IMPORTANT INFORMATION: This policy reflects the known requirements for compliance under The Affordable Care Act as passed on March 23, 2010. As additional guidance is forthcoming from the U.S. Department of Health and Human Services, and the New Hampshire Insurance Department, those changes will be incorporated into your health insurance policy. Please Note: This Plan includes a tiered provider network called the ElevateHealth Options Network. In this plan, you will pay different levels of Copayments, or Deductibles depending on the practice or location of the provider delivering Covered Benefits. Tier 1 is made up of ElevateHealth providers. includes all remaining Harvard Pilgrim HMO Providers. If a Provider changes a practice or location at anytime, the tier of that Provider may also change. Please consult your ElevateHealth Options Provider Directory prior to your services to determine the tier placement of your provider or facility. This Schedule of Benefits states any Benefit Limits and amounts you must pay for Covered Benefits. However, it is only a summary of your benefits. Please see your Benefit Handbook for details. In a Medical Emergency you should go to the nearest emergency facility or call 911 or other local emergency access number. A Referral from your PCP is not needed. Your emergency room is listed in the tables below. Clinical Review Criteria We use clinical review criteria to evaluate whether certain services or procedures are Medically Necessary for a Member s care. Members or their practitioners may obtain a copy of our clinical review criteria on our website at http://www.harvardpilgrim.org or by calling 1-888-888-4742 ext. 38723. Copayment Levels There are two types of office Copayments that apply to your Plan: a lower Copayment, known as Level 1 and a higher Copayment known as Level 2. Level 1 applies to covered outpatient professional services from the following types of providers: Primary Care Providers (PCPs); obstetricians and gynecologists; Licensed Mental Health Professionals; certified midwives; and nurse practitioners. Level 2 applies to most outpatient specialty care. If a provider is categorized as both a Level 1 provider and a Level 2 provider, Level 1 applies. For example, if a provider is both a PCP and a cardiologist, you will be responsible for a Level 1 Copayment. Your Plan may have other Copayment amounts. Preferred PCP ELEVATEHEALTH OPTIONS HMO - NEW HAMPSHIRE In addition, the Plan has partnered with certain PCPs to offer lower. These PCPs are known as Preferred PCPs. If you have an office with a Preferred PCP, your office Copayment will be less than if you had an office with a Provider that is not a Preferred PCP. For example, if you have an x-ray or have blood drawn as part of your office, please refer to Laboratory and Radiology Services to determine if additional cost sharing applies to these services.
