To document your training, fill out the following sections and place the training record with your OSHA manual or HIPAA manual as appropriate: 1. Date each training record 2. Name of your office on those records where required 3. Print the name of each employee attending this course on the lines provided on each form. 4. Each employee must sign the HB-300 training form. New Hazard Communication Plan & Rules The New Globally Harmonized Communication Plan was included in last year s handout. If you placed this new plan in your OSHA manual last year, you do not need this new plan. However, if do have this new plan, take the old Hazard Communication Plan from your OSHA Manual and replace it with the new attached Hazard Communication Plan. The New Globally Harmonized Hazard Communication Rules should be placed in your OSHA Manual along with the New Hazard Communication Plan. HIPAA Reminders Training Record The following employees attended a HIPAA Reminders Training session that covered Privacy Rules Reminders and Security Rules reminders. Date: HIPAA Privacy Reminders Never leave the patient s record or any PHI where other people can see it or take it. Do not discuss the patient s PHI in public places where it may be overheard.
