I/We assume all risks and hazards incidental to such participation including transportation to and from activities; and I/We do hereby waive, release, absolve, indemnify and agree to hold harmless the FLAT ROCK WARRIORS FOOTBALL ASSOCIATION, the organizers, sponsors, supervisors, participants, and persons transporting my/our youth to and from activities, for any claim arising out of an injury to my/our your whether the result of negligence or any other cause, except to the extend and in the amount covered by accident or liability insurance. I/We agree to return, upon request, any equipment issued to my child in as good condition as when received. I/We shall furnish a copy of a certified Birth Certificate as requested. I/We understand that no equipment will be issued until registration fees have been paid or payment arrangements made. Any check that is returned by any financial institution is subject to a $25 service charge. I/We understand no requests for refunds of registration fees will be considered after the first game. Parents will be expected commit to coverage of a one (1) hour time slot in the various volunteer opportunities during the season. PERMISSION FOR USE OF PHOTOGRAPH: Permission is granted to use my daughter s/son s picture in future advertisement (to include Facebook page) and literature for Flat Rock Warriors Football events sponsored and conducted by them. FLAT ROCK WARRIORS EQUIPMENT USE AGREEMENT Player s Name: Flat Rock Warriors Football Association will provide for your child s use, the following items: Helmet Game pants Game jersey Shoulder pads (tackle only) Flags (flag only) Players at all levels must provide the following items: Practice jersey Practice pants Mouth guard Cleats (any molded rubber cleat is acceptable) Optional equipment (such as forearm pads, elbow pads, rib pads or gloves) may be purchased at the family s discretion. The association will not be responsible for any loss of or damage to equipment purchased for personal use and used in the course of any practice or game. Replacement Costs For the items Entrusted to You: Tackle helmet: $85 Flag helmet: $45 Shoulder pads: $45 Should you fail to return it at the end of the season; you will be billed for the amount of any item listed above not returned after the designated equipment return date. PLEASE READ AND SIGN: I understand that if any of the equipment given to me is not returned in the same condition that it was given to me, (reasonable wear and tear accepted) the item or items will be replaced or satisfactorily repaired at my expense. In that event, I agree to pay the amounts assessed by Flat Rock Warriors Football. Furthermore, I agree that if I do not return the equipment by the end of the season in its entirety, I shall pay the replacement cost of each item plus any attorney fees and cost incurred to recover the agreed amounts from me. VOLUNTEER AGREEMENT Flat Rock Warriors Football is a volunteer organization and it depends on all participants to volunteer to benefit the football program. Each family will be required to assist in a volunteer duty for 1 hour per season. A schedule of duties will be sent out for each team. The children of families who fail to volunteer may not be permitted to participate in the game the week after the family member fails to follow through on their assignment. The board may permit the family to reschedule if appropriate. Volunteers will be assigned to any of the following: Spirit Wear Table Chain-Gang Game Announcers Home Game Field Setups/Teardowns Field Painting Field Cleanup Team Parent or other duties as needed Player s Name: Team: Parent s Names:

PARENTS CODE OF CONDUCT It is the goal of Flat Rock Warriors Football to promote the development of strong character, a right attitude and a sense of responsibility and citizenship in youngsters, using organized sports as a vehicle. As the parent of an athlete who is participating in sports with Flat Rock, I agree to the following terms: 1. I will endeavor to have my child present at all games and practices with all necessary equipment; if circumstances prevent his/her attendance, I will provide the coach with as much prior notice as possible. I understand that if my child is late to a game, it may limit their opportunity to participate. 2. I will remain at games and practices whenever possible. If I cannot do so, I will ensure that a responsible adult is present in the event of illness, accident, inclement weather, etc. If I am not at a practice or game, it is my responsibility to contact the coach regarding any announcements that may have been missed. 3. I will pay all registration fees in a timely manner. I understand that these fees will not be refunded after the first game of the season. 4. I will see that all uniforms and equipment provided for my child will be properly maintained, and I understand that I will be responsible for replacement costs if such is not returned in well-maintained condition to the designated party by the time requested. 5. I will familiarize myself with the league, and will assist my child in understanding the same. 6. I will request a conference with my child s coach if I feel there is a problem or question. If I do not feel I have received an adequate or acceptable response, I will follow the association s guidelines by next speaking with a board member. Should I still feel there are unresolved issues, I will request to speak with the board of directors. 7. I will respect the officials and their authority during games and will never question, discuss, or confront coaches at the game field, and will take time to speak with coaches at an agreed upon time and place. 8. At no time will I use profane language while in the presence of my child s coaches or teammates. I further understand that alcoholic beverages and tobacco products are prohibited at games/practices. 9. I will read the by-laws and policies and procedures of the organization, as posted on the web site, so that I am aware of the terms and conditions of participation in the association. 10. I will conduct myself in a seemly manner at all games and practices, setting a positive example. I also agree that if I fail to abide by the aforementioned rules and guidelines, I will be subject to disciplinary action that could include, but is not limited to the following: a. Verbal warning by official, head coach, and/or organization b. Parental game suspension with written documentation of incident kept on file c. ATHLETES CODE OF CONDUCT As a participant in sports with Flat Rock Warriors Football Association, I agree to the following terms and conditions: 1. I will strive to be on time for all practices and games with all necessary equipment. If I am going to be late or absent, I will attempt to notify the proper person as early as possible. 2. I will abide by the rules of the sport as set forth by the governing league. If I have a question concerning a rule, I will speak with my coach at an appropriate time. 3. I will conduct myself properly, demonstrating good sportsmanship at all times, both on the field and off. I will show respect for my teammates as well as for the players on the opposing team. At no time will I use profanity. 4. I will demonstrate respect for my coach/sponsor and will observe all rules that he/she sets forth. 5. I will be attentive at all games and practices, whether I am on the field or on the sidelines, if I am not playing, I will support those who are. 6. I will, by both words and my actions, set a positive example for those around me. I have read the above and agree to abide by such as a condition to participate in sports with Flat Rock Warriors Football Association. I also agree that if I fail to abide by the aforementioned rules and guidelines, I will be subject to disciplinary action that could include, but is not limited to the following: d. COACHES CODE OF CONDUCT As a coach for the Flat Rock Warriors Association, I agree to the following terms and conditions: 1. I understand that it is my responsibility to teach good sportsmanship, teamwork and discipline in addition to the rules and skills that apply to my sport. 2. I will see that all players have an opportunity to participate, and will put the well being of my players above my desire to win. 3. I will refrain from using tobacco products, alcoholic beverages and profane language at any time during games or practice, and will strive to ensure that assistants and athletes do the same. 4. I will adhere to all rules of my governing league, and will see that all participants are advised of such rules and policies. 5. I will provide schedules for all games and practices, and will notify parents and/or players as soon as possible if any changes arise. 6. I will maintain an open relationship with all players and parents, and will make myself available for discussion, should questions or problems occur. 7. I will not allow any problems that might occur with a parent to have an effect on my relationship with the athlete. 8. I will assume responsibility for all equipment relating to my team, and will see that it is returned in a timely manner to the equipment director. 9. I will strive to be a role model and positive example for all the players I work with, and will encourage them at every opportunity. 10. I understand that it is my responsibility to read, understand, and adhere to the by-laws of the Flat Rock Warriors Football Association and the Chesterfield Quarterback League.
