Center for Leadership, Activities, and Career Services EVENT REGISTRATION FORM
Overview Submit this form for any event, travel, or activity your RSO is planning for the school year. You can save this form for submission at a later time. However, a new form cannot be started until the previous one has been submitted. It does not need to be filled out for general meetings or inside information-only tables, however, any special activity (i.e. fundraisers, contests, programs, information tables outside on the grounds, travel, etc.) must be submitted on this form. This form is due at least three weeks or 15 business days from the day of the event/activity when safety plans, waivers and Physical Plant work orders are involved. Simple events can be submitted one week, five business days, in advance provided your advisor approves promptly. If you feel there are special circumstances, please call our office before submitting (231) 591-2685. This form can serve as a checklist for planning and executing activity for your RSO. Please answer all questions completely and accurately. All events must adhere to University Policies and State of Michigan/Federal Laws. RSOs should be aware of specific policies/laws regarding Risk Management and University Trademark Licensing. Call the Center for Leadership, Activities, and Career Services at (231) 591-2685 for assistance. Raffles must be approved by the State of Michigan Gaming Commission. For more information about Charitable Gaming, go to the website at www.michigan.gov/cg. Fundraisers involving food or beverages must be approved through the Mecosta County Health Department. A special license may be required. Please contact them directly at (231) 592-0130. A copy of this approval application must be in the Group's possession during the activity/solicitation.

Step 1: Log onto OrgSync On OrgSync, in your organization s portal, click on the Events tab. On the Events page, click on Create an Event.
Step 2: Submit your Proposed Event Fill out the form completely. For the first description box, what you write will be available to the public, so be sure to advertise appropriately.
Step 2: Submit your Proposed Event Who can see this? You may decide who is allowed to see your event. Public + Website: Anyone can view this event. Ferris State: Anyone who is a part of Ferris State and has an OrgSync account may view this event. (Your Organization): Only people in your RSO s portal may view this event. Selected Groups: Only people with certain positions (administrators, affiliated members, etc.) in your RSO s portal may view this event. Portal Sharing You may ask to be put on the OrgSync calendar in order to receive more promotion.
Step 2: Submit your Proposed Event Advanced Options: Assessments and Forms You may include verification of attendance or other forms required to attend this event. Pre-Registration Form: If your event requires that an individual fill out a waiver, or they must register through a national site, you may include one here. Post-Event Form: If you would like individuals who attend to fill out a survey or provide feedback about the event, you may include one here. Reflection: In order for participants to receive credit (either for a class or for volunteer service) for the event, they must respond timely to the reflection piece. Step 2: Submit your Proposed Event Fill out the form completely. You may experience more questions depending on the answers to previous ones. You will need to provide your contact information, as well as your RSO advisor s contact information, as they will have to approve of the event before the CLACS office can take action on it. For this description box, it will be for office use only, so please be very specific on the details of your event.
Step 3: Wait for Approval The Event Request Conversation can be used for you to receive updates and make changes as needed to get approved. If your event has not been approved yet, you may ask questions and make changes to your original application throughout the process here.
Step 3: Wait for Approval Your advisor will receive an email at the address you provide. They will need to approve the event that you have registered. Once that has happened, a CLACS representative will review the application and either approve or deny it. If denied, proper reasons will be given for denial, located on the conversation tab of the event.
