LOCKHART INDEPENDENT SCHOOL DISTRICT Request for Qualifications #14-011 The Lockhart Independent School District ( District ) is seeking Submissions in response to this Request for Qualifications ( RFQ ) for architectural/engineering services related to the following projects in the District's 2014 Bond Program: New Elementary School (w/500 student capacity; expandable to 700 students) Capital Improvements to Various Campuses Submissions shall be clearly marked: DEADLINE FOR SUBMISSIONS: No later than 2:00p.m. on the 7th day of August, 2014. By providing a Submission, each Architect agrees to waive any claim it has or may have against the District, its Trustees, agents and employees, and any reference sources, arising out of or in connection with the administration, evaluation, or recommendation of any Submission; waiver of any requirements in the Request for Qualifications; acceptance or rejection of any Submission and award of a Contract. Introduction The District invites architects and any other qualified professionals ( Architect ) to provide Submissions to provide architectural/engineering services for a new elementary school and capital improvements to various district campuses ( Project ) 1. Background and Scope of Services The Project consists of architectural/engineering services for the following projects: New elementary school with an initial student capacity of 500, with core spaces to support expansion to 700. The site selection process is underway. The District expects the facility to be open for the 2015-2016 school year. The construction budget for this project is approximately $10,000,000. Various capital improvements to existing campuses, including such things as HVAC upgrades, remodel for controlled building entries, site improvements, accessibility upgrades, fire alarms, kitchen improvements, restroom upgrades, etc. Work will be scheduled as appropriate to minimize the impact to students and campus operations. Final completion of all improvements is expected by the start of the 2017-2018 school year. The construction budget for these improvements is approximately $3,200,000. Contents of Submission The Submission shall be concise and complete. The District will require an original and ten (10) copies of the Submission from all interested Architects. The original should not be permanently bound, so that additional copies can be made. The Submission should include: 1. 2. Organization: 2.1 How many years has your organization been in business as an Architecture firm? 2.2 How many years has your organization been in business under its present name? Under what other names has your organization operated? 2.3 If your organization is a corporation, list the following: Date of incorporation; state of incorporation; President s name; Vice-President s name(s); Secretary s name; and Treasurer s name. 2.4 If your organization is a partnership, list the following above, describe it and name the principals. 3. Licensing: 3.1 List the names and registrations of your firm s principals and of those individuals proposed for this engagement. 3.2 Has a member of your team had his or her license suspended for any reason? 4. Experience: 4.1 List the categories of work your organization normally performs with its own staff (i.e.: architectural design, MEP, structural, civil, etc.). 4.2 List any sub-consultants in which your organization has some ownership and list the categories of work those subcontractors normally perform. 4.3 Claims and suits (if the answer to any of the questions below is YES, please attach details.) 4.3.1 Has your organization ever failed to complete any work? 4.3.2 Are there any judgments, claims, arbitration proceedings or suits pending or outstanding against your organization or its officers? 4.3.3 Has your organization filed any lawsuits or requested arbitration with regards to design contracts within the last five years? 4.4 Current Work: List the major projects your organization has in progress, giving the name and location of project, the service you are providing, construction budget, percent complete and scheduled completion date. 4.5 Work over last five years: List the major projects (particularly educational facilities) designed by your organization over the last five years. Specific Central Texas experience in the master planning and design of elementary school projects (minimum capacity of 700 students). Please include any renovations to school facilities as well. For each project, provide the name, nature of the project/function of the building, size (SF), location, original budget vs. final construction cost, completion date, owner and contact name. Emphasis should be placed on educational facility projects. 4.6 Project Specific Information: For at least one, but no more than three, of the projects described in 4.5, provide a concise narrative of lessons learned. Illustrative renderings, floor plans and/or elevations that depict the scope of the work are encouraged. Page 3

5. Financial Information: 5.1 Attach a financial statement, preferably audited, including your organization s latest balance sheet and income statement showing the following items: Current Assets (e.g., cash, joint venture accounts, accounts receivable, notes receivable, accrued income, deposits, materials inventory and prepaid expenses) Net Fixed Assets Other Assets Current Liabilities (e.g., accounts payable, notes payable, accrued expenses, provision for income taxes, advances, accrued salaries and accrued payroll taxes) Other liabilities (e.g., capital, capital stock, authorized and outstanding shares par value, earned surplus and retained earnings) 5.2 Name and address of firm preparing financial statement and date thereof. 5.3 Is the attached financial statement for the identical organization named under item 1? If not, explain the relationship and financial responsibility of the organization whose financial statement is provided (e.g., parent-subsidiary). 5.4 Will the organization whose financial statement is attached act as guarantor of the contract for design? 5.5 Provide name, address and phone number of bank reference. 5.6 Professional and General Liability Carrier. Attach certificate(s). 6. Personnel: Identify the specific individuals proposed for this engagement. Provide a resume for individuals and a discussion of their proposed responsibilities. Address specific project design experience with new elementary school projects and renovation projects of existing schools as discussed in 4.5 above. 7. Owner/Architect Agreement: The Architect should be prepared to sign AIA Document B101-2007, Standard Form of Agreement Between Owner and Architect, as modified by the District, attached as Exhibit A. 8. References: For the projects listed above in Item 4.5 identify a representative of the owner and a representative of the contractor (provide name, phone, and email) whom we could reference regarding your organization's services. Ideally the references should be for educational projects of comparable scope. Method of Selection The District will first select the most highly qualified Architect(s) based on demonstrated competence and qualifications, and then attempt to negotiate with the Architect(s) a fair and reasonable fee. TEX. GOV T CODE 2254.004. Page 4

Texas Administrative Code, Title 22, Rule 1.147 provides: An Architect may submit information related to the monetary cost of a professional service, including information found in a fee schedule, only after the governmental entity has selected the Architect on the basis of demonstrated competence and qualifications pursuant to the Professional Services Procurement Act. An advisory selection committee will be established to review the submittals and interviews with selected firms will be scheduled for a date in October to be determined. One or more Architects will be interviewed by the Board. The District reserves the right to select one or more Architect for performance of the work. The Submissions will be evaluated and ranked by the Board on the basis of the following criteria: 1. Experience with projects which include public schools of this type (K-12). 2. Personnel/Resources. 3. References. 4. Interview (if selected for interview). There should be no contact with the Board. The submittal of any firm contacting a Board Member may be rejected. Page 5

FELONY CONVICTION NOTIFICATION Texas Education Code, Section 44.034, Notification of Criminal History, Subsection (a), states a person or business entity that enters into a contract with a school district must give advance notice to the District if the person or an owner or operator of the business entity has been convicted of a felony. The notice must include a general description of the conduct resulting in the conviction of a felony. Subsection (b) states a school district may terminate a contract with a person or business entity if the District determines that the person or business entity failed to give notice as required by Subsection (a) or misrepresented the conduct resulting in the conviction. The District must compensate the person or business entity for services performed before the termination of the contract. This notice is not required of a Publicly-Held Corporation. CONFLICT OF INTEREST QUESTIONNAIRE For vendor or other person doing business with local governmental entity This questionnaire reflects changes made to the law by H.B. 1491, 80th Leg., Regular Session.
