Opening Day for the 2018-2019 School Year Wednesday, August 15, 2018 for all schools in Lompoc Unified School District GENERAL REGISTRATION INFORMATION ALL STUDENTS - GRADES TK-12 PARENTS OR GUARDIANS MUST ACCOMPANY STUDENTS WHEN REGISTERING IMMUNIZATION INFORMATION California law requires that all schoolchildren be immunized against Polio; Diphtheria, Pertussis and Tetanus (DPT); Measles, Mumps and Rubella (MMR); and Varicella. Kindergarten students are required to have Hepatitis B immunizations and Varicella is required for all out of state entrants. After a student s 7 th birthday, a DPT/DTaP booster shot is required to enter grades 7 12. Parents should call their doctor s office to schedule the student s appointment if the District has not yet received the required immunization documentation. Parents or guardians of students in any California school, private or public, are no longer able to submit a personal belief exemption for required vaccines. Only a permanent medical exemption from a licensed physician is allowed. Please see the table on the following page for more information (SB277). For additional information, please go to www.shotsforschool.org. First grade students must have a health screening within the first 90 days of entrance into first grade. Students must submit an up-to-date immunization record at the time of enrollment. A certificate stating that the child received a health checkup within the previous 18 months is acceptable. AB 1433, The Oral Health Assessment, requires that children entering public school for the first time in TK, K, or first grade have a dental check-up by May 31. Dental evaluations that have happened within the 12 months prior to school entry meet this requirement. Students whose immunization records have not been verified by the school nurse CANNOT BE ENROLLED. Students cannot be allowed to attend school until their immunization records are in compliance with the California s legal requirements. Page 1 of 7

KINDERGARTEN / TRANSITIONAL KINDERGARTEN (TK) Children entering Kindergarten must be five years of age on or before September 1, 2018. The District will offer Transitional Kindergarten at all school sites for children who turn 5 years old between September 2, 2018 and December 2, 2018. Children who turn 5 years old between December 3, 2018 and June 30, 2019 may apply for enrollment by submitting a transitional kindergarten application to the District Office. Applications will be placed on a waiting list that will be prioritized by birth date. ALL KINDERGARTEN AND TK STUDENTS Kindergarten students who registered in March, April, May or June of 2018 do not need to re-register. Parents registering students for Kindergarten should bring verification of the child s age and immunization records. CHILDREN NEED NOT BE PRESENT DURING KINDERGARTEN REGISTRATION. Vision and hearing screening of all Kindergarten children will be completed during the school year. Please refer to the IMMUNIZATION INFORMATION on page one (1). The first three weeks of school at all elementary schools will have a Kindergarten and Transitional Kindergarten Jump Start. All Kindergarten and Transitional Kindergarten students will be on a minimum day schedule during the three weeks of Jump Start. Beginning September 4, 2018 Kindergarteners and Transitional Kindergarteners will be on a full day schedule. TRANSPORTATION A complete bus schedule will be posted on the Lompoc Unified School District web site and at the schools just prior to the opening of school. Transportation for students enrolled on an intra-district transfer will NOT be provided by Lompoc Unified School District. Please bring address verification documents (rent receipt, utility bill), immunization records, and last report card (if available). IMPORTANT: PLEASE REFER TO THE IMMUNIZATION AND NEW STUDENT SECTIONS (above). All students attending LVMS for the 2018-2019 school year may pick up their packets in the office beginning Wednesday, August 8, 2018 from 9:00 to 11:30 a.m. and 1:00 to 3:30 p.m. The forms enclosed in the packet must be completed before students pick up their schedules. Parents new to LVMS are invited to an information night on Thursday, August 9, 2018, at 5:30 p.m. in the school gym. New students will return their signed packets to the LVMS gym on Thursday, August 10, 2018 and attend a WEB Welcome Day from 9:30 to 11:30 a.m. (lunch will be provided from 11:30 a.m. - Noon). New students will receive their schedules during WEB Day. Schedule pick up for returning students will be Friday, August 10, 2018, from 5:00 to 7:00 p.m., Monday, August 13 and Tuesday, August 14 from 9:00 to 11:30 a.m. and 1:00 to 3:30 p.m. in the LVMS office. The forms enclosed in the packet must be completed before students pick up their schedules. The first day of school is Wednesday, August 15, 2018, and will be a collaboration day. Classes begin at 8:20 a.m. and end at 2:00 p.m. Passing bell rings at 8:15 a.m. Back to School Night is Thursday, August 16, 2018 from 6:00 to 7:30 p.m. A PTA meeting will be held from 5:30 to 6:00 p.m. Picture Day will be Tuesday, August 21, 2018. VANDENBERG MIDDLE SCHOOL Registration for new students to Vandenberg Middle School will begin on Thursday, August 2, 2018. Page 4 of 7

SCHEDULE PICK UP An Emergency Care Card, completed and signed by parent/guardian, PLUS an acknowledgement and consent form signed by the parent and student, are required for students to pick up their schedules. Incoming 7 th Grade and new to VMS 8 th grade students will receive their schedules at Jump Start on Thursday August 9 th. Returning 8 th grade students may pick up class schedules on Friday, August 10 th from 11:00 a.m. 4:00 p.m. in the VMS Multi-Purpose Room (MPR). Campus will be open for student tours. Make-up day for schedule pick-up only will be on Monday, August 13 th from 9:00 a.m. to 12:00 p.m. and 1:00 p.m. to 4:00 p.m. on the patio area just outside the Administration Office. The campus will be closed on Monday for student tours. REMINDER Parents/guardians and student should read the handbook BEFORE they sign the acknowledgement and consent forms. JUMP START There will be a Jump Start Parent Night on Wednesday, August 8, 2018 beginning at 6:00 p.m. in the VMS MPR. Jump Start for 7 th grade students and any new 8 th grade students will be held on Thursday, August 9, 2018 from 9:15-11:30 a.m. Jump Start bus schedule and further information will be mailed in late July in postcard form. Students are encouraged to ride the bus to Jump Start. P.E. UNIFORMS Students are required to dress out every day for PE: navy blue athletic shorts (no short shorts or denim), a grey t-shirt and shoes appropriate for athletic activity. For your convenience, optional shirts and shorts with the VMS logo will be sold during PE the first week of school ($20). FIRST DAY OF SCHOOL The first day of school is Wednesday, August 15, 2018. Start time is 9:20 a.m. and dismissal is 3:15 p.m. STUDENT TRANSPORTATION LUSD Transportation Department will be issuing VMS bus student boarding passes beginning in 2018-2019 school year. Passes will be issued after the first few weeks of school when bus and seating assignments have been determined. More information will follow in August after school starts. Bus schedules will be available to bus students at Jump Start and on Schedule Pick-Up Day. Parents transporting their students to and from school should use the parent pick-up area at the intersection of Mountain View Blvd. and Acacia Street. Please exercise caution and be aware of student foot traffic when dropping or picking up your student in this area. VMS staff will be in place to direct/control traffic at school dismissal time and their direction should be followed at all times. Any questions or concerns regarding student transportation should be directed to LUSD Transportation Department at 805-742-3180. Page 5 of 7

CABRILLO HIGH SCHOOL New students need to call the counseling office at 805-742-2850 starting July 30, 2018 to set up a registration appointment. Office hours are 7:30 a.m. -12:00 p.m. and 1:00 p.m. - 3:30 p.m. Registration appointments require both the student and parent/guardian to attend. No class assignments will be made unless the registration packet is complete including up-to-date immunization records, birth certificate and address verification. August 3-7 8:00 a.m. 3:00 p.m. All pre-registered students (Grades 9 12) pick up information packets in the Attendance Office at Cabrillo High School (4350 Constellation Road) or at the LUSD District Office (1301 North A Street). Schedule Pickup Students must bring the parent-signed documents (from packet) with them to pick up their schedules. Students must be present to pick up their schedules. All students will have ID pictures taken before schedules are given to them. Students who wish to purchase school pictures must bring the picture packet with payment. Students will go to the cafeteria to return ALL signed paperwork, pick up schedules, have ID and school pictures taken, according to the following schedule. NEW STUDENTS: Orientations will be Wednesday, August 8 th at 10:00 a.m. or Tuesday, August 14 th at 1:00 p.m. Please pick up a registration packet and fill it out prior to the orientation. Parents or guardians and the student are required to attend an orientation. If newly enrolling students and their parents fail to attend, they must wait for the next available opening to enroll their student. All students must return their registration paperwork, completed, with parent signatures, at their Orientation/Registration. Maple High School classes begin Wednesday, August 15, 2018 at 8:40 a.m. DR. BOB FORINASH COMMUNITY DAY SCHOOL Dr. Bob Forinash Community Day School is a referral only school. Any new students will be referred by their home school and contacted individually for an orientation. RETURNING STUDENTS: The school has moved to the Ed Camino campus, 320 North J Street, Lompoc. First Day Packets may be picked up beginning Thursday, August 2 nd at the Forinash Community Day School administrative office located at 320 North J Street, Lompoc.
