals and upload documents into KanTime, our EMR. Verify and obtain authorization for insurance. Upload and Transmit OASIS to/from BlackTree, and export to the State. Process sliding fee scale applications based on patient income and Federal Poverty Guidelines. Provide data entry and maintenance for the Certified Home Health Agency software program as needed. Fazzi and Pools reporting monthly. Coordinate bi-monthly meetings with CHHA billing clearinghouse, BlackTree. When needed, executes direct nursing care to patients by assessing their physical, psychological and emotional status, by developing and implementing a plan of care. Participates in CHHA on call rotation for nights/weekends. FULL PERFORMANCE KNOWLEDGE, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS: Thorough knowledge of current Public Health nursing practices, both clinical and theory; thorough knowledge of prevention of and control of disease; thorough knowledge of current policy and procedures related to administration of skilled nursing services in home-care environment; good knowledge of community health nursing and community resources; skill in the application of nursing procedures and techniques of patient care; ability to organize and manage time effectively; ability to plan, organize and direct the training of others; ability to communicate effectively both orally and in writing; ability to establish and maintain cooperative working relationships; ability to accept and utilize guidance; ability to use a personal computer; physical condition commensurate with demands of the position.
MINIMUM QUALIFICATIONS: Either A.) Graduation from a regionally accredited or NYS registered college, university or one accredited by the NYS Board of Regents to grant degrees with a Bachelor's degree or higher in nursing. B.) Graduation from a regionally accredited or NYS registered college, university or one accredited by the NYS Board of Regents to grant degrees with an Associate's degree in nursing AND at least three (3) years of experience as a community health nurse or public health nurse; OR C.)Completion of an accredited course of training for Registered Professional Nurse, and a minimum of five (5) years of experience as a community health nurse or public health nurse. SPECIAL REQUIREMENTS: 1.) Licensure and current registration as a registered professional nurse in New York State. 2.) Possession of a valid NYS driver's license appropriate to the vehicles being operated or otherwise demonstrate the ability to meet the transportation needs of the position. Created: July 26, 2018
