The Prgram cntains a technical cmpnent t aid in the cllectin and distributin f imprtant data thrugh a virtual netwrk f supprts. The City plays a valuable functin by prviding backend IT supprt and an asset mapping tl. The Cmmunity Asset Map will allw infrmatin prviders t uplad their wn datasets and link infrmatin t the City f Surrey Sustainability Dashbard; ver time; the infrmatin cllected will assist in decisin making fr varius aspects f the Prgram. Phase I includes five (5) key strategies as fllws: 1. Supprt n the G - An early childhd mental health utreach strategy; designed t meet the needs f families befre they require intensive family interventin. 2. Neighburhd Gatherings - A fcussed series f events designed t bring residents tgether and cnnect with already existing initiatives such as Ready Set Learn in partnership with the schl bard. A Vlunteer Prject Crdinatr was hired and the inaugural meeting was held in Nvember; vlunteer training will be in January fr 24 Vlunteer Peer Ambassadrs. Fraser Health is lking at a three-prnged apprach with key messages t three separate target audiences: 1) public health nurses, 2) child care educatin staff and 3) vlunteer peer ambassadr training piece. The delegatin thanked the City and acknwledged the United Way fr their lng-term sustained investment t create sustainable change within ur existing systems. The delegatin nted that they are lking at recruiting vlunteers fr the Peer Ambassadr Prgram t represent the lcal diversity in the neighburhd; added supprt will be in place in 2016 t welcme all newcmers including Syrian refugees. The Early Check-in Prgram will begin when children are 18 mnths and will strive t retain cntact with children and families wh leave the cmmunity t track the success f the prgram. Studies shw that when residents are cnnected t cmmunity resurces and supprts it makes them feel cnnected and facilitates the desire fr peple t want t stay in neighburhds fr lnger terms. 2. The fllwing cmments were made: The Divisin was first established in 2010, the Surrey-Nrth Delta Divisin f Family Practice is cmprised f family physicians practicing in the area. Partnering with Fraser Health t prvide nurse practitiners fr: The Suth Asian Health Centre - Fcuses n primary care and chrnic disease management services in a culturally tailred manner. Abriginal Health - The Abriginal Health Prject invlves a nurse practitiner wh prvides care t abriginal cmmunities that is culturally relevant. Victims f vilence (in partnership with the Surrey Wmen s Centre) - Wmen wh cme t the centre are able t receive primary care frm a nurse practitiner. The Surrey Wmen's Centre als ffers a range f Crisis, Curt and Cunselling services t victims f dmestic vilence, sexual assault, child abuse and ther frms f family vilence. The Surrey-Nrth Delta Divisin f Family Practice prpses cntinued partnerships with the City f Surrey in the fllwing areas: 1. Sharing medical practice pprtunities and recruiting mre physicians; 2. Prmting the imprtance f physician attachment and cntinuus care in cncert with City f Surrey Ecnmic Develpment Department and the Scial Plicy Advisry Cmmittee; and 3. Imprtant issues and pprtunities that the SBOT is cmmitted t, in partnership with stakehlders include: 1. Achieving new standards f ecnmic vitality and livability, 2. Develping the range f quality ecnmic, cultural and educatinal pprtunities needed t mve Surrey frward; and 3. Engaging the business cmmunity in maintaining a fcus n strategies t create a higher standard f livability fr Surrey. Key SBOT strategies include: 1. Ecnmic Develpment - Prmting Surrey as a destinatin fr ecnmic investment, retail, residential grwth, cultural experiences and livability.
