General Summary of Function/Purpose of Position This position carries out a range of analysis and evaluation assignments for the ursing Facility Transition (FT) and Brain Injury Waiver (BIW) programs. This position is responsible for managing data collection, analysis, and reporting for the programs. This position will work with organizations from several distinct provider networks using various methods to collect, compile, and analyze program data and ensure thorough and accurate reporting. This position is responsible for verifying the qualifications of persons interested in transitioning from the nursing facility and authorizing MI Choice waiver agencies and Centers for Independent Living to work with the individual. This position also works with other department staff to develop a website for the MI Choice, FT, and BIW programs and assures appropriate content is posted to and maintained on the website.

Duty 1 General Summary: Percentage: 40 Verify the qualifications of persons interested in transitioning from the nursing facility and authorize MI Choice waiver agencies and Centers for Independent Living to work with the individuals. Collect, review and analyze program reports. Research, verify, and approve individuals identified as potential nursing facility transition candidates in the online nursing facility transition database using CHAMPS, Bridges, COMPASS and other IT systems. Make direct contact with agency personnel when issues or questions arise, using the database, telephone, or email. Develop and maintain methods to assure timely and accurate processing and tracking of nursing facility transition candidates. Provide assistance, direction, and consultation to waiver agency or Center for Independent Living staff as needed to assure accurate information is relayed to MDHHS. Determine if records should be unlocked to be updated by waiver agency or Center for Independent Living staff. Collect quarterly activity reports from Outreach Specialists and Housing Specialists. Analyze these reports for completeness and compliance with contract requirements. Recommend and implement program improvements, including improvements to the online system. Duty 2 General Summary: Percentage: 40 Serve as liaison for procedural and administrative aspects of the BIW program. Assure the program is administered according to the federallyapproved waiver. Propose, develop, and prepare operational manuals, supporting instructions and other documents for implementation of the BIW program. Recommend, develop, and implement data collection methods for the BIW program. Research, analyze, and prepare reports for this program. Consult with state and federal agencies, BIW providers, public organizations and other stakeholders regarding program implementation, administration, and quality improvements. Provide assistance, direction and consultation to BIW providers and others as needed to assure accurate information is relayed to MDHHS. Use and maintain computer databases for data collection, processing, and reporting systems for the BIW, including cost analysis and expenditure data. Recommend and implement solutions to issues that arise.
Duty 3 General Summary: Percentage: 15 Work with other department staff to develop quality websites for the MI Choice, FT, and BIW programs and assure appropriate content is posted to and maintained on each website. Assist other staff with onsite visits related to quality assurance as needed. Assist in the development and implementation of an online resource for home and community based services programs quality data, including the MI Choice waiver, ursing Facility Transition, and Brain Injury Waiver Programs. Identify quality measures to be placed on the website Assure the accuracy of the information on the website Post resources on the website and delete outdated material. Assist other staff as needed to complete mandatory quality assurance activities. Work with External Quality Review agencies to assure program data is reviewed and reported as required by each program. Recommend, develop, and implement methods to improve the quality of the programs within the section. Prepare reports and correspondence related to the quality assurance activities of the programs. Duty 4 General Summary: Percentage: 5 Perform other duties as assigned. Assist as needed to implement State and Federal policies, procedures, rules and regulations that affect home and community-based programs. The MI Choice, FT, MFP, and BIW programs are central to the Department s goal of increasing and improving the availability of Home and Community Based Services to the elderly and disabled populations and reforming long term services and supports in Michigan.
