I. Initial Selectee Accession Processing Steps: SUSPENSE: Completed within the next (90) days Below sets forth in detail the steps, documents, forms, and other related information needed to complete your Initial Selectee Accession processing. A. The Initial Appointment Packet: Do NOT send documents piece-meal. Send as one complete packet. Exceptions to piece mailing documents are explained in the list below. Our office will begin processing your packets shortly after they arrive. You will receive an email confirming receipt. Note: Please avoid using staples, dividers, tabs, etc. Also, do NOT send documents that have been printed on the front and back (unless the document original is printed on both sides) Mail documents to: Department of the Army Judge Advocate Recruiting Office ATTN: Reserve Accessions 9275 Gunston Road Fort Belvoir, VA 22060 1. Training Acknowledgement and Course List Form: Completion of this form does not confirm your attendance for any particular class date. The purpose of this form is to determine which class iteration (OCT, JAN or MAY) that you will NOT be able to attend and brief reason why. The goal is to get you ready for training as quickly and efficiently as possible. Knowing which OBC class does not work with your civilian employment and/or personal situation helps us determine processing priorities. You will be placed in a class on a first come first serve basis depending on the completion of your medical qualification for appoinment. If you are not medically qualified early enough to get a seat in a particular OBC class, then you will be moved to the next available class. Here is a quick run thru for the form: Quickly complete the contact information at the top of the form The next section only applies to prior service Judge Advocates (most of you will not be initialing this section) This section sets forth the expectations associated with your initial training (DCC and JAOBC). The final section lists the courses and the respective timelines associated with them. Here you will mark those course(s) that you CANNOT attend and provide a brief explanation why 2. DA Form 61 (Application for Appointment): [Samples and a blank copy are available on the download page]. Document MUST be typed. Document can be 1

hand signed (or digitally signed with Common Access Card for current military members; however, signature cannot be a typed name. Most of the information needed is self-explanatory. You will find sample forms for both prior service and nonprior service individuals. As indicated above most of you will be filling out your DA Form 61 with no prior service. Just follow along using those sample templates with your own personal information. Download a blank DA Form 61 (1B) and instructions (1B1) and a sample template that applies to your situation (no prior service) (1B2) or (prior service) (1B3). Then follow the sample template as you fill out the blank DA Form 61. 3. DD Form 368, Conditional Release (Current Service Military ONLY) Complete Section I, 1 to 3(e). JARO will then complete #4. See download instructions (1C1) for details. - For enlisted members: this form will not be processed through your unit until you have been deemed medically qualified. - For officers: this form will be processed through your unit based on when you will be attending your initial training (DCC and JAOBC). 4. DA Form 3574: if you have no prior military service or less than 8 years of prior military service. (This form may print out as 3 separate copies. We only need 1 copy.) Most of you will be filling out and submitting a DA Form 3574. Please read through and complete the signature block at the end of the form. The form can be hand signed or digitally signed (Common Access Card for current military members only); however, your signature cannot be a typed name. 5. DA Form 3575: if you have more than 8 years of prior military service. (This form prints out as 3 separate copies. We only need 1 copy.) The form can be hand signed or digitally signed (Common Access Card for current military members only); however, your signature cannot be a typed name. INITIAL DOCUMENTS AND FORMS 1. 6. Religious Practice Accommodation Statement of : Please read through and complete the signature block. This document will need to be printed out and hand signed. 7. Statement of Clearance: if you do not have a security clearance, please review and complete the signature block. This document will need to be printed out and hand signed. (OR) If you have one, complete the statement and also provide a signed letter from your Security Manager as to your current status and how long it is valid for. 8. Certificate of Good Standing: An original copy of your Certificate(s) of Good Standing will be required for each bar that you are registered to practice. This includes both active and inactive bar status. Photocopies or scans are not acceptable. This may take some time to secure, so make sure you plan accordingly. If you are not yet a member of the bar, you will mail this document separately once obtained. NOTE: You must be a member of a bar before you can be appointed in the USAR JAG Corps. You will also not be placed into a specific initial training class until you have passed your bar and provided us with your certificate of good standing. For planning purposes, here is the timeline suspense for each of the three (3) training classes (OCT/JAN/MAY): YOUR CERTIFICATES OF GOOD STANDING MUST BE RECEIVED BY THIS DATE IN ORDER TO BE ELIGIBLE FOR THAT CLASS OCT 2017 (204 th ) OBC Class: No later than MAY 15, 2017 JAN 2018 (205 th ) OBC Class: No later than SEP 1 st, 2017 MAY 2018 (206 th ) OBC Class: No later than DEC 15 th, 2017 9. Law School Transcript(s): Must be final transcript with date the degree was awarded/conferred. This must be an original copy or registrar certified copy of your final law school transcript. Photocopies or scans are not acceptable. If you used digital transcripts for your application, you will need to request official hard copy transcripts. You may send the previously opened transcripts (if applicable) that you used and scanned as part of your online application provided they meet the above noted standard(s). 3Ls only if you have not graduated, send a copy of the transcript provided with your online application; your final transcript can be mailed separately after matriculation. 10. Undergrad Transcripts(s): Must be final transcript with date the degree was awarded/conferred. This must be an original copy or registrar certified copy of your final 3

law school transcript. Photocopies or scans are not acceptable. If you used digital transcripts for your application, you will need to request official hard copy transcripts. You may send the previously opened transcripts (if applicable) that you used and scanned as part of your online application provided they meet the above noted standard(s). 11. Birth Certificate or Citizenship Papers: Original copies or certified copies of either your birth certificate or citizenship papers will be required as part of your hard copy file. This can sometimes take a while, so plan accordingly. This document will not be returned to you, so do not send your only copy. 12. Age Waiver Request: (if applicable) This is the same waiver that you prepared for your application just in hard copy form. You should already have it handy so no additional work is needed here. 13. Arrest-Conviction Waiver Request: (if applicable) This is the same waiver that you prepared for your application just in hard copy form. You should already have it handy so no additional work is needed here. 14. Statement of of New Original Appointment: Submit ONLY if you have prior commissioned military service. This statement sets forth your understanding that your rank in the JAG Corps will supersede any previous date of rank you held prior to entering into the Corps. For some of you that may include an adjustment to your date of rank to ensure you are competitive for your next JAG only promotion board. 15. Statement of of Ineligibility for Retirement Pension in the US Army Reserve: Submit only if you are age (40) or above. Mandatory retirement age in the reserves is at age (60). Those individuals who cannot reach (20) good years before their (60 th ) birthday will have to acknowledge that they are not eligible for a reserve pension. This only applies to the pension portion of your retirement. You would still be eligible for all other retirement benefits available at the time of your retirement from reserve service. Please Note: There are current changes in the works which may or may not impact your eligibility for a retirement-based pension and other related items. However, as of the drafting of this information paper, those details have not been finalized or published to the field. Just keep in mind that these initiatives may change your retirement eligibility in the future. 16. Copy of Physical Health Assessment: Only if you are currently serving in the military as a commissioned officer. Current Service Enlisted/NCOs must obtain a commissioning physical either through MEPS, their servicing Military Treatment Facility (MTF), or the DODMERB system. 4

17. Request to Waive DCC and/or JAOBC: (Only if you are currently serving or had prior service in the military and have attended a commissioned officer basic course). Use the template found on the download page and attach all applicable military training certificates, ORB, and other related documentation covering deployments and similar training/real world experiences.
