Who is Telligen? What is the QIN-QIO Program? Telligen: The Medicare Quality Innovation Network- Quality Improvement Organization (QIN-QIO) for Colorado, Iowa and Illinois QIN-QIO Program Purpose: To improve the efficiency, effectiveness, economy, and quality of services delivered to Medicare beneficiaries 3

Who is Telligen? What is the QIN-QIO Program? QIN-QIOs have performance-based contracts with the Centers for Medicare & Medicaid Services (CMS) help achieve national quality goals through focused efforts at the community level provide technical assistance and convene learning and action networks at no cost to support healthcare quality improvement at the community level 4

Clostridium difficile Infection (CDI) Project Telligen recruited 32 NHs to participate in CDI data collection using the CDC s National Healthcare Safety Network (NHSN) Participation helped establish a CDI national baseline for long term care (LTC) Next Steps Telligen will continue to support nursing homes in preventing CDI through educational offerings, online toolkits and individual technical assistance in their understanding of the principles of LTC Antibiotic Stewardship and reducing CDI in understanding how to accurately utilize the NHSN database 7

QAPI push Results 10 Colorado QAPI push homes continue to show a steady decrease in their composite scores as compared to Colorado non-qapi push homes whose composite scores have increased overall

11 Reductions in Antipsychotic Medications

National Nursing Homes Quality Care Collaborative (NNHQCC) Progress Recruited 166 homes in CO To date, 70 participating homes (43%) have met NNHQCC composite score goal <6.01 These long stay Quality Measures have shown the most rapid improvement in nursing homes: Pneumonia and Flu Vaccinations Antipsychotic Medications Bowel and Bladder Incontinence Depression 12

Reducing Readmissions Using a Community Approach Goal: Foster community coalitions of hospitals, nursing homes, and other post-acute partners with a focus on reducing avoidable readmissions and fostering collaborative relationships and communication. Involvement: Care transition collaboratives in the metro Denver area, Fort Collins, Greeley, Colorado Springs, Pueblo and the Western Slope 14
