nsdestory News from Unversty College London Hosptals NHS Trust May 2004 Natonal Smle Week How to keep your teeth sparklng whte Desgnng a healthy hosptal THE MIDDLESEX receves a royal vstor plus Wn a Champagne meal for two Unversty College London Hosptals

In bref Here s what s been happenng at UCLH n the last month. Executve voce s the new column n Insde Story whch wll gve you the opportunty to get to know the Trust s executve team a bt better. Ths month, we hear from Davd Amos, drector of workforce. Movers and shakers Here are four of our latest recruts at UCLH : Our new outpatent appontment call centre wll rval anythng the prvate sector has to offer f Ethan Cuthbert has anythng to do wth t. Formerly grants manager at the Alzhemer s Socety at the NHNN, Ethan s the new call centre manager. He sad: Ths s a good opportunty to help develop new processes wthn the NHS. I am lookng forward to mprovng the outpatent experence. Joan St Hll has joned UCLH as assstant drector of nformaton management and technology. A key part of her role wll be to manage the outsourced IT support servces contract to ensure users receve the support they need and that the Trust s gettng value for money. Joan has moved to UCLH from Network Ral where she was a programme manager. She sad: There are a lot of exctng changes gong on at the Trust and I look forward to beng part of them. Emma Morgan has traded Morrston Hosptal n Swansea for the brght lghts of London, havng recently joned us as a senor II radographer n MRI (magnetc resonance magng) at the Mddlesex Hosptal. Emma sad: I was keen to jon UCLH because t was a MRI role and that really nterests me. I m also lookng forward to workng n the new hosptal. Emma Lonel wll be the frst frendly face to welcome patents attendng the Qun Unt at the RLHH. As the unt s new receptonst, Emma wll be on hand to answer patents queres, book ther appontments and carry out day-to-day admnstraton dutes. She sad: I m really enjoyng knowng I can help patents. I m also lookng forward to movng back to the Queen s Square ste wth everyone once t s fully refurbshed. Complementary cancer care assessment clnc opens The UK s frst complementary cancer care assessment clnc has been launched at the Meyersten Insttute of Oncology at the Mddlesex Hosptal. Ths monthly clnc wll advse patents wth cancer on the approprateness of complementary treatments and how they can be best ntegrated nto ther conventonal care. No treatment s gven at the clnc, but those patents who are beleved sutable for complementary treatment wll be referred on to the Royal London Homoeopathc Hosptal (RLHH). The clnc has been developed by Sose Kassab, drector of complementary cancer servces at the RLHH, together wth Kay Eaton, nurse consultant/lead cancer nurse, and Alson Jones, clncal drector, cancer servces. Sose sad: There s a lot of uncertanty among patents about complementary treatments, especally wth the nformaton (and msnformaton) avalable on the Internet. Ths clnc wll gve patents a chance to have a realstc dscusson about the approprateness of addng these treatments nto ther care. Kay added: UCLH has a tradton of usng complementary treatments to help patents wth haematologcal malgnances. We want to look at ncreasng the avalablty of such treatments for patents of all ages and wth all types of cancer. Vascular outreach nurse apponted Sose Kassab and Kay Eaton To fnd out more, contact Sose on 020 791 88 or Kay on x925 Carolyn Morrce has been apponted as a vascular outreach nurse to promote a patent-focused approach to care of people wth perpheral vascular dsease (PVD) n partnershp wth GPs, the hosptal team and other healthcare professonals. She s workng alongsde vascular consultant, Edde Chaloner. PVD s the narrowng or blockage of the man arteres takng blood to the legs, whch can ncrease the rsk of heart attack and strokes. Carolyn sad: PVD s a bt of a poor relaton to coronary heart Vascular outreach nurse Carolyn Morrce dsease. My job s to rase awareness n the hosptal and the communty about PVD and help reduce the workload for consultants n clncs and GPs as well as mprovng patents care. Carolyn wll be targetng GP practces to fnd out current procedures for assessng patents wth symptoms of PVD and whether there s a need for educaton or support. In the future, t s hoped that a nurse-led clnc wll be developed to assess patents wth early symptoms of PVD. The Mddlesex receves royal vstor UCLH chef executve Robert Naylor, greets HRH Prncess Anne. Also shown Professor Malcolm Grant, Provost, UCL (far left) and UCLH charman Peter Dxon, (second rght) In her capacty as Chancellor of the Unversty of London, HRH Prncess Anne vsted the Insttute of Nuclear Medcne at the Mddlesex Hosptal last month. The focus of the vst was the PET/CT scanner. Professor Peter Ell, the nsttute s head of department and clncal drector for nuclear medcne sad: HRH Prncess Anne s vst was hghly sgnfcant as t underlned the successful co-operaton between UCL, (whch offered space for the faclty) the Trustees (who funded t) and the Trust, whch has lent ts support to the ntatve. Our PET/CT scanner, nstalled n January 2002, was the frst of ts knd n the UK, and has profoundly changed the management of patents wth cancer. Pharmacsts to prescrbe medcaton for the frst tme at UCLH from left to rght, Dr Robert Urquhart, head of pharmacy, wth Balram Malhotra, Dpty Josh, Becky Whte and Rob Shulman Four clncal pharmacsts at UCLH have successfully completed tranng allowng them to prescrbe medcnes for the frst tme. Becky Whte (nutrton), Rob Shulman (crtcal care), Balram Malhotra (rheumatology) and Dpty Josh (nephrology) have all qualfed n the frst wave of pharmacst supplementary prescrbers (SPs) n England. Ther qualfcaton means UCLH has the largest number of pharmacst prescrbers of any acute trust n the country. Supplementary prescrbng was recently ntroduced by the Government to extend the range of staff who can prescrbe drugs, thereby provdng patents wth qucker, more effcent access to medcnes. Pharmacst prescrbers work n partnershp wth a doctor to prescrbe medcatons for patents who have already had a condton dagnosed. The patent must agree to the supplementary prescrbng arrangement. Dr Robert Urquhart, head of pharmacy, sad: Supplementary prescrbng fts n wth our vson for extendng the roles of clncal pharmacsts at the Trust. It allows patents to beneft more fully from the sklls of our hghly capable clncal pharmacsts who are now able to prescrbe medcnes n ther specalst areas. Supplementary prescrbng s also about mprovng the qualty of care snce patents wll see ther pharmacst regularly and can ask them for nformaton and advce. Snce I joned the Trust n February ths year as drector of workforce I have set a goal of makng UCLH the employer of frst choce n healthcare. Ths s already the case n many parts of the Trust t needs to be so everywhere. The staff survey showed that staff at UCLH have a postve atttude to work and that they thrve n an organsaton that has strong leadershp and a clear drecton of travel. We also know that staff work hard and juggle many competng pressures to get the job done. The survey also told us that staff want to see more access to better tranng as well as greater staff nvolvement n the decson makng process. Why s t mportant that UCLH becomes the employer of frst choce? Because ths s how the Trust can delver our vson of a world class future. We am to develop a workforce that has the capacty, dversty, sklls and confdence to dagnose, treat and rehabltate the complex range and number of patents who have chosen, or been referred to, UCLH. To acheve ths vson we must operate wth vrtually no long-term vacances, and lmt the gap between posts beng flled when staff do leave. We must compete for, attract and keep the best-qualfed people and then create a workng envronment where ndvduals want to develop and advance ther careers. So what are we dong to become the employer of frst choce? The full mplementaton of Agenda for Change (see pages 4 & 5 for the latest news) and the new consultants contract wll mprove pay and condtons and encourage staff to take on more responsbltes and secure for themselves more rewardng jobs and careers. UCLH s workng towards becomng an employer where ndvduals can fully develop ther career. For too long, partcularly n London, anyone seekng a promoton smply moves to a neghbourng NHS trust. Ths s not always good for the ndvdual or the servce. We need to develop and promote sophstcated, state of the art and personalsed career, support, counsellng and lbrary servces for staff. How do we measure evdence of progress and achevement? I don t want to talk about aspratons, I want to celebrate reachng targets ncludng mproved attendance rates, reduced vacancy levels, less temporary staffng, better tranng and development opportuntes. Improved workng lves. Above all, greater satsfacton and real progress for those workng here. 4 A modernsed NHS pay system Agenda for Change s the bggest change to the pay of NHS staff for 50 years and wll affect all staff except doctors, dentsts and some senor managers. It wll put nto place a new scheme of pay and condtons for staff and wll replace Whtley councls. Eght natonal pay bands are beng ntroduced wth each staff member beng allocated to the new bands through the NHS job evaluaton scheme whch wll measure the sklls, knowledge and responsbltes that staff need n ther post. Your questons answered Why s Agenda for Change so mportant? The new system s the bggest pay reform n modern Brtsh ndustral relatons hstory and wll affect more than one mllon staff. Agenda for Change works to harmonse condtons of servce for staff across the NHS, provde a more transparent system of reward for staff workng flexbly and help to create the condtons for new knds of jobs. For the frst tme, staff wll be pad on the bass of the jobs they are dong and the sklls and knowledge they apply to ther job. When wll Agenda for Change start? Transfers to the new pay bands should take place from 1 October 2004 however local agreements are lkely to be made whch allow a seres of dates for staff to move to the new system by Aprl 2005. All payments wll be backdated to 1 October 2004. All staff sde organsatons have consulted ther members and many have sgned up to the agreement, but UNISON, AMICUS and SOR (radographers) have decded to consult agan followng fnal reports from those organsatons dentfed as Early Implementer stes who have been plotng the system snce June 200. mproved recrutment and retenton the development of new and changng job roles to support servce modernsaton more patents beng treated qucker and recevng better qualty care as a result of more staff workng n new, patent focused roles The key ams of the new system are: How wll the new system work? All staff wll be put n one of eght pay bands based on a job evaluaton process whch consders 16 factors wthn each ndvdual s job, ncludng the knowledge and sklls requred, responsbltes and the freedom to act. Pay wll ncrease n annual steps from mnmum to maxmum n each pay band. Ths wll be lnked wth the Knowledge and Sklls Framework (KSF) whch s a new system to support personal development and career progresson. How wll my job be assessed? A job evaluaton scheme, whch has been desgned for the NHS, wll be operated through local job matchng and job evaluatng panels comprsng traned staffsde and management sde representatves. More than 200 job profles have been developed natonally and wherever approprate jobs wll be matched to one of these. Based on the experence of the Early Implementer stes, t s antcpated these wll cover around 60% of jobs and the remander wll need to be evaluated locally through a natonally agreed process. Is there anythng I can do now to prepare for Agenda for Change? Every member of staff should ensure they have an up-to-date job descrpton and person specfcaton that has been agreed by the ndvdual and ther manager. Further gudance on the process for revewng job descrptons can be obtaned from the HR department. mprovng all aspects of equalty and dversty especally n the areas of career and tranng opportuntes ensurng equal pay for work of equal value mproved staff morale. What happens next? A project delvery board s beng establshed at UCLH that wll nclude the drectors of workforce, fnance, nursng and communcatons, board and corporate servce representatves, together wth staff sde leads. The Trust s also appontng a project manager to lead and co-ordnate the project. Where can I fnd out more? If you have any queres regardng Agenda for Change or are nterested n becomng nvolved, please contact Jane Ltster on 916 or John Golden va emal. Informaton about Agenda for Change terms and condtons of employment can also be found on the Department of Health s webste at www.dh.gov.uk You can fnd out how the Early Implementer stes are gettng on at the Modernsaton Agency at www.modern.nhs.uk/agendaforchange Detals of the project launch to be held n early June wll be advsed through team bref. Informaton wll also soon be avalable on the UCLH webste together wth regular updates to be provded through a monthly Agenda for Change newsletter. Workng together Implementng Agenda for Change at UCLH wll requre unprecedented levels of staff nvolvement. Ths new system s founded on the prncple of partnershp workng by NHS staff representatves and NHS employer representatves. Jane Ltster, assstant drector of HR, sad: We need staff from across the organsaton to become nvolved n a number of ways n ths massve project, from becomng traned job matchers and evaluators, to sttng on workng groups agreeng arrangements for the mplementaton of new terms and condtons. John Golden, char of staff sde, added: Agenda for Change provdes a real opportunty for us to work n a dfferent way together and nfluence the outcome of pay and terms and condtons ssues. But we do need unon members to come forward and become nvolved n the project to help spread the load. Learn a new skll Two partnershp pars (one staff sde lead and one management sde lead) have already undertaken job evaluaton tranng and wll now tran others at UCLH n the new system. These pars wll be runnng tranng programmes across the Trust durng the summer for staff nterested n becomng job matchers and evaluators. I put myself forward to become a traner because I wanted to fnd out more about Agenda for Change for myself and so I can nform other staff members and help them prepare for ts ntroducton. I thnk there s a lot of confuson around Agenda for Change and t s mportant for staff to have someone on ther level who s able to explan t to them. I also see t as a real opportunty for my personal and career development. Karen Devenney, modern matron for neurology, wll also be tranng staff n job evaluaton From a nursng perspectve there have been a lot of queres about Agenda for Change and n my role as modern matron I wanted to be able to dscuss the new pay deal wth staff and support them through the process. It s gong to be the bggest radcal shakeup of pay reform n the NHS for 50 years and I felt t was mportant to be nvolved. The revew s beng undertaken Trustwde and therefore offers good networkng opportuntes. My mentor s very supportve and helps to advse and motvate me. I feel that the mentorng has helped gve me confdence n knowng that I m movng forward on a personal level as well as a professonal level. Antt Kesalaht, staff nurse, John Young ward, NHNN I have recently changed my shfts to 12 hours and ths means I can have four days off durng some weeks. I fnd ths shft pattern much easer to work around as t gves me more tme to complete jobs, more tme to do plannng and my patents are also happer because they have fewer staff changeovers. Barbara Macdonald, mdwfe, EGA I thnk the 52 Club s absolutely fantastc! It has brllant facltes and t s so cheap. I regularly go after work to one of ther ftness classes and fnd t s a brllant way to relax and get ft. Jen Goddard, contact centre project manager I fnd t really helpful to know what s gong on n the organsaton and the face to face team brefng s a very frendly way of communcatng ths. We have recently had an away day for the outpatents department whch was also a good way to get to know new faces n the team. Polly Nye, staff nurse, the Heart Hosptal Flexble hours are fantastc! I have recently moved from dong shft work to workng nne to fve, Monday to Frday. The Trust s also helpng me to do my professonal nursng practce degree course by gvng me one day off a week for studyng and by provdng fnancal support. 5

6 Countdown to the new UCH: www.uclh.org/newhosp Improvng the hosptal experence through good desgn by Rachel Basha, nteror desgner, Llewelyn Daves Everyone recognses ether conscously or subconscously whether or not they feel good or bad n a partcular space. Hstorcally NHS hosptals have not created good envronments to ether be treated, vst or work n. Thnk of hosptals wth low levels of natural lght, poor ar crculaton, cluttered corrdors of medcal equpment and trolleys, cluttered sgns to drect you through warrens of endless corrdors, fluorescent lghtng and a propensty to use bege and magnola on all walls. Ward Recepton showng floor patterns that provde a sense of arrval and communcate the publc nature of the space. Desgnng hosptals to feel less nsttutonal and more conducve to healng s very challengng consderng the practcal constrants of such envronments. Experenced archtects and nteror desgners have used ther sklls to combat the challenges and hopefully make the new Unversty College Hosptal (UCH) feel postve wthout reducng the effcency and effectveness of the medcal servces. There are a number of key nterrelated desgn ssues n the new UCH whch wll provde a better hosptal envronment. Good plannng Good plannng allows the patents, staff and vstors to crculate around the hosptal n a logcal, effcent manner wthout unnecessary Proposed Man Entrance Foyer stress or delay. The new UCH benefts from havng a very clear and dynamc entrance foyer where vstors can easly dentfy the way nto the buldng wth the A&E entrance mmedately adjacent. From the man entrance foyer vstors are drected to the man lft core and the varous departments. The new UCH accommodates a maxmum 60 beds on each floor of the tower wth patent areas on the outsde and treatment, support and staff areas around the core. See next month s ssue of Insde Story for more nformaton on the desgn of npatent floor plates. Colour, Fnshes and Fttngs The selecton of colour, fnshes and fttngs are vtal to creatng an envronment where Outpatent watng areas. Maple veneer s used on the doors throughout the hosptal to reduce the clncal atmosphere. 12 11 to go 10 9 8 7 6 5 4 2 1 0 months patents and staff feel valued. Qualty fnshes and fttngs communcate a level of professonalsm and care. Ths can have an enormous mpact on the emotonal state of patents, staff and loved ones. The new UCH uses materals that restrct the spread of nfecton as well as creatng a less clncal envronment. Art The postve effects art has on well-beng and the healng process s now wdely documented. The Slade School of Art workng n conjuncton wth archtects Llewelyn Daves, has been assgned to develop art for the new UCH along wth the development of a long-term strategy for art throughout the hosptal. www.uclh.org/newhosp/art Teenage Cancer Ward proposal. Ths floor s funded by the Teenage Cancer Trust Envronmental Qualty The plannng of the new UCH consdered carefully the provson of qualty of ar, lght and nose levels. All patents beneft from wndows on the hosptal permeter that provde natural lght and the upper levels also beneft from panoramc vews all over London. Acoustc consderatons are mantaned for all rooms to ensure that patents are not dsturbed. Lght qualty n patent wards s ndrect and dffused to provde a restful atmosphere. Electronc Patent Record EPR demo goes lve For the frst tme, an on-lne UCLH workng verson of the EPR (on a local networked computer) s avalable whch means that the EPR team, plus key users, can start to have a detaled look at how the admnstratve facltes (currently performed by the PAS) wll work n practce. Nursng staff jon EPR team The EPR system s an off the shelf product, but UCLH s a complex Trust, so work s needed to ensure that the fnal system meets our needs. Suzanne Morkus SEC ntervew Rasng awareness of the benefts of good oral healthcare was the am of ths year s Natonal Smle Week (16 22 May) organsed by the Brtsh Dental Health Foundaton. Suzanne Morkus, tutor dental nurse n the School of Dental Nursng at the Eastman Dental Hosptal, tells us about her role and how to keep teeth sparklng whte. headlnes As the people best placed to know what s gong to work are those on the front lne, staff across all grades are nvolved n the project. Latest recruts are nurses Mara Rajwer and Helena Azzam who have joned the team as EPR analysts. Mara Rajwer, former cardac nurse at the Heart Hosptal, s lookng forward to her new role. I wll be focusng on process revew for clncal areas (.e. lookng at current workng patterns and clncal paths and how EPR wll change them), and assstng n creatng the clncal system. It s a great opportunty to use my clncal sklls and knowledge n a dverse way to support EPR mplementaton. It also allows me to consoldate my current studes - an MSc n healthcare technologes. Helena Azzam was happy as a sster on the neo-natal unt (NNU) at the Elzabeth Garrett Anderson and Obstetrc Hosptal (EGA/OH) but couldn t resst the challenge of jonng the EPR team. When PAS was frst ntroduced, I remember beng mldly excted that we could look thngs up nstead of What do you do? I have been teachng n the School of Dental Nursng for nearly fve years. I frst came to the EDH n 1976 and dd my own tranng at the school! I teach on the pre and post qualfcaton courses for dental nurse students whch nclude oral health educaton and nformaton about how to keep your mouth healthy. What do you lke about your job? It s so vared. It s also very rewardng to see the students qualfy especally those who have found the tranng a struggle. One of my tutors really nspred me when I was tranng I hope I am now dong the same and teachng tutors of the future. What are the man challenges of your job? Keepng up wth the paperwork and the markng. What are your top tps for keepng teeth whte? Havng a good oral hygene regme s very mportant. Make sure you brush your teeth effectvely twce a day usng a fluorde toothpaste, try and restrct your sugar ntake to mealtmes only, vst the dentst regularly for a check up at least once a year and avod smokng, red wne, AN UPDATE ON THE ELECTRONIC PATIENT RECORD (EPR) PROJECT: havng to wrte down chemcal pathology results n the results book before they were transcrbed nto the notes. Our NNU database, plus the clncal data repostory made thngs even better. Followng PAS tranng, I became fully electronc, tranng our ward clerks and workng on some deas wth one of our specalst regstrars. Now the future wth EPR looks so full of promse. I want to be n on t and then brng t back to NNU and the EGA/OH. The best bt s that fnally the users are beng allowed to help n the desgn of t. coffee and strong tea as they cause stanng. If somebody s serously thnkng about havng ther teeth whtened, they should seek professonal advce and not use cosmetc products bought over the counter. Why s smlng mportant n your job? A dental nurse should have a smle on ther face because t can go a long way towards relaxng a nervous patent. It also takes fewer muscles to smle than t does to frown so you wll have fewer wrnkles! What are you most lkely to say? I need to do my flng! What are you least lkely to say? All my flng s done! What work-related thng could you not do wthout? I couldn t be wthout my colleagues n the School of Nursng. MAY 7
