Discipline Entry/Attendance Audit v.2018.9 Discipline reporting when action assigned to the student removes them from class for even a portion of the day is required by law. Please review the excerpt below from the TSDS TEDS Data Standards. In order to assure we are reporting all DAEP, ISS and OSS disciplinary assignments, please perform the following audit. It should be done at the end of each six weeks and no less than each semester. 1) Run the Suspension/Expulsion and Attendance Audit Report a. Office/Attendance/Reports OR Office/Discipline Reports b. This report lists issues. Review and fix the issues and run it again for a clean report. c. You may have to make some notations for special circumstances on the report. Such as Early Release from DAEP, etc. 2) Run the Pre PEIMS 425 Report a. Federal/State Reporting/Reports/Pre PEIMS/425 b. Run showing ALL data. c. Clean up the errors d. Run again. 1) Suspension/Expulsion and Attendance Audit Report a. Go to Office> Attendance> Reports OR Office>Discipline>Reports b. Select the Suspension/Expulsion and Attendance Audit Report and create your template(s) by clicking the ADD button. (Once created you can modify it during the year, but you should create a new one for each school year.) NOTE: You may want to separate this report into two separate reports, one for DAEP, and the other for ISS/OSS. You can decide what works best for you. My example will show how run a combined report. c. Title your report with the year. d. Click the Ranges Button. Click SAVE. (unless you want to separate your report by grade or other parameter, you should leave the report open on the ranges) e. Set the date time frame you want your report to look at. f. Check the box Also Validate Attendance with no Suspension/ Expulsion. This will help you find missing Discipline records. g. Under the Suspension/Expulsion Types, check the bottom 3 options. In School Suspensions, Out of School Suspensions, and Interim Alternative Educational Setting(DAEP) boxes. For each one, you will also click the Select Absences Types radio button. h. Under the Available Other Types section, select the D Present in DAEP and add it to the Selected Other Types column. For In School Suspensions, use I IN School Susp For Out of school Suspension, use S Out of Sch Suspension in the Available Excused Types Click the Save button after each selection for each one. Analyzing the report The reports only lists students with possible issues. Look for the asterisk (*) in the report as it designates what does not match up. SAMPLE Report Daniel Has a Discipline Record for placement in ISS, but is listed on the report, because the wrong Absence Type code was entered by Attendance. For ISS, it should show I PI (new for 2019). Mario Does not have a Discipline record where DAEP was assigned, but does have D PD (present in DAEP) attendance entered. NOTE: There is no discipline Offense data listed on the report.) Joseph Has a Discipline record indicating an action on 8/21 for ISS, with an action detail on 8/22 of ISS served, but there is no attendance record showing that placement. Jaden Has a discipline record with an ISS placement AND the attendance code is correct I PI. He is on the report simply because a portion of the day was not coded as I PI in attendance. 2) PRE PEIMS 425 REPORT a. Go to Federal/State Reporting > Texas State Reporting > PEIMS >Pre PEIMS b. Select the 425 Disciplinary Action Record c. Add a new report template for the current school year. (Remember the year for PEIMS is the year we end on.) d. Do not enter any grades in the grade ranges. Sample Report 1) Student 1 is on the report because the Difference Reason was left blank. The student was issued 1 day and is showing served 1 day, but the difference reason was not entered as 00. a. The difference reason code is a mandatory code. 2) Student 2 is on the report because the Difference Reason code of 03 was entered for the student indicating there was a difference, BUT, the record shows 1 day issued and 1 day served, which would indicate that there should not be a difference reason.
