Introduction Organization of outpatient care in communities - one of the major health problems of Ecopolis. According to World Health Organization in the Russian Federation more than 65% of resources for health incurred as a costs for the provision of hospital services while it is about 40% in EU. As a result, outpatient care, which is an integral part of how the national health system and general socio-economic development of society, has become one of the most problematic areas in health care. These problems are particularly evident in the small towns where are only a small staffing of the medical authorities in the outpatient clinics of various specialties. In some regions are not sufficiently developed specialized advisory techniques.

Not all hospitals are connected on roads, without that the organization of qualified and timely medical care by hospitals being not possible. Not up to the established dispensary observation of patients. This is true even for established pharmacies by observation. The coverage of prophylactic medical examination of patients of working age is low, accounting for pharmacies includes, at best, half of all registered patients. Solving these problems is possible through the priority development of primary health care services primarily in small towns.

Due to its role and position at primary health care this is the first major phase of the ongoing process of health care. Efficiency and quality of the entire health care system from the preservation of working capacity of the country and the decision of the majority of medical and social problems of families and vulnerable groups: children, disabled and the elderly. The reforms are intended to improve medical treatment at minimal cost. How in the world it is obvious that the transfer of the maximum amount of aid of the nonprofit sector not only more cost effective but also rational. The main objectives of the reorganization of ambulatory versos are today: improving the availability and quality of outpatient care in the regions, the development of day hospitals in ambulances and the development of the clinics.

Project Summary This project aims to improve the social development of regions with a view to improving the accessibility and quality of health care for all groups of population and is particularly important for the residents of remote settlements that are included in a future Ecopolis network. Strategic directions of the project are defined as follows: The dynamic monitoring of public health with priority on prevention activities, transmission of the maximum possible level of support into day hospitals, local hospitals, creating home-care services, expansion of outpatient surgery together with outpatient rehabilitation services as near as possible to the patient's residence; implementation of targeted programs to prevent the most common diseases ( Healthy Heart, School of Preventing Diabetes, Healthy Food etc.) implementation of programs and initiatives for healthy lifestyles.

Project Description A clinic should be installed in close proximity to residential areas All technical equipment is situated in hospitals in buildings with two floors, one above the working surface. During the construction of modern high-tech hospitals cost prefabricated structures will beinstalled in using the design and quality of medical and related equipment and their operation will permit the usance within one construction season.

A special feature of this project is to develop new planning and design solutions and a flexible approach to the placement of the medical finding, taking into account the needs of specific regions to exploit new technologies and building materials and equipment. In developing the project modern energy-saving solutions for heating, ventilation, air conditioning and lighting for use are provided. All technical solutions incorporated in the project are optimal from both technical and economic points of view. In terms of organisation of jobs it is expected to be missing negative things by objective and subjective reasons for the great majority of medical facilities. (For example in the industry in Russia 55.3% of establishments are actually without employment and the degree of deterioration of the medical equipment is about 70% in many medical institutions ). A clinic is focused on providing a wide range of outpatient medical services, namely the maintenance of the diagnosis, prevention and treatment. Investigations and proceedings will be conducted based on the latest equipment from world leading producers in Europe and the United States that have already made a name and enjoy the confidence of patients and medical staff.

As part of health care for the well being of the patients clinics include all major industries like advice, diagnosis, restorative treatment, dental care, emergency care and an outpatient clinic. The planned capacity of the clinics will be about 250 visits per shift during the work time of 8.00 bis 20.00 h, so the two-shift operation will cope with 500 visits per day.

Advisory Department The main tasks of the separation are: provision of specialized courses for medical assistance, the definition of recommendations for further observation and treatment in outpatient and inpatient facilities. A rational organization of the counseling centers will provide the population with affordable and qualified health advice, especially in the preclinical stage.

Diagnostic department The main objectives of the practice are: the necessary laboratory and diagnostic tests, computer based, using modern methods, the examination of patients, the integration of the various types of diagnostic tests, the introduction of diagnostic algorithms to obtain the shortest at full possible time. With a diagnostic department, the development and implementation on the ground economically, clinically effective and of high quality implementation at diagnostic procedures. As part of the diagnostic department there will be found: X-ray room, ultrasound room, a study of functional status, office, office endoscopy and clinical laboratory. All rooms are equipped with modern hightech diagnostic equipment from leading manufacturers and foreign companies.
