State the PHA s Mission for serving the needs of low-income, very low-income, and extremely low income families in the PHA s jurisdiction for the next five years: The mission of our agency is the same as that of the Department of Housing and Urban Development: To promote adequate and affordable housing, economic opportunity and a suitable living environment free from discrimination. 5.2 Goals and Objectives. Identify the PHA s quantifiable goals and objectives that will enable the PHA to serve the needs of low-income and very low-income, and extremely low-income families for the next five years. Include a report on the progress the PHA has made in meeting the goals and objectives described in the previous 5-Year Plan. Goal: Leverage private or public funds to create additional housing opportunities (acquire or build units or developments) The Authority continues to explore opportunities to develop additional housing through the low income housing tax credit program Goal: Improve public housing management (PHAS) score: The Authority has achieved high performer status for the past two years. We will continue to strive to maintain our high level of performance. Goal: Designate development for particular resident groups: We have renewed the designation of Stevens Towers as an elderly development in 2014. We will continue to monitor the need for any other necessary designations. Goal: Increase the number and percentage of employed persons in assisted families: As of January 20, 2015 93% of all income received by public housing families came from earned income, social security or pensions. Only 6% of income came from TANF or some other form of public welfare. The data clearly demonstrates our resident s success in obtaining employment. Goal: Participate in HUD s goal to end homelessness: The Authority has obtained 130 VASH vouchers specifically designated to assist the homeless. In addition, the Authority has obtained funding through the Continuum of Care to provide housing to seriously mentally ill homeless individuals. Goal: Undertake affirmative measures to ensure access to assisted housing regardless of race, color, religion, national origin, sex, familial status and disability. The Authority ensures equal access to all of its housing programs. For example in Lebanon County 2.2% of the population is African American and 9.3% identifies as Hispanic. In the Authority s housing programs 10% of participants are African American and 49% are Hispanic. This data clearly demonstrates that the Authority has succeeded in providing access to all citizens of Lebanon County.

PHA Plan Update (a) Identify all PHA Plan elements that have been revised by the PHA since its last Annual Plan submission: 6.0 Element 1: The Public Housing Admissions and Occupancy Policy is being revised to include the new Pennsylvania state mandated veterans preference. Element 2: The Public Housing Admissions an Occupancy Policy was revised effective January 1, 2015 to comply with revised HUD regulations and Fair Market Rents published in October 2014. There have been no changes to the other elements of the previous Annual Plan. (b) Identify the specific location(s) where the public may obtain copies of the 5-Year and Annual PHA Plan. For a complete list of PHA Plan elements, see Section 6.0 of the instructions. All supporting documents are available at the Housing Authority s main administrative office, 7.0 Hope VI, Mixed Finance Modernization or Development, Demolition and/or Disposition, Conversion of Public Housing, Homeownership Programs, and Project-based Vouchers. Include statements related to these programs as applicable. There have been no changes to the previous annual plan for any of these items. 8.0 8.1 Capital Improvements. Please complete Parts 8.1 through 8.3, as applicable. See Attached Capital Fund Program Annual Statement/Performance and Evaluation Report. As part of the PHA 5-Year and Annual Plan, annually complete and submit the Capital Fund Program Annual Statement/Performance and Evaluation Report, form HUD-50075.1, for each current and open CFP grant and CFFP financing. See Attached 8.2 Capital Fund Program Five-Year Action Plan. As part of the submission of the Annual Plan, PHAs must complete and submit the Capital Fund Program Five-Year Action Plan, form HUD-50075.2, and subsequent annual updates (on a rolling basis, e.g., drop current year, and add latest year for a five year period). Large capital items must be included in the Five-Year Action Plan. See Attached 8.3 Capital Fund Financing Program (CFFP). Check if the PHA proposes to use any portion of its Capital Fund Program (CFP)/Replacement Housing Factor (RHF) to repay debt incurred to finance capital improvements. HUD approval on file. 9.0 Housing Needs. Based on information provided by the applicable Consolidated Plan, information provided by HUD, and other generally available data, make a reasonable effort to identify the housing needs of the low-income, very low-income, and extremely low-income families who reside in the jurisdiction served by the PHA, including elderly families, families with disabilities, and households of various races and ethnic groups, and other families who are on the public housing and Section 8 tenant-based assistance waiting lists. The identification of housing needs must address issues of affordability, supply, quality, accessibility, size of units, and location. See Attached 9.1 Strategy for Addressing Housing Needs. Provide a brief description of the PHA s strategy for addressing the housing needs of families in the jurisdiction and on the waiting list in the upcoming year. Note: Small, Section 8 only, and High Performing PHAs complete only for Annual Plan submission with the 5-Year Plan. Leverage private or other public funds to build/acquire units or developments; Renovate or modernize public housing units; Undertake affirmative measures to ensure accessible housing to persons with all varieties of disabilities regardless of unit size required.

Additional Information. Describe the following, as well as any additional information HUD has requested. (a) Progress in Meeting Mission and Goals. Provide a brief statement of the PHA s progress in meeting the mission and goals described in the 5- Year Plan. See goal update in section 5.2 10.0 (b) Significant Amendment and Substantial Deviation/Modification. Provide the PHA s definition of significant amendment and substantial deviation/modification A substantial deviation or significant amendment or modification, which requires public notice and comment, will be required if a change is made to the Plan or any component thereof, related to the following: Demolition or conversion of units; Implementation of a homeownership program; Additions of non-emergency work items with an estimated cost greater than 50% of the Authority s annual Comprehensive Grant (items not included in the current Annual Statement or 5 Year Action Plan). Development of new Public Housing or any mixed finance development activities. Changes made to the Plan, or any component thereof, which do not relate to the above mentioned issues will not be considered substantial or significant and will not require public notice and comment. 11.0 Required Submission for HUD Field Office Review. In addition to the PHA Plan template (HUD-50075), PHAs must submit the following documents. Items (a) through (g) may be submitted with signature by mail or electronically with scanned signatures, but electronic submission is encouraged. Items (h) through (i) must be attached electronically with the PHA Plan. Note: Faxed copies of these documents will not be accepted by the Field Office. Plan Comments

Resident Advisory Board Meeting Summary March 24, 2015 3:00 P.M. During a meeting of the Lebanon County Housing Authority s Resident Advisory Board held on March 23, 2015 at 3:00 p.m. at Stevens Towers, the proposed Annual Agency Plan for Fiscal Year 2015 was reviewed with those in attendance, as follows: RAB Members: Rose Snyder, Gary Snyder, Juan Rivera, Pablo Hernandez, Randall Poole, Gary Lowe, Sr., Beth Bates, Kathy Flory, Madeline Amill, Henrietta Treadwell, Nancy Clay Housing Authority Staff: Bryan Hoffman, Sue Bowman, Susan Galbraith, Cindy Shumaker Guests: David Desper, Stevens Towers resident 1. As background to the Agency Plan discussions, Mr. Hoffman gave a video presentation using charts showing the Federal budget allocations for public housing and related subsidy gains or losses. He pointed out that utilities are the Housing Authority s single largest expenditure. In response to a question from a RAB member about the possible use of solar panels, Mr. Hoffman pointed out that the Housing Authority is not able to use any of its reserves to pay for capital improvements. Mr. Hoffman went on to note that, since the Authority s capital needs far outstrip the available funding, we have to choose our most urgent needs. Thus, we have been concentrating on roof replacements for the past several years, as well as for the next few years. He then went on to provide an overview of the Agency Plan, with emphasis on the proposed Capital Fund Program expenditures. It was noted that the first $200,000 of each year s CFP spending currently goes to debt reduction on the loan used to completely renovate Stevens Towers back in 2005; another $40,000 per year is allocated to administrative costs, while fees and design costs run about $25,000 per year. The 2015 funding figure for LCHA is $567,582, or about $5,000 more than the funding for 2014. Mr. Hoffman explained that HUD requires that same number to be used for estimating CFP budget figures for the upcoming four years. It was also pointed out that, under project-based management, capital improvements funding is done on a project-byproject basis, rather than on an Authority-wide basis. 2. Ms. Bowman stated that HUD s change in the way flat rents are computed has increased these figures significantly for most bedroom sizes; thus an update is needed to Chapter 11 of the Authority s Admissions and Occupancy Policy to give an added priority to any overhoused or underhoused household for which a

transfer to an appropriately sized unit would lessen their flat rent burden. Additionally, given HUD s emphasis on transferring overhoused families to free up those units for new and potentially higher-paying tenants, the weighting on the transfer list is now being changed so that overhoused and underhoused families are both given equal points in the rating system. 3. A Stevens Towers resident asked about the possibility of providing WiFi for residents of that building. Mr. Hoffman explained that doing so would require more bandwidth from the Authority s Internet provider at an added cost. Residents then asked whether they could pay an access fee to help defray that cost; Mr. Hoffman explained that the Authority is precluded from charging a fee for such services. 4. Mr. Hoffman noted that that Housing Authority s Section 8 waiting list has been closed since July 2010; however, the Authority is currently updating the applicants remaining on that list and foresees possibly reopening the Section 8 waiting list, at least for some period of time, later in 2015. 5. Mr. Hoffman added that the Authority will also be updating its public housing waiting lists in order to obtain information about households Veteran s status in order to comply with a new Veteran s preference law enacted by the Pennsylvania legislature in late 2014. 6. Mr. Hoffman encouraged RAB members to stay abreast of the Housing Authority s activities and/or to review documents like the Agency Plan and Admissions and Occupancy Policy on-line at the website www.lebanoncountyhousing.com. 7. It was the consensus of the RAB membership that, given the current budgetary restrictions, the 2015 Agency Plan meets with their approval.

MINUTES OF PUBLIC HEARING Lebanon, Pennsylvania April 8, 2015 A public hearing on the 2015-2019 annual Agency Plan for the Housing Authority of the County of Lebanon was held at 1:45 p.m. in the Community Room of Stevens Towers, 930 Willow Street, Lebanon, PA. There were present: Housing Authority members: George Kaufman Neva Luttman Amy Leonard Walter Zehring Staff: Bryan Hoffman, Sue Bowman and Karen Raugh of Lebanon County Housing Authority; Elizabeth Bowman and Daniel Lyons, Lebanon County Redevelopment Authority 1. Housing Authority Executive Director Bryan Hoffman called the public hearing to order at 1:50 p.m. There were no members of the public present. 2. Mr. Hoffman reviewed that the hearing had been properly advertised in the Lebanon Daily News, all residents of public housing had received a notice about the hearing, and the hearing notice had been posted at all Housing Authority offices, as well as on the Authority s website. He pointed out that a meeting of the Authority s Resident Advisory Board had been held on March 24, 2015 with 12 program participants in attendance; no negative comments were received at that time. 3. Mr. Hoffman noted that he had received no written comments about the annual Agency Plan from any Housing Authority stakeholder nor any member of the public. He added that both the County of Lebanon and the City of Lebanon had provided their signed statements that the Housing Authority s 2015 Agency Plan is consistent with their respective Consolidated Plans. 4. Housing Authority resident board member, Neva Luttman, indicated that she is sometimes approached about how the Housing Authority spends the money it receives. Mr. Hoffman responded that two-thirds of that money goers towards the payment of utilities and assorted maintenance contracts and without subsidy from HUD, it would be impossible to balance the Authority s budgets. Mr.

Zehring suggested that the Housing Authority post information indicating to residents how the money from subsidies and rents is spent. 5. There being no further questions or comments received at this time, the public hearing was adjourned at 1:57 p.m.

Public Housing Admissions and Occupancy Policy List of Modifications January 2015 Cover Updated to reflect revision 1.14 to the plan Table of Contents Updated to reflect repagination of some sections Chapter 5 Section B Local Preferences Add a new item 7 as follows: 7. Veterans Preference Applicants who are an active duty member of the United States armed forces or who have received an honorable or general discharge from such forces shall receive one point. This preference shall extend to the household of which the service member or veteran is a member. In addition, the surviving household members of a deceased service member or veteran who died of service connected causes shall receive one point, subject to the following conditions: The service members or veterans death occurred during active duty service or within five years of discharge from the service; and The death occurred not more than five years prior to the data of application for housing; and The applicant claiming the preference must be the spouse or minor child of the deceased service member or deceased veteran. Points awarded for the above listed preferences shall be cumulative. Order of applicant selection among applicants with the same number of preference points shall be based on the date and time of the application. Table 1 on the following page provides a matrix of the various cumulative values of the preferences listed above.

Designated Housing

DESIGNATION OF STEVENS TOWERS/WASHINGTON ARMS AS ELDERLY ONLY DEVELOPMENTS Approved by HUD November 9, 2005 Designation of Public Housing Housing Authority of the County of Lebanon Page 1

I. PLAN CONTENT The Housing Authority of the County of Lebanon (HACL) requests HUD s approval to designate its two high-rise buildings for individuals and households that are 50 years of age or older. The following is basic information on these two highrises: Stevens Towers (PA 52-1B) is a high rise constructed in 1970. It is currently undergoing a major renovation, which will take it from a total of 100 units down to 80. This will be achieved by combining 40 smaller units into 20 larger one bedroom apartments. Following its remodeling, Steven Towers will have 40 standard one bedroom apartments, 20 standard efficiency apartments, and 20 large one bedroom apartments. As a result of the remodeling, the number of accessible units will increase from two to seven. Stevens Towers is located near historic downtown Lebanon. It is located close to churches, the community library, farmer s market, banks, and the hospital. Washington Arms (PA-52-3) opened in 1976. It is a 60 unit, 4-story building that includes 5 two bedroom apartments (1 of which is handicap accessible), 25 one bedroom apartments (of which 2 are handicap accessible) and 30 efficiencies. It is located within one block of the county s only hospital and two blocks from a rehabilitation facility. It is near the central business area. The Authority s main offices are located in this building. Both buildings have electronically controlled entrances to a single building. The entrances and the common areas are fully accessible. The common areas enable the residents to participate in group activities. As discussed further in the following sections, the Authority proposes to expand the number of accessible apartments in another site and to open its Voucher waiting list to disabled persons. The Authority will also apply for additional Housing Choice Vouchers if such become available. All residents of public housing and Voucher participants have access to the Authority s social services department. One case worker specializes in senior clients only, while another caseworker handles non-elderly tenants. For younger residents with disabilities, the social services provided generally involve the provision of information and referrals to appropriate community agencies. These services will be available to those who are not able to obtain apartments in the two high rises as the result of the designation of these buildings for those 50 years of age and over. As demonstrated on the following chart, 63 of the residents of Stevens Towers are over 70 including 31 percent who are over 80.
