Petition for Phase 2 1. I have attended phase I for a minimum of two (2) weeks. 2. I have signed all required agreements and releases. 3. I have completed all necessary assessments and interviews. 4. I have received my JDC handbook and drug testing color. 5. I have attended court as required. 6. I have passed the handbook quiz I,, feel that I have successfully completed all requirements to move to Phase 2 of the Juvenile Drug Court Program. The following are reasons why I feel that I am ready for the added responsibility and privilege of phasing up to Phase 2: This client is officially approved for movement to the next phase of JDC

Petition for Phase 3 1. I have attended and participated in phase 2 treatment services for a minimum of Six (6) weeks. 2. I have received and signed my formal treatment plan. 4. I am employed, in school or in a training program. 6. I have achieved at least thirty (30) consecutive days of documented abstinence. 7. I have attended court as required. I,, feel that I have successfully completed all requirements to move to Phase 3 of the Juvenile Drug Court Program. The following are reasons why I feel that I am ready for the added responsibility and privilege of phasing up to Phase 3: Primary Counselor: Date:

I have co-facilitated two (2) treatment groups for at least thirty (30) minutes 6. I have achieved at least forty-five (45) consecutive days of documented abstinence. 7. I have attended court as required. I,, feel that I have successfully completed all requirements to move to Phase 4 of the Juvenile Drug Court Program. I have achieved at least sixty (60) consecutive days of documented abstinence. 6. I have attended court as required. I have attended and participated in phase 5 aftercare services for a minimum of eight (8) weeks. 3. I am employed, in school or in a training program. 4. I have achieved at least ninety (90) consecutive days of documented abstinence. 5. I have attended court as required.
