Health Education Wessex is responsible for the planning, development, education and training of the and public health workforce across Hampshire, Dorset, Isle of Wight and Salisbury. We also provide medical training for the States of Jersey and GP appraisal services for Jersey and Guernsey. We believe that the key to improving the health and of the 2.8 million people living in Wessex is investment in the skills and values of the 52,000 people working in the NHS and in primary care. Within the field of postgraduate medical education and training, we manage across primary and secondary care for these health communities totalling around 2,400 doctors in training at any one time across 12 Trusts and 160 GP practices. In addition, we support the workforce development for GP Practice Nurses and Public Health practitioners as well managing a bespoke GP appraisal service for established GP Practitioners to meet the requirements for revalidation in accordance with the statutory requirements set by the General Medical Council. We manage training programmes for postgraduate medical training according to the statutory standards set by the General Medical Council (GMC) and have responsibility for establishing and maintaining quality management systems for all posts and programmes as required by the GMC. The work of the Deanery is guided by the principles embedded within the NHS constitution. Rotation Information The Wessex Child & Adolescent Psychiatry rotational training scheme covers Poole (total population 140,000) and part of East Dorset (approximate population 40,000), Southampton and the surrounding areas (total population 387,000), and Portsmouth

and the surrounding areas (total population 545,320). In other words the scheme covers a lovely part of the South Coast with all the benefits of the sea, beautiful countryside and historic cities. The Scheme consists of 8 full-time ST4-6 Trainees, one being a lecturer s post based in Southampton. Usually, 5 trainees rotate between Southampton and Portsmouth, spending 2 years in one place and one year in the other and 2 trainees rotate between Poole and Southampton. However, there is flexibility within this system to meet individual trainee s needs. The Scheme supports Less Than Full Time (LTFT) training and can provide for some additional flexible trainees. Child & Adolescent Psychiatry Specialty training to obtain a CCT in Child & Adolescent Psychiatry is 3 years full-time or pro-rata equivalent. Study and Training The primary aim of all posts is the training programme developed and there is a region wide syllabus and minimum standards of education agreed by all Trusts within the rotation. The Deanery is committed to developing postgraduate training programmes as laid down by GMC, Colleges and Faculties and by COPMED - the Postgraduate Deans Network. At local level college/specialty tutors work with the Programme Director and Directors of Medical Education in supervising these programmes. Trainees will be expected to take part in these programmes (including audit) and to attend meetings with their nominated educational supervisor. All posts within the training programme are recognised for postgraduate training by the General Medical Council (GMC) in accordance with their standards for training. Study leave is granted in accordance with Deanery/Trust policy and are subject to the maintenance of the service.

All posts have a service element and the following covers the majority of duties. There will be minor variations in different hospitals but the list is aimed at covering the majority of duties: 1. and part of East Dorset (approximately 40,000). It is a general Tier 3 service, dealing with a comprehensive range of mental health / developmental problems in children and young people up to 18 years old. This also provides comprehensive services in CAMHS-LD (Learning Disability)There is close working with Primary Mental Health Workers. There is also close working with both the social services and education Departments. A Core trainee in Psychiatry spends 6 months at the CDC, Poole and 6 months in the Shelley Clinic, providing an opportunity for the ST4-6 trainee to provide supervision. There are close links with the paediatric department (hospital and community) and opportunities to further develop paediatric liaison and gain experience with patients with learning disability and other developmental difficulties. The clinic provides regular consultation and support to paediatrics, social services and education, as well as services in primary care, e.g. school nurses, Relate, Connexions. Experience can be offered in a wide range of treatment approaches, including family therapy, psychodynamic individual therapy, brief individual therapy, cognitive therapy, medication, liaison and networking. There are opportunities for partaking in the various CAMHS management groups, e.g. clinical governance, and a multiagency operational group. The ST3-6 trainee is expected to attend the monthly interteam clinical meeting and monthly consultants peer group. There is access to journals/books at the Postgraduate Library, Poole Hospital and the library in the psychology department at St Ann s Hospital. Maple Young Peoples Intensive Support Service and Pine Cottage, Poole Pine Cottage Adolescent Unit/ Pepper Lodge is an 8 bed adolescent inpatient facility (shortly to become an 8 bedded facility with Tier 4 Home Treatment experience in addition to inpatient experience) catering for young people between the ages of 11-18 years across the whole county of Dorset (Dorset population approximately

750,000). It is based in St Ann s Hospital and there is access to their duty CT1-3 in psychiatry on an as required basis. As well as nursing staff, the unit has access to occupational therapy, art psychotherapy, a teacher, a consultant family therapist, an OT assistant and a number of health care assistants. The Maple Service, which works in conjunction with the inpatient unit, is a Dorset-wide specialist adolescent service set up to provide multi-disciplinary intensive treatment and community outreach for young people between the ages of 10-18 years. As well as the professionals mentioned above, the Maple Service has clinical psychologists, a senior practitioner social worker, and seconded educational psychologists. This service provides therapeutic input to adolescents with complex needs who require a level of care not usually provided by a traditional CAMHS out-patient service. Community outreach can be provided where there is poor engagement with traditional services. There are teaching opportunities including presentations at the junior doctors journal club and case presentation meetings at St Ann s Hospital. The opportunity for examination coaching of CT1-3 trainees planning to undertake MRCPsych examinations is available. The Shelley Clinic, Boscombe The Shelley Clinic provides a general tier 3 community outpatient CAMH Service. It sees young people up to the age 18 years from Bournemouth Borough (total population 164,000) and part of East Dorset (population 85,000). The multidisciplinary team allows opportunities for the ST 3-6 trainee to gain experience in a wide range of therapies including family therapy, psychodynamic individual therapy, brief individual therapy, cognitive therapy, medication, liaison and networking. This service works closely with Primary Mental Health Workers, as well as social services, education, Primary Care and voluntary services.

The Senior Trainee is able to supervise a junior trainee in Psychiatry and provide teaching to other junior medical staff, Primary Mental Health Workers, members of the multi-disciplinary team and medical students. Dorset Health Care Trust provides an Early Intervention in Psychosis Service, which is nurse led providing services for young people from 14-35 years. The service works jointly with consultants from locality CAMHS teams in developing care packages for young people with psychosis. In addition, there are also CAMHS nurses based in the local Youth Offending Team and young adults Drug and Alcohol Service, which give the senior trainee the opportunity for special interest sessions in forensic experience and substance misuse training. Mental health services for children and young people with learning disabilities are provided by the learning disabilities team. There is an outreach team and a learning disabilities psychology service. Portsmouth and South East Hampshire In this placement the senior trainee is employed by Sussex Partnership NHS Foundation Trust or Solent NHS Trust (East). There are 3 community Child & Family Therapy Services in the Portsmouth and East Hampshire area which all provide a similar service and the senior trainee is based at one of these clinics. Although the 3 clinics are managed by different organisations, there are close clinical and managerial links. The clinics receive referrals of moderate to severe child and adolescent mental health problems (i.e. tier 3) but each area works closely with their own team of Primary Mental Health Workers who see children in the mild to moderate range of difficulties (i.e. tier 2). The Trusts have contracts with Leigh House Inpatient Adolescent Unit in Winchester (tier 4). Small scale district paediatric liaison experience is possible.

Each Community Child & Family Therapy Service sees patients up to 18 years of age. Each clinic receives approximately 500 referrals per year from a wide range of professionals. They each have a multidisciplinary team which includes child & adolescent psychiatrists, clinical psychologists, community nurse specialists, family therapists, psychotherapists and administrative support. The Fareham and Gosport clinic also has an art therapist. In each clinic the senior trainee has the opportunity to gain experience in family therapy, cognitive behaviour therapy, individual psychodynamic psychotherapy, the creative therapies, brief focused therapies and solution focused and narrative therapies as well as experience in psychopharmacological therapies, extensive consultation to other agencies and liaison work. There is also the opportunity in Portsmouth to carry out psychodynamic psychotherapy with a suitable case under supervision of a senior psychotherapist and also get experience in infant mental health. The senior trainee has an opportunity to work with the psychology team which provides mental health input to children with learning disabilities at the Child Development Centre in Portsmouth. He/she also has the opportunity of observing assessments by the autism assessment service. All teams place a high priority on both individual and team supervision and the trainee is able to work as a co-therapist with a range of professionals. The senior trainee has the opportunity to provide supervision for a junior trainee in child psychiatry at Havant Child & Family Service as well as teaching medical students, presenting clinical cases at academic meetings and teaching non-medical professionals.

Community CAMHS Posts Havant Child & Family Therapy Service, Civic Centre Road, Havant (employed by Sussex Partnership Trust) Portsmouth City Child & Family Therapy Service, Falcon House, St James Hospital, Portsmouth (employed by Solent Health Care NHS Trust) Fareham & Gosport Child & Family Therapy Service, Osborn Clinic, Fareham (employed by Sussex Partnership NHS Trust) Specialist Placements The senior trainee will also have the opportunity to work with high risk, disaffected young people in many settings through a placement with the Hants and IoW Forensic CAMHS Team and placement with the CAMHS Mental Health Inreach Team at Swanick Lodge (a local authority run secure unit having both welfare as well as criminal justice beds). Almost by definition this is a forensic population and one that all agencies find very hard to engage and manage. The necessary processes of assessment and management are particular and comprehensive. Networking tasks are central to success. The trainee is involved in the multidisciplinary working of the complex caseload held by this service. Such work will involve assessment, therapy and management of the many facets of the cases. Supervision of the trainee s work will be enhanced by the multidisciplinary team s own supervision resources (internal supervision expertise, and external supervision of the team). This experience involves close working and liaison with other statutory and voluntary agencies, in particular Local Authority, Youth Offending Teams, Drugs and Alcohol Services, Specialist CAMHS, housing and other relevant bodies. There is plenty of relevant opportunities for visits to and

work with cases within prisons, secure units, children s homes, hospitals (adult psychiatric, forensic, adolescent psychiatric, adolescent forensic), various special education resources and other important organisations in the area. Because the service is closely linked to the Portsmouth District Youth Offending Teams, court experience and the production of court reports is possible. There are opportunities for providing consultation and clinical supervision of work undertaken by other agencies, as well as training of other agencies alongside other team members. The trainee is expected to be involved in managerial and internal training aspects of the team. Solent NHS Trust Senior trainees on placement in Southampton, and at Leigh House Adolescent Unit are employed by Solent NHS Trust (West). They will provide out-of hours on call for the rota covering Leigh House Hospital and the Southampton on call rota. There is an opportunity for senior trainees based anywhere in the scheme to arrange special interest sessions (e.g. one session per week for 3 months) with consultant child and adolescent psychiatrists with a special interest in learning disability. They are based in Eastleigh, Portsmouth, Dorset and Ashurst. The experience includes: spending time in a special school in a classroom for children with severe learning difficulties; seeing cases for assessment jointly with members of the learning difficulties team; attending clinics in special schools; liaising with others teams e.g. social services Children with Disabilities team, Safeguarding Children (Child Protection) teams and with the specialist Connexions personal advisor; and attending community paediatric clinics.

Leigh House In-Patient Adolescent Unit Leigh House Hospital is a 20 bed hospital for young people with severe psychiatric illness between the ages of 12 and 18 years. There is also a day-patient service and a limited out-patient service for second opinions. Leigh House serves a wide area that includes Hampshire, Isle of Wight and the Channel Islands. Most of the young people in the hospital have eating disorders, psychoses, major affective disorders or severe OCD and anxiety disorders. The average length of stay is four months. There is a well staffed school attached to the hospital and an impressive occupational therapy department. Young people and their families have the opportunity of a range of therapies including family therapy, cognitive behaviour therapy, cognitive remedial therapy, DBT and Mind Skills groups, psychodynamic group work and art therapy. There is a good working relationship between the different members of the multidisciplinary team regarding the management of cases and the functioning of the unit. The senior trainee has plenty of opportunity to take part in multidisciplinary assessments of complex cases and review meetings in addition to opportunities for training (supervision in cognitive behavioural therapy, family therapy and pharmacological interventions). There is weekly supervision, both individual and group, from the two educational supervisors. The Paediatric Unit, Southampton General Hospital Placement in this post is usually divided between 6 months based at Bursledon House Children s in-patient unit and 6 months based on the paediatric wards in Southampton General Hospital working with the child mental health team (Paediatric Liaison). Bursledon House Children s Unit is a 12 bed residential and 6 place day unit service for children 0-16 years based in the grounds of Southampton General Hospital and

has input from paediatricians, psychiatrists, psychologists, occupational therapists, psychotherapists, other therapists and a small school which has become a pupil referral unit. Patients with a range of mental health, paediatric or mixed paediatric/mental health problems are referred, including those with psychosomatic disorders, chronic fatigue syndrome and neurological problems. Patients are referred from a wide range of geographical areas from psychiatrists and from acute and community paediatricians. Admissions range from short term assessments to longer term assessment and management, including rehabilitation e.g. following severe head injury. There is very close working between paediatricians and mental health professionals. Southampton General Hospital has a wide range of secondary and tertiary paediatrics, including respiratory, neurology, oncology and endocrine services and a paediatric intensive care unit. The paediatric liaison team provides input to all paediatric services in the hospital. On the paediatric wards the senior trainee assesses inpatient and outpatient referrals from the paediatricians, together with Paediatric Liaison Team. Referrals range from cases following deliberate self harm to those with longstanding and complex chronic illness (including eating disorders). The senior trainee also provides consultation to the various paediatric services and disciplines and some staff support. There is an opportunity to attend the consultant paediatric neurology clinic and other paediatric clinics. The trainee is expected to take part in teaching medical students, paediatric nurses, and be involved in the courses run by the College of Nursing. They also have the opportunity to supervise a junior trainee in psychiatry. There is a well established multidisciplinary service for the assessment of children with a possible autistic spectrum disorder and the trainees are able to attend these assessments.

Brookvale Adolescent Service, Southampton The Brookvale Service provides an experienced and specialised community psychiatric service for adolescents with mental health problems. The service provides for young people between the ages of 14 and 18 years, their families and carers, and a range of referring agencies, including health, social services, education and voluntary services. The service is based in a large detached house in Portswood, Southampton, and has a multi-disciplinary team consisting of nurses, social workers, community and art therapists, psychologists and psychiatrists. Clinical consultative work takes place in a number of different settings. Brookvale specialises in the provision of packages of intensive community input to young people, and their families, who have acute mental illness or severe psychological distress and disturbance, and who may require admission to inpatient care due to the risks. This is provided as an alternative to inpatient care. The senior trainee has the opportunity to get involved in the assessment, formulation and management of such cases in the community as well as gaining experience of working with older teenagers who have developmental disorders (e.g. ADHD, ASD). Experience of a variety of psychological therapies under supervision of senior therapists (including CBT, Cognitive Analytic Therapy, Family Therapy, Narrative Therapy) and psychopharmacological treatments is also available. There is also ample opportunity to gain experience of multi-agency working and providing liaison to other statutory and 3 rd sector agencies. Educational and clinical supervision is provided by consultant psychiatrists. The Behavior Resource Service

The BRS provides specialist experience of intensive multi-agency working for young people with complex needs and severely challenging behaviour. This Tier 4 community team has a multi-agency membership including consultant a psychiatrist, psychologists, learning disability nurses, social workers, a teacher, educational psychologist, family therapist and RMNs. The consultant psychiatrist works across Brookvale Southampton and BRS is able to provide placement at the BRS for trainees who opt for this special interest experience. This involves an overview of the issues involved in working with local authority partners, therapeutic services for children in care, experience of medico-legal issues especially in the arena of welfare Secure Orders and other avenues of multi-agency working. The community team provides an assertive outreach approach adapted from multi-systemic therapy. Many of the service users have mild learning difficulties. Approximately 50% to 60% are looked after children. The service has no specific exclusion criteria and accepts young people with multiple difficulties in every arena of their life. This placement provides a unique opportunity for detailed multi-faceted assessment of young people and their environment. The multi-agency work also includes assessment and treatment of young people with a history of offending and forensic concerns and the BRS is the base for the western arm of the Hants & IoW Forensic CAMHS (providing input to Southampton City, New Forest and Southern Parishes). There is an opportunity for undertaking medico-legal work particularly regarding young people involved in the criminal justice system and structured risk assessments and consultation under supervision. There is ample opportunity to get involved in research activities as the Senior Lecturer is based Southampton along with several research assistant psychologists. The clinic has close links with the DBU (Development and Brain Unit) at

Southampton University. There is also opportunity to get training in the New Forest Parenting Programme. CAMHS/learning disability experience with the LD consultant and nurse specialist is a key component of this training placement. Southampton Child & Family Clinic, Orchard Centre, Weston Hospital Clinic This is a community based service for children and young people up to the age of 14 years. The clinic has a wide range of clinicians including consultant clinical psychologist, a senior occupational therapist, social workers trained in play therapy, family support workers, mental health nurses (including nurse prescriber) and psychiatrists. There is a family therapy team in the clinic. There is a weekly meeting for referrals and allocations, regular staff meetings and a monthly good practice meeting. The Team also provides supervision to the Saucepans Team (Community CAMHS Development covering the localities in Southampton, which brings together school nurses, health visitors, education workers, family support workers and mental health nurses and provides a short term Tier 1 and Tier 2 service). There is ample opportunity for the senior trainee to become involved in the teaching of medical students and other disciplines. The clinic has a well resourced A-V teaching suite. There is also opportunity for the senior trainee to get experience providing relevant supervision for a GP ST1 trainee (doing a joint community paediatrics and CAMHS placement). A consultant psychiatrist has started a SSU (Special Study Unit) at the Southampton University Medical School for 3 rd year medical students on Learning Psychiatry through Films which will involve the screening of relevant films by professionals from CAMHS and Adult Mental Health for 3 rd Year medical students.the Orchard Centre also hosts the Orchard Film Club, membership of which is mainly mental

The senior trainee accepts that s/he will also perform duties in occasional emergencies and unforeseen circumstances at the request of his/her colleagues both senior and junior. It has been agreed between the professions and the department that while trainees accept that they will perform such duties, the Secretary of State stressed that additional commitments rising under this sub-section are exceptional and in particular the trainee should not be required to undertake work of this kind for prolonged periods or on a regular basis.

Some of the senior trainees will also have the opportunity to provide clinical supervision and mentorship (if appropriate) to junior psychiatric trainees (CT2/ CT3). Curriculum The Scheme aims to provide and promote training, education and professional development in Child & Adolescent Psychiatry to a standard expected by the Royal College of Psychiatrists (currently based on College Advisory Papers, College Curriculum and ARCPs (Annual Review of Competency to Progress)/ Annual RITA Appraisal Form), and the GMC Guidelines for Good Medical Practice. At the end of their training, specialty trainees will be expected: to have gained a thorough knowledge of normal child development; be able to assess children and families in order to make a diagnosis of psychiatric disorder if present and to be able to formulate the presenting problems and to develop treatment plans; work as a cotherapist with other members of the multidisciplinary team; have experienced a wide range of treatment modalities such as behaviour therapy, family therapy, cognitive therapy, solution focused therapy, brief focused individual therapy, long-term individual psychotherapy, group therapy and psychopharmacological treatments; worked within multidisciplinary teams; worked with other agencies (for example, attended Children s Services case conferences, convened multi-professional planning meetings and provided assessments for Statementing procedures); provided consultation to other agencies and professionals, e.g. social workers, educational professionals, voluntary agencies; dealt with clinical emergencies including deliberate self-harm and provided psychiatric assessments for the courts. Teaching Additional Training and Education

Trainees will be supported if they wish to undertake further training, education or qualifications by recognised bodies, e.g. family therapy training, cognitive behaviour therapy training, an MSc or an MD. However, there is no automatic additional funding available, apart from each trainee s individual study leave budget. Information about locally available courses and procedures to apply for them and issues regarding using study leave/ special interest time will be discussed at the STC (Specialty Training Committee). The Academic Programme Trainees will be expected to attend this 3 year rolling Academic Programme which is held at Ashurst Hospital, Southampton on the 2nd and 4th Wednesdays of the month during term-time. The Programme includes a 2-3 day long experiential workshop per year, a Spring and Autumn Academic day under the auspices of the Southern Branch of ACAMH, and attendance at the academic component of the Wessex Child & Adolescent Psychiatrists meetings (occur every 4 months). The development and delivery of the programme is a combined effort between trainees and the Academic Course Organiser. It includes seminars, journal reviews, clinical case and video presentations, research meetings, outside speakers and a Balint group (which is supervised by a Consultant Child Psychotherapist). It is also an opportunity for trainees to meet together as a group. Time to attend the academic programme will be protected. Each placement has a bench library as well as access to university/hospital libraries. Attendance on the programme is pro-rata. Research All the senior trainees will need to fulfill the research requirements as outlined in the College curriculum. It is important that all senior trainees familiarize themselves with the curriculum requirements. Trainees will have 2 protected sessions per week for

Audit Audit plays a crucial part in clinical governance and senior trainees will be expected to participate in audit training and audit projects in each of their placements. Teaching There are opportunities to teach medical students from Southampton University 9through a well coordinated and highly rated CAMHS attachment for medical

students including a SSU Special Study Unit Learning Psychiatry through Films ), CT1-3 psychiatry trainees, other doctors e.g. GP trainees and paediatricians, and other members of related professions, e.g. health visitors, social workers, teachers, nurses, midwifes, etc, members of the multidisciplinary CAMHS teams and non-professionals, e.g. support groups, parents, school children.
