Any question in which yes is answered, we ask if the condition is current. All medical information provided becomes a part of the member s electronic health record. The information can be updated at any time to reflect the most up to date information possible for our physicians. 11

Adding Dependent Click My Family Then Click the My Dependents tab Dependents less than 18 are managed by the primary member on the account. The primary member is responsible for completing the medical information for the minors before a consultation for them can be requested. 13

Select the member the consult is for Provide Consultation Details For minors, a section will appear requiring selection of the adult authorized to be on the phone during the consult. An adult is required to be on the phone during the consult for any minor under 18 Select the state in which the member is located. This will expand for product selection (ex. General Medical, Derm) Once selected the option for phone or video will appear. Then the option for as soon as possible or scheduled: Image Upload option if desired Select the phone number the physician will use to contact the member Enter any necessary notes for the physician (ex: ask for room 515) Determine whether you want a copy of your consult to go to your PCP Select your Pharmacy 17

Confirmation A section of nonrequired medical information not collected at the initial medical history completion will display at the bottom of the screen. It is optional to the member to complete. The consult confirmation number is provided A physician will contact the member to conduct the consultation 20

FAQs Who are the Teladoc doctors? Teladoc doctors are U.S. board certified in Internal Medicine, Family Practice, Emergency Medicine or Pediatrics. Our doctors average 15 years practice experience and are licensed in your state. Our doctors incorporate Teladoc into their day-to-day practice as a way to provide people with convenient, affordable access to quality medical care. Does Teladoc replace my doctor? No. Teladoc doctors do not replace your primary care physician. Teladoc should be used when you need immediate care for non-emergent medical issues. It is an affordable, more convenient alternative to urgent care and ER visits. Is the consultation fee the same price, regardless of the time of day or night? Yes! Teladoc charges one flat rate per consultation. How quickly can I talk to the doctor? A doctor will call you back within an average of 16 minutes and likely within an hour. If you miss the doctor's call (whether you are away from the phone or you have anonymous call blocker on), you will be returned to the consult queue. FAQs How do I pay for a prescription called in by Teladoc? When you go to your pharmacy of choice to pick up the prescription, you may use your health/prescription insurance card to help pay for the medication. Like any prescription, you will be responsible for the co-pay based on the type of medication and your plan benefits. Can I provide consultation information to my doctor? Yes. You can access your portable electronic medical record at anytime. Download a copy from your online Teladoc account or call 1-800-Teladoc (835-2362) and ask to have your medical record mailed or faxed to you. Can you provide services related to psychiatric or dental care needs? Not at this time. Teladoc provides care for non-emergent medical issues. If the Teladoc doctor recommends that I see my primary care physician or a specialist, do I still have to pay the Teladoc consultation fee? Yes. Just like any doctor appointment, you must pay for the consulting doctor s time. How do I pay for the consultation? You may pay with a credit card, debit card, or PayPal. 2015 Teladoc, Inc.
