Practice Profile St Johns Medical Practice Sevenoaks Kent TN13 3NT www.stjohnsmedicalpractice.co.uk Looking after a list of 10,000 + patients Vacancy Details To start New Year 2017 Currently are looking for a Full time / part time GP on a salaried basis. This ideally will be Monday, Wednesday and Friday, but we can be flexible to a degree for the right applicant. The position may then become that of a partner by mutual agreement if so desired. Informal visits, welcome by appointment. -01732 747216/207 About the Practice We are a friendly and hardworking practice centred on excellence in patient care The Practice was formed in 1984 in a small local building. Our present purpose built premises were completed in 1988 and extended in 1995. We have excellent well established internal systems, enabling us to deal efficiently with the demands of present day medical practice.. We are fortunate to have a very experienced team of GPs supported by a loyal and efficient admin team with low turnover. The team is motivated and committed to the highest standards of patient care. Educational forums are regularly attended both in practice and in the locality with consultants attending.gps are supportive to each other in every respect, both clinically and empathetically. We have had our first CQC

inspection in August this year and currently await our report. Feedback on NHS choices has been good from patients. The GP Team We have 5 Partners and 1 salaried part time GP at present plus regular locum help. One of our partners has recently resigned, hence this vacancy for replacement. Provision of high quality clinical care is the main objective, with due regard for prescribing and cost saving efficiency whenever possible. Involvement with CCG incentives and proactive savings strategy is very important within the team. All GPs are required to be in touch and participate in CCG meetings as appropriate. All are proactive in differing areas of responsibility. All work to not only provide excellent patient care, but also to enhance income by supporting incentives and high quality data entry. Increasing prevalence s currently a high priority. Nursing Team Currently we have a two practice nurses, one of whom has prescribing and a minor ailments diploma. We also have an HCA. They provide supporting roles to the GPs and provide the chronic disease management clinics, plus all the usual treatment room procedures. Regular meetings are held with the Nursing /GP team. The Admin Team & Practice Manager We have a strong supporting team of reception and admin staff who are responsible to the Senior Manager. Recently we have reconfigured management responsibilities to ensure the role is manageable. We now have a three way management split with the most experienced staff responsible for different areas of the business. The staff are reliable and loyal and do a very good job in complimenting the clinical staff by handling the majority of admin and management tasks. Staff turnover is very low and with a wealth of experience and sense of humour they ensure the smooth and efficient running of the practice.

Computer System We use the INPS Vision system. It is supported by Docman data workflow software. We are also using ICE for test results. We use EPS Electronic prescribing for a large number of prescriptions. We are fortunate to have an IT administrator and belong to the NVUG user group which runs annual conferences and supports GP practices for all clinical data issues and training on the use of the system. QOF & Income We have very high QOF achievement.. We have managed to stay on top of all changes thrown at us in recent times in regard to CCG changes, Enhanced services, CQC liability, challenges in the demand of patients and the evergrowing workload. We work well as a team and value each other as individuals. We pay attention to generating income from all possible streams whilst keeping clinical care at the top of the agenda. Services Provided in the Practice We provide a comprehensive range of medical services. Minor surgery. A full range of family planning services, including coil fitting is also provided. All the usual clinics are offered Travel, Asthma, COPD, Diabetes, Well person, health checks, chd, contraception, plus advice on weight loss and smoking cessation referral. We also have a physio outreach clinic for MTW post op patients and host AQP ultrasound. GPs can directly refer. We actively participate in all Enhanced Services, including extended hours commitment. Currently we have to provide 5 hours per week. This takes place once a week with three clinicians working concurrently and sharing a rota which is determined three months ahead so everyone has ample notice of their shift. This would be a necessary part of commitment for an incoming GP.

Appointments We are always trying to be innovative about the way in which we work. We change our system from time to time as demand indicates. Mornings are not pre booked until the day starts. Patients may pre book afternoon appointments up to 6 weeks ahead. The surgery is open from 8.00am -6.30pm Mon Fri. We have worked hard on ensuring patient access is as easy as possible, with a balance of educating patients in the correct use of the system and sign posting to the correct clinician or appropriate service. Our receptionist team try to signpost whenever possible when given information by patients on their needs. We are sensitive about this and although we ask for the reason for an apt, we are respectful if the patient declines info to the admin team. Community Hospital All our phlebotomy is carried out at the hospital haematology clinic. Blood is not usually taken at the surgery unless it is indicated as being absolutely necessary or helpful to elderly frail patients to save them an extra trip. Minor Injuries Unit & Minor Ailments Service We are lucky to have a local Minor Injuries Unit operating at the local community hospital which is open from 8am -8pm daily 7 days a week and run by a nurse practitioner team. There is now also a minor ailments service running alongside it. This is very useful for the practice. The staff are trained to identify and sign post patients to this service where appropriate. Patient Group Participation We have been running a patient group since November 2011 The Practice Manager and a GP meets with them quarterly. The aim is to connect with the patients and allow them to have a voice about our services, make suggestions and understand the changes with service provision and federations. They can also help to organise talks and be supportive to other patients. We try to recruit new patients to attend our meetings opportunistically and by flyers. They attend at flu clinics and help us with notice boards etc.

Out of Hours Cover We are assigned to IC 24. They cover all practices in this patch between 6.30pm 8 am Monday to Friday and all weekends and Bank Holidays. The Premises Modern purpose built surgery, which has been extended to capacity. The rooms are arranged over two floors. We have full facilities for disabled patients as well as electronic opening front doors, and a lift for wheelchairs and pushchairs to the first floor. We have an average car park for the staff and patients. Doctors have their own spaces. The surgery is on the corner of the junction with St Johns Hill and Camden Road. We are about ½ mile from the town centre and close to the Bat & Ball crossroads and train station which crosses the A25, and gives a close link to the M25/M20 Our Mission & Ethos St Johns Medical Practice aims to deliver the highest quality primary medical care. Our desire is to change and develop with the needs and expectations of our local community, whilst maintaining a highly skilled and professionally motivated health care team. We deal with change in a positive way as an opportunity rather than a threat. The continuing education and clinical skill mix of our medical team will be matched with a commitment to educate our patients to use the system appropriately and to the best advantage. We will endeavour to promote health education and preventative lifestyle advice. This will be available by consultation, literature and our website. We wish to promote and maintain our patient satisfaction to a high degree. We are always pleased to receive constructive comments and criticisms in our endeavour to care for our patients to the highest standard. Complaints will happen and we shall deal with these in house and treat them as an opportunity for sharing and learning from experiences. We are committed to delivering our services with skill, sensitivity and compassion.
