This position is responsible for providing a comprehensive occupational therapy service for clients 18 years and over. The position involves provisions of centre based but primarily home based individual treatment programs to clients and their carers. Group treatment programs are offered either at AccessHC sites or other appropriate community facilities. The position encompasses promotion of health and wellbeing across the spectrum from prevention, early intervention through to a range of treatments for various conditions. The role is expected to foster an environment that reflects our consumer focus of client centred care that is coordinated within a multidisciplinary team approach. Position Functions Key areas of accountability Client Assessment & Care Provide holistic assessment of clients which addresses their physical, emotional and socio-economic needs. In consultation with the client and /or carer develop goals and document care plans Actively involve clients/carers in decision making about their care Provide timely feedback to other service providers outlining assessment of client and recommendations eliver appropriate and safe clinical interventions directed by evidence based practice to meet the goals and needs of the client and their carers. Identify where clients need additional support services and make appropriate referrals to other service providers both internal and external. Key Selection Criteria Relevant qualifications in Occupational Therapy and current Victorian river s licence emonstrated skills and experience in the delivery of Occupational Therapy services Experience and understanding of issues arising when working with people who are frail aged, disabled, have chronic conditions or live in a disadvantaged community group. emonstrated ability to work in a team Well-developed written and verbal communication skills.
