REGISTRATION INFORMATION Registration forms and payment must reach CSULB at least fourteen (14) days prior to the date of your first scheduled workshop. Each workshop listed in this catalog will contain its own unique deadline date. Deadlines for registration and/or payment of the pre-packaged workshops are based on the date of the first workshop in the series. COMMUNICATION CEU Program staff will send communication by way of email. It is imperative that you provide an active email address. You should note that many ISPs and institutional email administrators automatically block, delay, or junk-folder email from CSULB. Please check your junk folders often or add CSULB to your safe email list. PAYMENT The only acceptable form of payment is by personal check or money order made payable to CSULB Foundation. Payment should be mailed to: Deborah Repp CSULB School of Social Work, SSPA-161 1250 Bellflower Boulevard Long Beach, California 90840-4602. LATE FEE A late fee of $25.00 will be assessed for all missed deadlines. Contacting the office to indicate your intent to register does not eliminate the assessment of late fees. The deadline to register and pay for any workshop is fourteen (14) days prior to the workshop. In the case of pre-packaged workshops, the deadline is fourteen (14) days prior to the first workshop in the series. CONFIRMATION/RECEIPTS CSULB will send you a confirmation email/receipt for payment after your registration and payment has been processed. All workshops will take place within the CSULB campus; however, the room location cannot be confirmed until the Monday prior to the scheduled workshop. This email will indicate which workshop(s) you registered for and how much you paid, including any late fees. Please review it carefully for accuracy. A certificate of completion will be issued at the conclusion of each workshop. Only the hours you are present can be credited, therefore, if you arrive late or leave early your certificate will be adjusted accordingly. CANCELLATION/POSTPONEMENT OF WORKSHOP BY CSULB There may be times when CSULB will need to postpone or cancel a workshop. CSULB staff will make all reasonable efforts to notify you by email at least seven (7) days prior to the workshop date. If a workshop is postponed you will be offered a choice of attending on the new date or receiving a full refund for that workshop, including any late fees you paid for the rescheduled workshop. If a workshop is canceled you will be given a full refund of all fees, including any late fees you paid for the canceled workshop. If you paid for a series of workshops your refund will be calculated based on the discount rate you received. REFUND/NON-ATTENDANCE POLICY Due to the nature and cost of processing registration fees, refunds will not be issued for workshops that do take place. However, you may request a rain check for any missed workshop due to unforeseen circumstances. Your request must be made within seven (7) days of the missed workshop and can either be good for the next time that workshop is given (usually in the next academic year), or can be applied to other CEU workshop sessions within this academic year. Additionally, it will be your responsibility to contact CEU Program staff for information regarding future workshop dates and to submit a new registration form at the appropriate time. Page 4

SPECIAL DISCOUNTS Discounts for individual workshops are available for CSULB faculty, staff, field instructors, alumni and MSW students. Additional discounts are available to anyone registering for either the Pre-Licensure Package or the Post-Licensure Package. Please see registration form for details. LOCATION The workshops have all been held in the Foundation Building, Room 175. This is a conditional reservation that we have set up. Due to the slight possibility that the Training Center we are using may have a last-minute need for the room, we will send everyone an email verification of location (either F-175 or a room in the SPA building) by the Monday before the workshop.
