Our medical providers are focused on maintaining and improving your current health status, educating you about proactive ways to boost your health to stay well and becoming your partner in your journey to good health. Provider Information Valerie Beard, MD Dr. Beard, a board certified family practitioner, practices at New Hope Family Medicine part of Riverview Medical Group and offers expert care to patients of all ages and throughout every stage of life. She specializes in the diagnosis and treatment of acute and chronic illnesses, women s health services, pediatrics and adolescent health, preventative medicine and wellness and skin care. Lisa Conner, FNP Lisa received her nurse practitioner education from Ball State University. As a family nurse practitioner, she enjoys caring for entire families. Health and wellness has always been a part of Lisa s life as she has competed in three NCAA Division I swimming championships. She enjoys the opportunity to encourage patients to pursue a healthy lifestyle as well. Marla Guzman, MD Dr. Guzman, a board certified family physician, received her training at Indiana University and serves as the Medical Director of the WWS Health & Wellness Center. With more than 30 years of experience, Dr. Guzman focuses on prevention and health maintenance, which makes her a popular physician in the community.

Hours: Monday: 7 am-noon (7-7:30 am, lab draws only) Tuesday: 3-7:00 pm Wednesday: 6 am-11 am Thursday: 11 am-5 pm Friday: 8 am-noon Riverview Health Westfield Pharmacy Riverview Health Westfield Pharmacy is a full-service pharmacy capable of filling all your prescriptions. Additional services offered include complimentary mail order and compounding. The pharmacy is located on the Westfield Schools campus at 322 West Main Street. Hours: Monday, Wednesday, Friday: 8 am-4 pm Tuesday and Thursday: 11 am-7 pm
Appointment Scheduling Follow the below steps to schedule your appointment: 1. Go to https://westfieldwellness.acuityscheduling.com. 2. Select the type of visit and provider (if there is a preference). 3. Select a date for your visit and select a specific time from the available time slots. 4. Fill in the patient information and click continue. 5. You ll be taken to a screen where you ll need to answer a few questions that will help make your visit as smooth as possible. This form includes information such as the reason for your visit. 6. Click the finish button to confirm your appointment request. 7. You ll have the option to register for an account. This will make scheduling future appointments quicker and easier. 8. A confirmation email will be sent to the email address you provided when making your appointment.
Appointment Types Taking care of your and your family s health should be convenient. With this in mind, we strive for minimal wait times and provide an appointment length that is long enough to meet your needs, but still allows you to get back to work in a timely manner. Appointment times have been predetermined based on the care you re requesting and the time involved in meeting your request. Please note that the WWS Health & Wellness Center should not be used as a walk-in office. We re happy to work you into the schedule if possible. However, we d like to be respectful of patients with scheduled appointments. For an acute care need, including sinus congestion, cough or urinary tract infection, the time allotment is 20 minutes or one time slot. Patients who only need a lab draw or injection (e.g., flu shot, allergy shot, etc.) need to schedule a 10-minute lab draw/vaccine appointment. For a longer appointment or a new patient appointment, such as a CDL, Pap test or well-child check, you ll need to schedule a 40-minute appointment. These appointments are generally longer and require more provider time with the patient. A reminder is included on the scheduler for your convenience. Referrals The WWS Health & Wellness Center is staffed with experienced physicians and nurse practitioners who can see all age groups from infants to elderly adults. Rest assured, we re prepared to address everything from a diaper rash to diabetes or hypertension, as well as other conditions that are acute or chronic. There may be some instances where a specialist is needed to assist the provider in giving you the highest-quality care. In these cases, after conferring with you about preference, the staff would refer you to a specialist. We ll maintain open communication about your healthcare, including visit notes and any procedures or follow-up needed. What to Bring For your first appointment to the WWS Health & Wellness Center, we ask you to fill out a medical summary sheet, which includes questions about your past history, your family s past history and your surgery history, among others. It may be helpful to bring your medication bottles with you so we can accurately document the medications in your electronic medical record. If there are old medical records you d like to have included in your current record with us, feel free to bring those as well. We can also request these records from your previous medical provider if we have his or her name and your signature. Subsequent appointments will not include the medical summary sheet. Some patients may prefer to bring their medications to every visit especially if there have been any changes made from another medical provider or an emergency room visit. If the reason for your appointment is to receive vaccines, you re required to bring all childhood or appropriate adult vaccine records to your appointment.
Labs The WWS Health & Wellness Center staff is prepared to draw your blood during normal business hours. There are two medical assistants present every day, so there s time to work in this type of visit without an appointment. If you prefer to make an appointment for this service and have an additional need, feel free to schedule one time slot for your visit, and we can draw your blood during your appointment. All labs drawn at the WWS Health & Wellness Center will be sent to Riverview Health Laboratory. Results will be sent back to the provider who ordered the tests. If you see a specialist and he or she has requested blood work, we can draw your labs for you regardless of being a patient at the WWS Health & Wellness Center. The results will go back to your specialist so he or she can follow up with you. There are some common lab tests that are routinely drawn at the WWS Health & Wellness Center. If a test is ordered and it s not a part of our standard panel, we ll review with your Human Resources department whether or not the lab is a covered benefit through the WWS Health & Wellness Center. No patient information will be disclosed during this process. Vaccines Vaccines (based on age and gender) recommended by the Centers for Disease Control and Prevention, which do not include travel vaccines, will be offered in the WWS Health & Wellness Center. We recommend calling the WWS Health & Wellness Center at 317.867.6699 to make sure the vaccine you re requesting is available. You re required to bring all childhood or appropriate adult vaccine records with you to your appointment.
Prescriptions Generic prescriptions are available at the WWS Health & Wellness Center with no copayment or charge to the patient. To obtain a medication or request a refill, please refer to the following guidelines: Generic prescriptions can be filled free of charge only if you ve been seen and prescribed the medication by a provider at the WWS Health & Wellness Center. If you re requesting a medication to be filled that s been prescribed from an outside medical provider, you ll need to make an appointment with a provider at the WWS Health & Wellness Center. Additional medical records may also need to be obtained. The providers at the WWS Health & Wellness Center will not be able to write you a prescription if you haven t been seen or assessed for the symptoms requiring the medication. For refill requests, please make sure you are up to date with the necessary appointments and lab work (e.g., cholesterol testing). This may be required before a refill can be ordered. For pharmacy pick-up at a Riverview Health Pharmacy location, you ll need to submit your request 24-48 hours in advance. It ll take 3-5 business days for mail orders to be delivered to the home address on file. Pain medications are not included with the free generic medications, but they can be filled with applicable copayments. Some generic medications will continue to be available onsite at the WWS Health & Wellness Center. These medications include those that treat acute illnesses, allergies, rashes or minor aches and pains. Instead of taking a prescription to the pharmacy, these medications can be prescribed and given to you at your appointment with a WWS Health & Wellness Center provider.
