ROTARY DISTRICT 7020 GIFT OF LIFE REPORT FOR YEAR 2010-2011 PROVIDING HEART SURGERY FOR CHILDREN IN THE CARIBBEAN.

. FACTS: POPULATION STATISTICS: The Caribbean consists of many Islands with approximately 14 Million inhabitants. There is an estimated 400 children born each year in the Caribbean with Congenital Heart Disease who will die an early death if heart surgery is not preformed. Congenital Heart Disease is the number one birth defect afflicting children around the world. There are hundreds of children on the waiting list in the Caribbean for cardiac intervention that will save their lives. Every year, that number increases. Our Goal is to develop a Sustainable Pediatric Cardiac Care Program ISLAND POPULATION Jamaica 2,700,000 Haiti 10,033,000 British Virgin Islands 19,100 Trinidad 1,339,000 Bahamas 304,837 United States Virgin Islands 108,612 Cayman Islands 53,172 Netherlands Antilles 100,018 Anguilla 14,254 St. Martin 68, 500 Saint Barthelemy 8,732 Turks and Caicos Islands 18,122 JAMAICA: This country is centrally located with easy access to the other Caribbean islands, either by boat, or via a major International Airport located in Kingston, Jamaica. With a birth rate of 45,000 per year, producing approximately 180 children per year who will require heart surgery, Jamaica is an ideal location to build a full service sustainable Heart Surgery Program to serve the Caribbean. Because of this need, Pediatric Cardiovascular Services are currently being developed at Bustamonte Hospital for Children (BHC) in Kingston Jamaica.

FACILITY: BHC is the only dedicated Children s Hospital in Jamaica. The Hospital has 4 Operating Suites (Theatres) with a PICU, Recovery Area, and Multiple Wards to treat Burns, Surgical, Medical, Respiratory, Neonatal, Emergency and Accident patients. One of the Theatres is dedicated specifically to Cardiac Surgery. They have Laboratory, Pharmacy, and Radiology Services on Site, and Blood Bank Services are off site. The Hospital is a very large converted British Military Base with connecting pathways to each Ward, and beautiful courtyards between

Dedicated Cardiac Operating Room Suite. Echo with TEE, Anesthesia Machine, Bypass Pump ect all in place Ward 8 specifically outfitted for Pediatric Cardiac Surgical post op patients after ICU care LOCAL PHYSICIANS and PROVIDERS: The following providers make up the Local Pediatric Cardiovascular team. This team is currently performing approximately 1 simple cardiac surgical case per week independently. Dr s Shirley Foster and Charmain Scott work with Visiting Medical Team and newly donated Sonos 5500 Echo Machine VISITING MEDICAL TEAMS: The following Visiting Medical teams, supported by various agencies, provide training on an annual basis, usually 1 week every 3-4 months. During a typical week with a visiting team, 10 20 surgical cases will be preformed, providing training of increased complexity to the local team. Each team brings their own supplies, staff, and often times very needed equipment which is donated to the Program at BHC. JAMACIAN CHILDRENS HEART FUND: The mission of Jamaican Children's Heart Fund is to facilitate and address the urgency reflected in the increasing, rather than decreasing list of infants and children with congenital heart disease in Jamaica. CARIBBEAN HEART MENDERS: Caribbean Heart Menders provides life-saving surgery for children in need as well as helps train medical professionals and develop local hospitals to be able to treat patients on their own. We are a team of medical professionals and philanthropists who volunteer our time and talents to help children in third world Caribbean countries who were born with congenital heart defects GIFT OF LIFE RI D-7020: Gift of Life International is based upon the network of Rotary Clubs in District 7020 which span the Caribbean. This program specifically focuses on providing life saving cardiac surgery to children from developing countries within the Caribbean and around the world HEART CARE INTERNATIONAL: We provide life-saving heart care for children suffering from congenital heart disease who live in developing countries. We work to enable the local medical communities in the countries we visit to treat such patients independently

CHALLENGES: SPACE: Space challenges for the Cardiac Surgical Program are two fold. There is not enough space for the intensive care patients, or the equipment and supplies. Due to the lack of Intensive Care Space, getting surgeries scheduled can be challenging for the local team. Surgical cases are frequently cancelled due to lack of bed space, or skilled nursing availability. When visiting cardiac teams arrive, the rest of the surgical services normally provided at BHC must be suspended to provide space for patients. This is an obvious disruption to the general surgical services provided to this population when a visiting medical team is working. The donated supplies and equipment line the hallways with no storage space. A Toshiba Cardiac Catheterization lab has been donated, with no space to house it. Donated Supplies and Equipment for Program lined up in the hallways. There is no storage space allotted to cardiac surgery. General ICU area used temporarily for visiting Cardiac Surgical training teams. Very tight quarters.

WHAT HAS BEEN ACCOMPLISHED? 1. Over the last 5 years, Visiting Medical Teams have been working with the local providers at BHC. Over 200 children have already received life saving surgery. The Local Team is now performing 1 simple cardiac surgical case per week independently. 2. Training of additional Anesthetist and Cardiologist in UK is nearly completed. 3. Staff Training. The Administration at BHC has committed to dedicate 6 full time nursing positions to cardaic surgery. In return, Chain of Hope is funding the appointment of a cardiac nurse specialist who will relocate to Jamaica for one year to train the local staff. 4. Rotary District 7020 has created a Gift of Life Board with Director Dennis Chong of the New Kingston Rotary Club. 5. New Cardiac Cateterization Lab has been donated, and waits in storage in the USA for a Cath Lab Space to be built at BHC. 6. Committed team of stakeholders to bring the project to fruition. Commitment of Ministry of Health (SERA), Hospital Administration, Local and Florida Rotary Clubs, Visiting Medical Teams, and Supporting Agencies to build a full service Cardiac Surgical Program at BHC! Members from Hospital Administration, SERA, Local Physicians, Agencies, Rotary and Gift of Life come together in the common goal for building a Pediatric Cardiovascular Program to serve the Caribbean The Conference Room - February 27 th, 2010 Rotarians from Ocho Rios and Downtown Kingston host visiting Medical Team from Florida

7. Teaching and training of skilled cardiac Clinicians. 8. Submission of request for grant funding of US$1.1mil from Digicel Group by New Kingston and Downtown Kingston Rotary Clubs to build new Cardiac Surgery block this to be completed and opened with Digicel Group by March 2012. Construction to commence July 2011 in condition of approval by Digicel. 9. The new Intensive Care Surgery block for Bustamante to double the existing number of beds as well as provide vital cardiac facilities which include a new cardiac operating theatre and a cardiac Catheter Laboratory all under one roof. 10. Commitment with MedShare to supply medical supplies and equipment on a yearly basis to the program: Yearly request to be sent to MedShare for shipment to the BHC. 11. Fund raising with Chain of Hope carried out raising approximately Ja$2mil cash to acquire supplies to the Hospital cardiac unit. 12. Hosting of the third Annual Larry King Cardiac Conference on Saturday April 9, 2011 Sponsored by The Larry King Cardiac Foundation 13. Matching Grant with Rotary Club of Downtown Kingston and D-6890 Gift of Life Clubs for heart surgeries to be performed on 12 children within D-7020 Gift of Life program. 12 surgeries have been successfully carried out to date ending March 2011. 14. Other Matching Grants are been proposed to continue funding surgeries for Gift of Life children 15. Fund raising and Corporate Sponsors will be approached during the year 2011 2012 to seek yearly contribution towards a goal of 100 surgeries per annum at a cost of US$5,000 per surgery per child. 16. The RI District 7020 Gift of Life center will be a focus and highlight of Gift of Life International during this year. The purpose of this report is to seek the District 7020 approval for the continuance and support of this Gift of Life program for future years. The proposed budget is attached with this report for the continuance of the program.
