Multi Regional Audit of Blood Component use in patients with liver cirrhosis Brian Hockley Liver Audit Executive Summary Report: Summary Report_2014 1. Welcome & Apologies YS welcomed everyone to the meeting and apologies were noted. Introductions were made around the table as new committee members have joined the RTC. Dr Simon Davies, Consultant Anaesthetist from York and Marie Collinson-Wallace the new Patient Representative. 2. Minutes of the last RTT Meeting held on 27 th February 2013 and matters arising The circulated Minutes (Paper 1) were accepted as a true record and will be placed on the website as Confirmed. 3. Y&H RTC Budget 3.1 Report and review of commitments (Paper 2) 2

OC went through budget report and confirmed that we have already spent in the first quarter 50% of the budget. Other expenditure this year will be RTC meetings x 2 along with the annual symposium in October 2014. We need to rely upon income from sponsors to support this event. Sponsorship is getting increasingly difficult as we have sent ten invitations to companies and we have not received any replies. If anyone has any contacts or suggestions please contact us. 4. Regional Educational Activities 4.1 Educational Activities 30.04.2014 Tiny Transfusions Feedback The feedback was excellent and again was a successful event. Summary of Evaluations Nurse Authorisation The group discuss a possible venue with a view that the Leeds Blood Centre can no longer be used. The White Rose Centre could be a possibility as easy access for speakers to attend, although it was mentioned that the RTC can reimburse speakers expenses if necessary but this year s budget is very limited. There are plans in place to run a Nurse Authorisation course later this year, in November 14 and then again next year June 2015. RTC Annual Study Day 8th October 2014 - Fifty Shades of Red, Pinderfields. Only one speaker left to confirm, however we have another consultant haematologist on standby who has offered to speak on component transfusion to patients with PNH. We have a diverse programme and there are 90 delegates now registered. We have 30 reserved spaces for Haematology Registrar s and these places will be released 6 weeks prior to the event if not taken. Jehovah s Witness group are also attending with an exhibition stand. We have an anonymous donor for the competition prize this year. Quality Audit Workshops There has been some further interest in quality audit workshops with Edwin Lindsay which were 3

very well evaluated last time and aimed specifically towards laboratory staff. We could look to run this after April 2015. GS added there is a general problem trust wide releasing lab staff. AD advised the renal anaemia management guidelines will be published shortly and AD will be trying to engage with GP's via KG to incorporate general anaemia management education into their training schedule. 4.2 Audit Working Group update AD confirmed all trusts should have received their hospital slideshow and a summary of the results will be presented later and the focus will be on the tools available. BBTS has chosen Y&H to present our results. All RTC's were asked to submit their projects to the BBTS and Y&H are one of the regions chosen to present at their conference in September. This was felt to be a very good reflection for the region. We have also received positive feedback from the report. Massive Haemorrhage Audit Presentation Anne Davidson Massive Haemorrhage 5. Issues from Hospital Transfusion Teams 8.1 Issues from Hospital Transfusion Teams HTC Key Achievements: - Mid Yorks - accurate table recording NPSA competencies, linked to electronic ESR - Calderdale & Huddersfield - able to extract blood usage and wastage data and is discussed at HTC meetings (YS asked if he could see their data if they can adapt for his trust); Use cell salvage quite widely - Leeds training on track with a monthly competency report; risk management now on board and also working with heads of nursing HTC Key Difficulties: - Pressure on staff with pathology modernisation - Mid Yorks - platelet usage has increased, single dose now in place - York platelets have also increased, staffing issues having to put lab staff on shift patterns and pulling back on resources during the day - Lack of use of cell salvage in most trusts - Sheffield Teaching - platelet usage in oncology is increasing 5

Key Issues for RTC Chair to raise nationally: Yorkshire & The Humber Regional Transfusion Committee - North Lincs & Goole - Increased use of intraosseous needles in resuscitative procedures with many protocols indicating that samples obtained may be used for blood sampling including cross match. No validation of analysers for performing these tests. Risk that intraosseous samples may be forwarded to transfusion laboratories for testing without indication of sample source. Issue raised via NHSBT Customer Service visit. Action: YS - To raise at the next NBTC meeting the issue regarding IO needles and risks involved when sent to transfusion labs for testing 8.2 Round table discussion TI - review of sample keeping and BCSH guidelines. Round the table discussion on which hospitals keep blood samples when the patient has been transfused. RE Hull do not store as you would risk getting incorrect data. If your trust can afford to invest you can freeze the whole sample if necessary. GS Rotherham keep the whole blood samples in the fridge for 28 days. RH - Barnsley have storage issues and cannot afford another fridge. 9. Platelet Usage and Wastage Platelet Newsletter Platelet champion newsletter above for information. AD suggested for the region to do a Quick Audit on platelet use as we have seen a 4.5% rise in platelets for this region compared to the National rise of 1.4%. This will demonstrate any double dosing and/or identify inappropriate use. This Quick Audit system has been piloted in the NW region and AD would like support from the RTC. 10. AOB PBM recommendations have now been circulated. SB asked how many hospitals use Hemocue with POC. AD suggested to contact SHOT/Tony Davies regarding incidents and recommended practice related to use of some POC devices for Hb estimation e.g. blood gas machines AD asked the committee if they were happy regarding this venue taking into account the locality; car parking and hire costs. The total room and catering costs are slightly less than Don Valley House. Although the screen is quite small and data can be complicated to read it was agreed to use the venue again for November. 6
