Request fr Annual Educatin, Incme & Health Funding Prpsals FY2018-2019 INTRODUCTION United Way f the Battle Creek and Kalamaz Regin (UWBCKR) envisins a vibrant cmmunity where all peple realize their full ptential. UWBCKR leads shared effrts by engaging diverse peple, ideas and resurces. UWBCKR seeks Innvative/Interim and Supprtive funding prpsals frm eligible rganizatins cmmitted t imprving cmmunity utcmes under ur impact areas f Educatin, Incme, and Health. In additin t ur wrk in Basic Needs, we are fcused n three cmmitments t ensure every persn can learn mre, earn enugh and live a healthy life: Educatin: Imprve high schl graduatin rates and reduce racial and ecnmic disparities in graduatin rates. Incme: Increase the number f ecnmically stable husehlds in the regin. Health: Imprve family and infant health and reduce racial and ecnmic disparities in related indicatrs. Additinally, UWBCKR is fcused n investments in prgrams that will demnstrate bth the ability t make measurable prgress n targeted Cmmunity-Level Educatin, Incme and Health Gals and t reduce the impact f racial and ecnmic disparities fr prgram clients. CRITERIA FOR ELIGIBLE APPLICANT ORGANIZATION An applicant rganizatin (whether applying itself r acting as lead agency r fiduciary) must adhere t the fllwing eligibility criteria fr funding cnsideratin: Tax-exempt under sectin 501(c) (3) f the Internal Revenue Cde r as a Public Entity. Operate within the Battle Creek/Kalamaz regin and/r serve residents f the regin. Prhibit discriminatin based n race, clr, religin, age, sex (including pregnancy), natinal rigin, marital status, sexual rientatin, gender identificatin, veteran status, disability, genetic infrmatin, height r weight. The fllwing agency-level financial dcuments are required t be submitted as a part f the full prpsal: - Mst Recent Statement f Activities (Prfit and Lss Statement) - Mst Recent Statement f Financial Psitin (Balance Sheet) - Financial dcuments must be upladed frm yur server int yur prfile n the e-cimpact System** - Prpsals submitted with a Fiscal Spnsr will need t submit stated financial dcuments fr the Fiscal Spnsr 501(C) (3) Organizatin **Nte If yur rganizatin has submitted agency financials as part f a Strategic r Basic Needs funding request in the last 18 mnths, additinal agency financials are nt required. TECHNICAL ASSISTANCE UWBCKR staff will prvide the fllwing technical assistance upn request: 1) assessment f prgram alignment within the Educatin, Incme r Health cmmunity level gal; 2) identifying apprpriate prgram-level data t cllect and reprt; and 3) cmpletin f funding applicatins via the e-cimpact system. GRANT APPLICATION REVIEW PROCESS Cmpleted grant applicatins submitted by the deadline f May 18, 2018 will be accepted fr review and cnsideratin fr funding by UWBCKR s Cmmunity Impact Cabinet. Grant applicatins nt submitted r partially cmpleted by the stated deadline will nt be accepted fr review and cnsideratin. REPORTING and PARTICIPATION REQUIRMENTS If the prpsed prgram is apprved fr funding, the applicant rganizatin is respnsible fr the management and versight f the prgram and will receive a Funding Cntract explaining the expectatins as a grantee f UWBCKR. This cntract shuld be signed and returned t United Way, indicating that the funded partner understands and accepts the cntract cnditins. At the cmpletin f the eligible time perid fr the grant, there will be a Year-End Grant Reprt required fr submissin via the e-cimpact system. 1

Additinal bligatins as an identified Funded Partner f UWBCKR include, but are nt limited t, the fllwing: Participatin in UWBCKR annual campaign activities Submissin f client success stries Cmpliance with branding guidelines Participatin in requested site visits, Peer Learning Circles, Nn-Prfit Leader s Rundtable meetings, and ffered trainings and ther events hsted by UWBCKR ver the duratin f the grant perid. All Funded Partner Agencies f United Way are required t cllect and submit client demgraphic data n prgram participants as a cnditin fr accepting an awarded grant. Specifics n the cllectin f client demgraphic data will be discussed with prgram staff and UWBCKR staff during the eligible grant perid. Innvative/Interim grantees will be required t submit client-level demgraphic and prgram utcme achievement data. Supprtive grantees will be required t submit utcme achievement data (**nline submissin n the i- CResults Data System will be required by Wednesday, July 17, 2019). Final prgram narrative and financial reprting fr all prgrams n the e-cimpact Data System (**nline submissin will be required by Mnday, August 12, 2019) Infrmatin and instructin n bth the i-cresults and e-cimpact nline systems will be discussed at the scheduled Grant Training Sessins.
