Vision+ Anticoagulation Training Day Plan Overview As you may be aware, Warfarin is the main Anticoagulation treatment for the prevention of stroke and thromboembolic conditions in certain patients with Atrial Fibrillation (AF). Recently published NICE (National Institute for Health and Clinical Excellence) draft guidelines permit the use of new oral anticoagulation drugs (NOACs), along with other risk factors their use is likely to be based on INR (International Normalised Ratio) control. Vision+ Anticoagulation Manager contains tools for managing all aspects of anticoagulation regardless of the patient's drug regime. It includes Adverse Event Reporting, advice on Dosing regimes and presents supporting materials for structured Education and Reviews. When you select a patient in Consultation Manager an alert window will warn if the patient is missing essential anticoagulation information that will assist in the management of their care. There is also an easy to use Clinical Template where you can quickly record relevant information. In addition, a suite of reports will capture data and provide a comprehensive reporting tool. In all Anticoagulation Manager comprises: Alerts for missing/due data Anticoagulation Templates which include:anticoagulation Therapy management screen Warfarin management and dosing tool Diagnosis Indication screen INR monitoring screen Management screen for New Oral Anticoagulant Drugs Other Assessment screen showing other factors that may affect the patient's anticoagulant treatment Reporting facility which allows you to work on patient lists or create a mail merge Who should attend the training? Practice Manager, Clinicians, Relevant Administrative Staff General Prerequisites Understanding of existing practice protocols for anticoagulation management. 2
Aims and Objectives Aims The aim of this training day is to demonstrate how to use and implement the new Vision+ Anticoagulation Manager tool. Objectives By the end of the day the training delegates will be able to: Download and Maintain the Anticoagulation tool. Implement the Vision Anticoagulation Manager according to their own practice protocols. Record and monitor anticoagulation patients in consultations and during administrative work using the templates. Record and maintain anticoagulation dosage and realise that weekly dosage, not daily dosage is displayed in the patient journal (important!). Change practice and patient settings as required. Create Vision+ reports on Anticoagulation data. Note Timings below are approximate and can be adjusted to suit practice preferences. 3
Course Content Timings Aimed at Detail Up to 40 mins Practice Manager/I T administra tor Explanation of what Anticoagulation consists of e.g. template, reports, Warfarin monitor Review practice protocols with the PM/IT Manager Ask how they currently deal with INR monitoring Review paper based systems Make notes of who does what e.g. INR result comes in, given to gp, GP records INR and works out daily dose, staff phone patient Download the template and explain how to use Download Web files Give the IT/PM an overview of the Warfarin tool (consider who will do what) Discuss INR history Five weekly dosing histories and how to add to the dosing history screen Demonstrate how to link to a Diagnostic indication Discuss how the dosing tool screen works Demonstrated how this information is embedded in the Anticoagulation Template (show the PM/IT Manager to get a feel for what the clinicians will use). 4
1 hour Clinical Staff Start the training on the Warfarin Monitor option within Anticoagulation Manager (as this is the quickest and simplest way to record data) Discuss INR history (shows the last 5). Five weekly dosing histories and how to add to the dosing history screen. Demonstrate how they can change the screen layout. Look at Trends. Print options. Demonstrate how to link to a Diagnostic indication Discuss how the dosing tool screen works add an INR to a dummy patient and explain. Round up and Round down, Use, Clear, Missed days, recalls. Explain there is also an Anticoagulation template which includes this but can be used for additional information. 30 mins Practice Manager/I T Administra tor Explain Practice Settings point out recalls Discuss Patient Settings Provide an overview of Anticoagulation Template including Dosing tool, care pathways, print options Look at the Practice Reports, this includes Anticoagulation manager and Warfarin monitor reports Discuss how to combine alerts if this option is enabled 30 mins Reception (ir relevant) Show reception how to view the Daily dose using the Warfarin Dose screen. Emphasise they should check date, INR, daily dose and next recall Point out that a weekly dose is displayed in the journal and not a daily dose 5
