2. Context and Scope HammondCare is an independent Christian charity that exists to improve quality of life for people in need. We specialise in aged and dementia care, palliative care, rehabilitation, mental health services for older people, and other related health and aged care services. HammondCare seeks to embed evidence based best practice in its services and we provide these services through home care, subacute hospitals and residential aged care. Bringing these health, hospital and aged care services together, HammondCare has been able to develop innovative, flexible care models designed to serve people with complex health or aged care needs, regardless of their circumstances. Summary of the Role The role of the Level 1/ 2 Exercise Physiologist is to work within the Mission and Mission in Action of HammondCare to ensure a high standard of Exercise Physiology service is provided to patients of the Hospital, along with appropriate support and advice to their carers. The Exercise Physiologist will participate as a member of the multi-disciplinary team to ensure a patient centred, evidence based, continuous quality improvement work environment that is consistent with the values and ethics of HammondCare and Exercise and Sports Science Australia. The essence of this role: Provision of effective and efficient Exercise Physiology service to clients and appropriate support and advice to their carers Participate as a member of the multi-disciplinary team in assessing patients level of functional mobility and care needs, setting care goals and providing appropriate treatment. Participate as a member of the HammondCare team in the application of the organisation s Mission and values to the pursuit of its objectives in a client centred, continuous quality improvement work environment 3. Key responsibilities Responsibilities Capability Outcomes 1.0 Client Care Assess and treat clients with a high standard of professional practice, maintaining the ethics and standards of HammondCare and Exercise and Sports Science Australia Contribute to client care as part of a multidisciplinary team and maintain good communication with medical, nursing, allied health and other staff and voluntary workers Assess, prescribe, implement, monitor and upgrade appropriate rehabilitation exercise programs for clients Referrals are addressed in a timely manner in accordance with protocols for designated caseload. Builds trusting relationships with patients and their families/carers to support effective care provision. Has effective working relationships with the MDT and the MDT value the service provided.

Responsibilities Capability Outcomes Document client s progress as required in accordance with industry guidelines Observe and report on the condition and progress of clients to team as appropriate Before using equipment in client care, check contra-indications, conduct necessary tests and issue appropriate warnings Reassess clients response to treatment on a regular basis and modify accordingly Assess the standard of interventions by utilising outcome measures Liaise with Allied Health Assistant to ensure that work flows efficiently and that safe work practices are used. Involve clients in their own case management through education Respect and maintain clients privacy Teach carers and co-workers the skills they need to assist clients Ensure care continuity by obtaining information and treating directions from previous carers and passing such information to subsequent carers by discharge summary and other means. Ensure necessary arrangements are made to facilitate admission to acute care facilities when required, and readmission on clients return. Actively participates in multidisciplinary team meetings for designated caseload. Completes documentation of comprehensive Exercise Physiology assessments, program and interventions in patient medical files in accordance with HammondCare and department policies. Outcomes of therapy are evaluated and documented with relevant handover given to other services as necessary. Medical records indicate that patients referred to Exercise Physiology receive a comprehensive service that addresses the patients physical activity and exercise needs and contributes to specialist multidisciplinary care. 2.0 Operational Management Activities Participate in development and promotion of Exercise Physiology services. Actively participate in the implementation of an effective and appropriate quality Exercise Physiology service. Participate in regular evaluation of Exercise Physiology practice. Exhibit a commitment to quality service and participate in activities to enhance continuous quality improvement. Attend and contribute to team, case, department and other meetings as needed. Read and understand the Policy and Procedures Manual(s) for the Department and HammondCare, and work within the principles contained therein. Behaviour and performance are consistent with professional expectations, HammondCare s MIA and Capability Framework. Attends and participates in relevant meetings. Undertakes department support and development activities as directed by supervisor or Manager and completes in agreed time frames. Prepares and leads education sessions as agreed with supervisor. Participates in a minimum of one Quality

Monthly statistics are completed in specified time frame and will accurately reflect workload. 3.0 Professional development/education Maintain and update knowledge by regular reference to journals and texts. Identify skills and knowledge required to fulfil the role and to attend appropriate staff development determined during ADM (and linked with the Physiotherapy Department and/or service business plan) and/or supervision sessions. Actively participate in continuing professional development in line with ESSA accreditation requirements. Clinically supervise Exercise Physiology students as requested. Contribute to the education of staff, students, other health workers, volunteers and carers. Provide in-services to other departments and organisations as requested. Participate in mandatory training as required. Participates in department professional development program and activities. Participates in clinical supervision with designated Senior Physiotherapist or Physiotherapy Manager according to agreed schedule. Attends relevant education to develop skills and knowledge, as agreed in ADM, and uses what you learn. Prepares and leads education sessions as agreed with supervisor. Participates in student supervision appropriate to level of experience. Mandatory training is completed within the required time frames. 4.0 Other Ensure the efficient use of supplies. Ensure all interactions portray HammondCare as a professional and caring provider of Palliative, Rehabilitation, Aged Care Psychiatry and Aged Care services. Comply with formal complaints and grievance procedures of the organisation Communicates openly and honestly with supervisor, manager and colleagues.
