Department of Health Professions Nursing Program Graduating PN/LPN Application to ASN Program Thank you for your interest in the Health Professions Programs at - UM. As you prepare your application for submission, there are a few items to consider. This application is for current Licensed Practical Nurses (LPN) or students who will be successfully completing their second semester applying to the Associate of Science Registered Nursing Program (ASN). If you are not already enrolled at - UM, an application for admission must be completed as well. Please follow the application directions carefully. In order to be eligible to apply to the Associate Science Nursing program at - UM the following must be demonstrated: General Associate of Arts (AA) Program Requirements BIOH 201 & BIOH 202N Anatomy and Physiology I 4 BIOH 211 & BIOH 212N Anatomy and Physiology II For students who 4 completed the one semester (6 or 7 credit) Anatomy & Physiology course, you must complete BIOH 211 & 211N M 121 OR M121 College Algebra or 3 M115 Linear & Probability or M 152 Pre-Calculus with Algebra or M 153 Pre-Calculus with Trig or M 171 Calculus WRIT 101 English Composition 3 PSYX 100S Introduction to Psychology 4 CHMY 121 Introduction to General Chemistry 3 CHMY 122 Introduction to General Chemistry Lab 1 NUTR 221N Nutrition 3 NRSG 100 Introduction to Nursing 1 Completion of BIOH 201 & BIOH 202 (A&P I) and BIOH 211 & BIOH 212 (A&P II), with a minimum of a B grade within 2 attempts. All other pre-nursing courses completed as outlined in catalogue with a grade of C or better. Maximum of two attempts per course. Most recent grade earned will be used to calculate GPA. It is not required, but highly recommended for the Associate of Science Nursing Program, (ASN) that BIOM 250N Microbiology in Health Sciences, BIOM 251N Microbiology in Health Science Lab and SOC 101S Introduction to Sociology be completed prior to entering the program due to a scheduling conflict with ASN clinical days. Your application will be evaluated for minimum eligibility and then ranked in three steps. Each step and category is carefully considered and valuable in the application process. The three steps of the application process: 1. Submit application materials 2. Take proctored ATI Predictor test the week applications are due (see below) 3. Interview (of top 30 ranked applicants) Step 1: Cumulative GPA (all classes taken since high school) of at least 2.75. Successful completion PN courses and direct application OR unencumbered LPN nursing license. Selective GPA from Associate Degree and Practical Nursing Program courses (50% of application score). Step 2: Proctored ATI Predictor Test score. Schedule time with Administrative Associate the week of April 18 22 nd at Computer Lab (25% of application score). The predictor test will be offered: i. Tuesday, April 19, 2016; 8:30 a.m. to 12:30 p.m. AD 16 ii. Tuesday, April 19, 2016; 2:30 p.m. to 6:30 p.m. AD 15 (limited space) iii. Wednesday, April 20, 2016; 8:00 a.m. to 12:00 p.m. AD 16 iv. Friday, April 22, 2016; 10:30 a.m. to 2:30 p.m. AD 14 Please call the Administrative Associate at 243-7846 to schedule test once you have turned in your application paperwork. Allotted time for the test is 3 hours plus registration and payment time. You will need to bring some type of electronic payment to purchase the test online. The cost is $50.00. Step 3: The top thirty ranked applicants will be invited to an interview with a selection committee. Date, time, and place to be scheduled late May/early June (25% of application score). Appointments will be scheduled by Nursing Program Administrative Assistant. The application is ranked on the scores of the ATI Predictor test, GPA, and interview. Rev. 3/2016 2
Selection Process 1. Step I: Applications will be read and ranked. All applicants will be notified regarding their selection status as soon as possible after final grades for the semester have been posted. This process takes approximately 1-2 weeks. Please do not call or email. Notification letters to all applicants will be sent out as soon as applications are ranked. 2. Step II: Top 30 ranked candidates will receive an invitation to interview. Selected candidates are required to participate in an interview for consideration. The Nursing Program Student Application Review Committee (SARC) will then make final selections for acceptance into the AS N Program. The decision of SARC is final. SARC reserves the right to decline any nursing program applicant based on the application packet and interview process. 3. ASN accepted students will be notified by letter as soon as possible. Please Do Not call the Administrative Assistant to inquire about acceptance status. Please note: Successful completion of the ASN Nursing Program does not guarantee a Montana State nursing license. Nurses cannot practice in the profession of nursing until they have successfully passed a National Certification Exam. Acceptance for taking National Certification Exams is approved or denied by the Montana State Board of Nursing on an individual basis. If you have a criminal conviction, you may be denied. PN SEMESTER 2 OR LICENSED PRACTICAL NURSE APPLYING TO ASSOCIATE OF SCIENCE NURSING PROGRAM A letter stating which program you are applying to with your address and phone number. Completed Nursing Program Application form. Application to must be done online or at the Admissions Office if accepted to the Nursing Program and you are not currently a Missoula College student. It is a separate application. Transcripts, evaluation of transfer courses and/or waivers: Official transcripts from all colleges and universities attended. This means ALL schools attended post high school. Unofficial transcript of courses currently in progress. For UM or students with courses in progress, a completed Transcript Release Form for grades currently in progress at time of application. (You do not need to pay the fee.) PN License if not applying directly from practical nurse option. Enclose a copy of your current, unencumbered LPN license. Fall 2016 Application for Nursing: This application is for students applying to the 1 + 1 Associate of Science Nursing Program starting August 2016. You must have a current, unencumbered LPN license OR be in the process of completing the practical nurse option, and have completed all prerequisite classes.
