-For Residents of Kanagawa Prefecture- Make your own MY ME-BYO Record to manage your health and prepare for emergencies MY ME-BYO Record is a personal health record app for residents operated by Kanagawa Prefecture. It is free of charge, so please use it for your health management and also be ready for emergencies. It is for everyone to use 1 What is My ME-BYO Record? My ME-BYO Record is an app operated by Kanagawa Prefecture useful for your health management. My ME-BYO Record can record and manage your medication and other health information in the list or graph form via computers or smartphones. My ME-BYO Record can manage a variety of health information such as height, weight, blood pressure, number of steps, the results of health checkups, Maternal and Child Health Handbook, medications, medical history, and allergies. <PC Version> <Smartphone App Version> 1

- For Disasters - Earthquakes can occur anytime. In times of such disasters, paper record such as your Medicine Notebook may be lost. Then you will not be able to keep track of the names and types of the drugs you regularly take. If you have kept your medication data in MY ME-BYO Record, you can access it from your smartphones or PCs anytime and anywhere. Even if you can not use smartphones in an emergency situation, you can access via any PC available your medical and health data of MY ME-BYO Record stored in the server of Kanagawa Prefecture. It includes not only the names of drugs, but also other information such as emergency contacts, allegies, drug side effects and medical history, which is critical in receiving care in emergencies. By recording such medical and health information beforehand, you can receive support smoothly from doctors, nurses and pharmacists in times of disasters. You have an allergy, is that right? How neat! Your physical conditions are visualized with the record of weight, blood pressure, steps, medication and checkup results. Whenever you have your weight and blood pressure measured, take medicine or get a health checkup, make sure to feed your data into My ME-BYO Record. You can enter the data from each page of My ME-BYO Record on PCs or smartphones, as illustrated next. 2

Send an e-mail as it is without changing the title or the text. (2) A registration e-mail will be sent to your e-mail address. (3) Complete the user registration according to the directions in the e-mail received. Smartphone apps are very convenient; they automatically count steps taken daily and record information easily by scanning a QR code received from a pharmacy using your smartphone camera. [What is ME-BYO?] In general, ME-BYO is a presymptomatic state of neither ill nor healthy, a long-standing concept in oriental medicine. In Kanagawa Prefecture, we view that mental and physical conditions always change continuously between health and illness, not as two separate states of healthy or ill. Improvement of ME-BYO is not just preventing specific diseases; it is to achieve a healthier mind and body in changing mental and physical states. My ME-BYO Record will visualize health information to help improve ME-BYO. 10
