Even when appropriate clinical steps lead to a correct diagnosis, diagnostic errors may still occur due to failures in follow-up systems. Problems with follow-up and care coordination lead to poor patient outcomes, patient dissatisfaction, care team burnout, inefficiencies and redundant testing - all contributing to poor business performance. This presentation will provide you with resources and tools that will help to reduce the incidence of follow-up system failures in your organization. Following this session, the participant will: Understand causes and contributing factors of breakdowns in diagnostic care processes and follow-up systems that lead to patient injury, malpractice claims and poor business performance. Identify the risks to patients and the impact on your organization that follow-up system failures can cause. Karie Minaga-Miya, RN, JD, MS, CPHRM Senior Risk & Patient Safety Consultant, MMIC Karie Minaga-Maya is a senior risk and patient safety consultant for UMIA. With degrees in both nursing and law, plus more than 30 years in health care and risk management in hospital, clinic and other health care settings, Karie brings a powerhouse combination of expertise to her role. She is also a certified professional in health care risk management (CPHRM); a designation from the American Hospital Association. Her combined legal, nursing and administrative experience puts her in an ideal position to help policyholders address risk and safety issues, as well as work with them on early intervention to help avoid malpractice claims, and deal with issues around disclosure, clinical bioethics, error prevention and much more. In addition to being available for our policyholders, Karie helps identify risky practices that repeatedly incur liability, and collaborates on programs that help mitigate those risks. Prior to UMIA, Karie served as regional risk manager for Intermountain Healthcare, accountable to lead all the legal, risk, and compliance issues for 550 beds, 30 clinics, and over 200 employed physicians. During the past 34 years, Karie has held administrative and leadership roles in hospital, clinic and academic settings, including academic practice management at the University of Utah School of Medicine. Douglas D. Bradford, MM,PT SPEAKER BIOGRAPHIES Doug Bradford has a Physical Therapy degree from the University of North Dakota and Masters in Management degree from the University of Mary. He served as Director of Rehabilitation Services at St. Alexius Medical Center for 35 years and was a corporate trainer and mentor for new managers and supervisors. He is currently providing leadership and staff training services for businesses. Doug has had a lifelong interest in human resources, leadership and employee behavior. His expertise relates specifically to the values, beliefs and expectations of individuals in the workplace, employee behavior and the circumstances and events that drive performance. KrisAnn Norby-JahnerAttorney, Vogel Law Firm KrisAnn Norby-Jahner practices in the areas of employment law, healthcare practice management, ERISA litigation and compliance, and education law. She is a frequent lecturer on a variety of employment law and practice management topics for business owners, HR professionals, healthcare managers, and school districts. She has taught courses as a professor at the University of Minnesota Law School, Hamline University, North Dakota State University, the University of Mary, Bismarck State College, and Kent State University. In addition, KrisAnn has been four times named a Rising Star by the Super Lawyers Magazine and a Top Female Attorney by the Minneapolis - St. Paul Magazine.

SPEAKER BIOGRAPHIES Renae Moch, MBA, FACMPE Director, Bismarck-Burleigh Public Health Renae Moch, MBA, FACMPE, is the Director of Bismarck-Burleigh Public Health. In this role Renae directs the delivery of comprehensive public health programs and services for Bismarck and Burleigh County. She coordinates the local behavioral health and substance abuse task force to address issues in the Bismarck-Mandan area. Renae has worked in healthcare administration for both the public and private sector. Prior to her role as Public Health Director, she worked as a Practice Management Strategist for the American Academy of Family Physicians (AAFP) in Leawood, Kansas. In this role she assisted family physicians with practice management and compliance issues and developed tools and resources to increase efficiencies in medical practices. Renae is certified in medical practice management and a Fellow in the American College of Medical Practice Executives (FACMPE) which is the highest level of distinction in the medical practice management profession. She earned a Bachelor of Science degree in Business Administration with a minor in Business Communications and an MBA in Health Administration from the University of Mary in Bismarck, ND Buzz Sessions What is a buzz session? A buzz session consists of small groups working together simultaneously in the same room discussing issues relating to medical practice management and healthcare, thus creating a "buzz" of noise generated through discussion. The buzz sessions will provide a unique opportunity to listen, learn and share information on topics relating to your profession. Topics for discussion and participation will be available to choose from during conference registration.
