CONFIRMATION OF FULL DISCLOSURE I, (Print Name), certify that I am aware that I have a continuing obligation to provide full and complete disclosure of criminal or disciplinary information _to the law school and the bar. I have carefully considered whether I need to provide further information to the school. Specifically, I have considered whether I may have failed to include any information in my application: That occurred while I was a juvenile. That involved disciplinary actions at educational institutions, whether regarding academic matters (i.e. academic dishonesty) or conduct issues (i.e. underage drinking; possession of alcohol in violation of school rules; fighting or disorderly conduct). Because I was not found guilty. I understand that full disclosure involves reporting the charge to the law school and the bar, with information regarding the disposition, whether that charge resulted in a conviction, a guilty plea, a nolo contendre plea, probation before judgment, or a requirement that conditions (such as community service or substance abuse education) be fulfilled as part of a diversion or alternative sentencing program, or a dismissal. That I took steps to expunge from my record. I understand that the law school and the bar are inquiring whether I have ever been charged and not whether an item is on my record. That involved citations (including traffic charges of certain types), whether or not there was an arrest. Any situation that involved alcohol use, drug use or property damage must be reported even if the charge was presented in the form of a traffic or other citation. Charges of "reckless driving" are reportable, as are any situations that result in revocation or suspension of a driving license. Parking tickets and minor moving violations are not reportable to the school. I have carefully considered whether I have met my obligations as I now understand them. If I need to add information, I will immediately contact Dean Crowley and seek to amend my application.
