Focus + Approach

We will live out loud - Values Accountable & Transparent Collaborative Listen & Learn Evidence-Informed Inclusive Engage Lead Compel Action

How do we improve our population s health? Collaboration Systems Change Social Determinants of Health Collective Impact Improvement

Population Health with a Social Determinants of Health Lens Why are some people healthy and others not? What are the best ways to intervene that improves the most people s lives? What are the best ways to reduce costs? Framework Adapted from the University of Wisconsin Population Health Institute Model

Foundation Geographic Data and Focus Data drives geography of focus All of St. Petersburg and South unincorporated Pinellas County

Listening Sessions What did we ask St. Petersburg s residents? 1. What is your personal experience with healthy living? 2. What is your experience accessing support for healthy living? 3. What barriers do you face in seeking healthy living? 4. What will it take for St. Pete to be the Healthiest City in the US? 5. What is your personal commitment to healthy living?

Listening Sessions What did we ask St. Petersburg s providers? 1. How should we promote community wellness? 2. What should the Foundation focus on first? 3. What should the Foundation not do? 4. What conditions are required for meaningful collaborations? 5. What conditions are missing to support the best collaborations?

Listening Sessions What did we hear from residents? Health disparities by location, race, class, age Many residents expressed this sentiment Food is expensive! Especially healthy food! Grandparent quote many residents echoed cost/availability of food Cannot afford health care Single mom in college repeated by many residents Generational Apathy coined by a resident a loss of residents who care about the community. Residents want the Foundation to bring the community together. Motivate more residents to be involved.

Listening Sessions What did we hear from providers? Break down silos to help the community work together and have an impact Build trust between the Foundation and the community improvement requires buy-in and trust. Transparency, inclusiveness, motivational support, and highlight successes. Lead we re missing leadership with a respect for history and existing community assets. Be neutral and an honest broker. Never stop listening.

Ready to begin PLEASE NOTE!!! This is a presentation we have not yet released the Request for Proposals (RFP). The RFP is the only guiding document. We are orienting all of us to our approach in this presentation. The RFP supersedes any other instruction.
