School Facility and Services Location, Building Access and Hours: o Dental Hygiene CLINIC: 5883 Rue Ferrari, Suite 125, San Jose, Ca, 95138 o Telephone number: 408-960-0183 o o Sherry Heaney- Contact name for questions regarding the school, equipment, etc. Directions to Clinic entrance; upstairs on 2 nd floor, building open 6:00am-6:00pm daily for duration of exam. Parking: No parking passes are required, park only in unmarked parking spots. Please do not park in spots labeled Jan Marini. Security and Emergency Services: Onsite Security contact information and protocol: o In the event of an emergency, contact the site coordinator, Sherry Heaney or Kim Clark. Serious emergency- dial 911. ATM Machine: No ATM is available on campus Food Services Onsite: Vending machines are available on the first floor. 2. Preparing for the Hygiene Exam Patient Screening: The campus facility is not available for patient screening by Candidates. Radiographs: X-ray machines and film processors are available for any additional required films (conventional/analog only). No fees are required for the films or processing equipment. Administration of Local Anesthetic: The clinic does not provide a qualified practitioner to administer local anesthetic to Candidate Patients during the exam (see below). Specify fee associated with service, if applicable. (Some schools provide this service free of charge.) A. Practitioner(s) from the school is NOT AVAILABLE to administer local anesthesia to Candidate Patients. There are no exceptions to this policy. Equipment, Instruments and Expendable Dental Materials: Refer to the respective WREB Candidate Guide for a list of materials and armamentarium that the Candidate must provide. A. Handpiece and Prophy Angle Hookups: The chairs are equipped with a 3-hole, slow speed hand-piece compatible with Midwest RDH hand-pieces and or similar hand-pieces. Carrington College will not be providing slow-speed hand-pieces nor will they be available for rent. The facility will provide disposable prophy angles free of charge to all Candidates. B. Sonic/Ultrasonic Devices: WREB allows the use of sonic/ultrasonic devices as an adjunct to calculus removal. All operatories are equipped with Parkell ultrasonic units for use by any/all Candidates. C. Air/Water Syringe Tips: Air/Water syringe tips are disposable and provided for all Candidates. Carrington College San Jose, CA Page 3 of 10
D. Blood Pressure Cuffs and Stethoscopes: Carrington College does not provide blood pressure cuffs or stethoscopes for Candidate use. E. Expendable Materials: Expendable dental materials supplied by the school are noted in Appendix A. Add the expendable list to your document (items not listed are responsibility of Candidate). Sterilization Services: Are not available onsite. Candidates retaking onsite will need their instruments sterilized prior to retake clinic entrance. 3. Preparing for the Local Anesthesia Exam Equipment, Instruments and Expendable Dental Materials: The Local Anesthesia Exam is not offered at Carrington College. 4. Preparing for the Restorative Exam Equipment, Instruments and Expendable Dental Materials: The Restorative Exam is not offered at Carrington College. 5. During the Clinical Exam Dental Operatory Units: The clinic is equipped with Danseareau chairs and workstations that can accommodate right and/or left-handed Candidates. Infection Control (Surface Asepsis): Disinfect the dental unit to include patient chair, light handles, counter tops, patient tray, tray and hand-piece cart, light box. The clinic follows CDC guidelines and OSHA protocol. Disinfecting wipes are available throughout the clinic. Barrier wraps, barrier tape and barrier sleeves are all available on the clinic floor with other clinical supplies. Instructions for setting up units Prior to and after cleaning and disinfecting all surfaces, wash your hands with antimicrobial soap. Gather all the barriers you need and begin setting up your treatment area. Supplies for the exam will be located on a table outside the compressor room. You may choose to wear exam gloves for setting up your operatory, however, clean hands contacting previously cleaned and disinfected items is acceptable. Place a headrest cover over the chair headrest and a plastic barrier over the air/water, saliva ejector, high volume evacuator, and ultrasonic hand-piece. Cover your tray with a disposable tray barrier prior to placing your disposable supplies. Place a disposable saliva ejector tip and high-volume tip in the hoses. Cover light switch, light handles and chair levers with barrier tape. Place a disposable sleeve over pens and pencils. Cover your clinic supplies with a patient bib. Location of Sharps Disposal Containers: Red sharps containers are located at the end of each operatory throughout the clinic. Clinical Treatment Area Procedure- End of Exam: a. Remember to wear your gloves, safety eyewear and mask for all clean-up activities. b. Remove contaminated surface covers without contaminating the previously disinfected surfaces. Begin with the head rest cover and place the other contaminated items inside, gloves, bibs and other disposables. It should then be disposed of in the large covered garbage containers found throughout the clinic. c. Contaminated needles and used anesthetic cartridges are regulated waste and Carrington College San Jose, CA Page 4 of 10
placed in the Sharps containers. LA protector cards are disposed of in the trash. A. Hazardous Waste Disposal (exam-type specific): o Hygiene: Blood/saliva-soaked waste and any/all disposable patient care items can be deposited in any receptacle at the end of each operatory. B. Sharps Disposal: Sharps containers are located at the end of each operatory. Needles and used cartridges should be disposed of in the Sharps containers. Emergency Medical Equipment: Emergency equipment is located in the Sterilization area at the back entrance to the clinic. A portable oxygen cart, Peritrievers, emergency kit, and a first aid kit are all located in the Sterilization area. An AED is located in the patient reception room. In the event of an emergency, please notify WREB personnel and/or dial 911 from the clinic phone (dial 8, 911 for an outside line). 1. The emergency kit contains; Epinephrine auto-injector, Back-up Epinephrine ampules 1:1000, 1 mg/1ml, Diphenhydramine 50mg/1ml, Albuterol inhaler, Nitroglycerin Spray 0.4 mg/spray, Ammonia inhalants, Oral glucose gel, Aspirin, 2 syringes, CPR mask. 2. Peritrievers, used to retrieve broken instrument tips are available in the drawer in the sterilization area. 3. Two blood pressure cuffs will be available, one standard and one for a larger diameter arm. 4. Phone is located in the clinic office. To access an outside line, dial 8, then 911. 5. An AED is located on the wall in the DH reception area and will be included in the clinic tour. Fire extinguishers: One fire extinguisher is located in the clinic One fire blanket is located in the DH Lab Eyewash Stations: Located in sterilization (bottle of emergency eye wash) MSDS Manual: Located in sterilization Exposures and Exposure Personnel (wear your PPE and handle sharps carefully) 6.
