Call for Nominations for the 2018 CHA Digital Innovation in Health Award Enhancing the experience of patients and aged care consumers through Technology Following the success at the 2017 CHA National Conference of the Arts in Health Award, HESTA is again partnering with Catholic Health Australia to sponsor a new Digital Innovation in Health Award. The Award finalists will be invited to present at the 2018 National Conference, 27-29 August 2018, in Melbourne, where the Award winner will be judged and announced. Criteria: Your project or initiative needs to have been live and in use sometime between 1 January 2017 and 30 June 2018. Your project or initiative needs to have been activated or provided at one or more facilities or services conducted by Catholic Health Australia member organisations. Aims of the Digital Innovation in Health Award: This Award aims to acknowledge projects or initiatives which enhance the care or experience of patients and/or aged care consumers through the use of IT. Digital Innovation in Health is the application of IT initiatives in health promotion, service management, ageing well, preventative health, delivery of health care and aged care and also within a health or aged care setting. Call for Nominations for the 2018 CHA Digital Innovation in Health Award Enhancing the experience of patients and aged care consumers through Technology Award Criteria Your nomination should be able to demonstrate innovation through digital solutions or IT to achieve success in two or more of the following criteria: 1. Engaging the patient/consumer in their recovery and quality of life. 2. Enhancing the patient/consumer experience. 3. Utilising IT to improve the quality of health care or aged care delivered. 4. Better patient or consumer outcomes through the application of digital solutions. 5. Using digital innovations in health prevention education and activities. 6. Promotion of health and wellbeing for patients, consumers and the community. 7. Promotion of Mission and/or values of the health or aged care service provider in patient/consumer interactions. 8. Evaluation of the impact or outcomes of the Digital Innovation. Award Application Please submit your nomination via the form on the following pages by the due date of Friday, 3 August 2018. All submissions will be judged and shortlisted submissions will be invited to present at the National Conference in the Digital Innovation in Health Plenary Session on Wednesday, 29 August 2017 at 9:30 am before a judging panel of three who will select the ultimate winner of the Award. Each shortlisted nominee will have 10 minutes 7 minutes to present their project/initiative and 3 minutes for Q&A with the judges. Presentations may include visual/audio media. Please note that those shortlisted who wish to attend other conference sessions will be required to purchase a registration. The winner will be presented with a trophy and a prize of $5,000 to further Digital Innovation in Health initiatives or professional development with the winning Catholic health or aged care provider. Nomination Form Part Five Results Results: (max. 800 words) For example, show evidence of how the project/initiative has improved outcomes for patients, residents or consumers. This may be either direct or flow-on depending upon the nature of the project. For example, how medication management was improved, how operating times were reduced, how workflow practices improved as a result of using the tech, etc. Also relevant will be how sustainable and cost effective the project/initiative is, whether it has been evaluated, how many could benefit from it, and so on.
