CERTIFICATION AND AGREEMENT I certify that all information given on this application is true and correct. Pre Employment Drug Testing Release Champlain Enterprises, Inc. dba CommutAir is required by FAA Regulations to conduct DOT/FAA regulated preemployment drug testing for employees who perform the following safety sensitive functions: Flight Crewmember, Flight Attendant, Flight Instruction, Aircraft Dispatcher, Aircraft Maintenance or Preventative Maintenance, Flight Instruction and Ground Security Coordinator. Applicants for any other job functions are also subject to pre employment drug testing as outlined in CommutAir s Drug and Alcohol Program Policy. I understand that I will undergo a urine drug screening test for the following drugs: Marijuana, Cocaine, Opiates, Amphetamines, Ecstasy (MDMA) and Phencyclidine (PCP) I authorize the submission of the results to CommutAir. A verified positive test result, a verified adulterated specimen as well as a refusal to test will result in immediate withdrawal of the offer of employment. I understand that as an employee of CommutAir I will be subject to random, post accident, reasonable cause, return to duty and follow up drug testing as outlined in the company s Drug and Alcohol Misuse and Prevention Policy and FAA guidelines.
