GRANT GUIDELINES: Recipient must be a 501(C)(3) non-profit organization as defined by the Internal Revenue Service. Recipients should be in existence for at least 3 years. No grants will be given to individuals. Recipients are required to be based or operating in the geographical areas defined in the mission statement. The grant request should relate to the health and welfare of children. Grants are generally made in June of each calendar year. NOTE: THIS SECTION MUST BE COMPLETED BY CHARITIES WHO RECEIVED A GRANT IN THE PREVIOUS YEAR IN ORDER TO BE CONSIDERED FOR A NEW GRANT. This questionnaire has been designed to assist the Grants Committee in the process of evaluating the overall impact of the funded grant awarded to your charitable organizations in the previous year. This report is the committee s primary opportunity to measure the results of the projects The Honda Classic supports, in an effort to be a more informed and strategic grant provider, while ensuring accountability for expenditure of grant funds. We appreciate your time and honest assessment of the project that was funded, including any challenges or difficulties encountered during implementation. Section 2 - Results If the project the funded grant has supported is currently on-going, please provide a detailed update as to the progress of the project, what specific impact the grant has had thus far, and how the charity plans to utilize the grant to complete the project. However, if the project is completed, please provide a detailed summary of the results and how the funded grant directly impacted the success of the project. What, if any, unanticipated results, challenges, obstacles or unforeseen successes did the charity experience during the implementation of the funded grant?
Section 3 - Tracking Describe the benchmarks and procedures the charity applied to measure the success and/or progress of the project and the utilization of the funded grant against expectations. Section 4 - Promotional / Partnerships Provide a description of how the charity sought to promote the project and enhance the visibility of the effort, and what, if any, new partnerships the charity established as a direct result of funded grant?
Section 5 - Impact How many children or families will be effected by receipt of the grant in Palm Beach and/or Martin County if awarded? Section 6 - Reach How many children or families does your charitable organization reach each year in Palm Beach and/or Martin County?
7. Media Samples / Public Relations Please provide any specific Media, Public Relations, and/or Community Relations exposure the charity earned during the process of the grant-funded project (attach any photos, articles, email blasts, audio/video clips, and/or copies of social media posts - including dates and media channels on which they appeared). American Honda Birdies for Children program is a vehicle for local charities to team up with The Honda Classic and present their donor base with a chance to win a new Honda automobile, while at the same time, the charity receives 100% of the proceeds plus a 10% matching contribution courtesy of The Honda Classic.
